[The chronic action of lead on the vascular system: the problem of ecological pathology].

Problem of chronic effects of the lead (L) on the cardiovascular system is presented from the viewpoint of the ecological pathology. The review summarizes: sources of L in the environment, industries in which L is widely used in the technological process, occupations in which L intoxication is possible, routes of L penetration into the organism and the organs of its accumulation, mechanisms and variants of chronic L effects on the vascular system. In case of chronic L toxicity the vessels of various organs and system are affected; the pathologic process is of a vasculitis or arteriosclerosis type and is predominantly observed in the arteries of small and mean caliber. The mechanisms of vascular L effect is not sufficiently studied.

[Heart rupture as a complication of myocardial infarct].

The main causes of sudden death of patients suffering myocardial infarction are described. The authors established based on autopsy data the frequency, location, time of development, causes of possible pathogenetic mechanisms and morphology of ruptures of the heart in acute myocardial infarction.

[Pathomorphology and morphogenesis of obliterating endarteritis].

The author has examined the pathomorphological arterial changes in 37 amputated legs of patients with obliterating endarteritis. He has studied the principal morphogenetic stages of the disease. Obliterating endarteritis was shown to have an inflammatory origin. Its morphogenesis was found to be characterized by the main stages appearing in the following order: fibrinoid swelling and necrosis of the intima; lymphoid-macrophagal infiltration of the intima (sometimes with presence of giant multinuclear cells) involving the media; intimal hyperplasia and thrombosis occluding the vascular lumen; sclerosis of the vascular wall. At different levels of the amputated limb one could find different stages of the pathologic process due to the undulating character of the disease. The distal sites of the major arteries (mainly located in the lower third of the crus) were the first to undergo changes, and the disease progressed proximally.

[Method of specimen collection for histological examinations in obliterating vascular diseases of the lower limbs].

The proposed method is based on the use of cross circular cuts. Magistral blood vessels are taken for histological examinations in standard sites in one block with the surrounding muscles in which changes in small vessels are looked for. By this method the condition of the vascular system of the extremity may be studied at different levels with minimal traumatization of the vessels and their pathological formations, the time of specimen collection is shortened, and multifactorial statistical analysis may be done.

[Automated description of pathohistological changes in the blood vessels: creation of the bulk of original data].

An algorithm of systematized description of histopathological changes in blood vessels has been developed for introduction of the data into a computer memory. The work of a physician with the computer in the dialogue regimen through a display is described. The list of histopathological changes in the vessels proposed may be used also in other methods of registration (questionnaires, systematized cards).

[Pathohistologic and ultrastructural features of drug-induced membranous glomerulonephritis].

Nephrobiopsies of 20 patients with drug-induced membranous glomerulonephritis were studied. Renal injury was produced by diuretics (7 patients), gold salts (6), penicillamine (4), captopril (3). Drug-induced membranous glomerulonephritis in contrast to idiopathic one ran a benign course after proper therapy based on discontinuation of the drug. Main pathomorphological features of secondary membranous glomerulonephritis that help to differentiate it from the primary one were established: absence of total injury of the glomeruli, hypercellularity of the damaged glomeruli, pathomorphological changes only of the part of the capillary loops, presence of both subepithelial and mesangial deposits.

[Lead--an etiologic factor in vascular disease: main evidence (lecture)].

Review of the literature showing that lead is one of the most potent etiological stimuli in the development of the characteristic vascular pathology (vasculitis, atherosclerosis, arteriosclerosis, arterial hypertension). The attempt is made to summarise sparse data on the routes and mechanisms of the lead vascular damage pathogenesis.
