Delivery following colon interposition.

BACKGROUND: Colon interposition carries a significant complication rate due to attenuated arterial blood supply, because the interposed colon segment depends on a single vascular pedicle. CASES: We report for the first time five vaginal deliveries in two women following the operation, illustrating the dilemmas encountered in choosing the delivery method in these patients. CONCLUSION: We suggest that vaginal delivery following colon interposition is feasible. If cesarean section has to be performed, extra care must be exercised not to damage the vascular pedicle, particularly when there is a need for mobilizing or palpating the posterior aspect of the uterus.

Over-the-counter oral analgesia for postcesarean pain.

OBJECTIVE: The purpose of this study was to evaluate patient satisfaction for oral analgesia for postcesarean pain management. STUDY DESIGN: A prospective study was conducted on women who underwent cesarean section with regional analgesia. Women were treated with ibuprofen 400 mg in liquid-filled capsules on the first postoperative day by two modes of drug administration: on patient demand (n = 60) and at predetermined regular intervals (n = 54). Pain intensity and satisfaction were self-evaluated by the patients with use of a visual analog scale. RESULTS: Patients in the fixed time interval group had lower pain scores (by 38%-54%, P <.001) compared with the on-demand group. Satisfaction scores in both groups were high but superior in the fixed time interval group (87.5 +/- 18.8, median 94.5, vs 78.6 +/- 21, median 85, P <.001). CONCLUSIONS: Oral, nonnarcotic, postcesarean analgesia provides satisfactory pain relief. Patient satisfaction can be further enhanced by providing the medications in fixed time intervals rather than leaving it to patient request.

The effect of meperidine and promethazine on fetal heart rate indices during the active phase of labor.

BACKGROUND: Visual interpretation of fetal heart rare monitoring is subject to intra- and inter-observer variability. OBJECTIVE: To examine the effect of intrapartum administration of meperidine and promethazine on fetal heart activity measured objectively by a computerized system. METHODS: Fourteen healthy women with normal pregnancies at term were studied during the active phase of labor. Fetal heart rate was recorded with the Oxford Sonicaid system 8000. Recordings were performed for 40 minutes prior to and after maternal intravenous administration of meperidine 50 mg with promethazine 25 mg. RESULTS: The combination of meperidine and promethazine caused a significant decrease in the number of accelerations of 10 beats per minute (9.7 versus 2.6, P = 0.002) and 15 beats per minute (5.2 vs. 1.4, P = 0.003), time spent in episodes of high variation (14.8 vs. 2.0, P = 0.005) and short-term variation (7.8 vs. 5.0, P = 0.003). On the other hand there was an increase in the time spent in episodes of low variation (5.3 vs. 19.7, P = 0.009). CONCLUSIONS: Maternal administration of meperidine with promethazine has a significant effect on FHR indices during the active phase of normal labor.

[The pine processionary caterpillar Thaumetopoea pityocampa].

The pine processionary caterpillar Thaumetopoea pityocampa (Lepidoptera: Notodontidae) is considered to be a serious pest of medical importance. The hair on the dorsum of the last instar larvae of the moth may cause urticarial reactions (erucism) as well as eye problems and temporary blindness. In Israel, the pest occurs in all pine plantations as well as on ornamental pine trees in urban areas. The biology, ecology and management of the moth population are discussed as well as the mechanism of action of the urticarial hairs and their medical significance. Awareness of the life cycle and ecology of the pest may reduce the contact of the population with the urticarial hairs and prevent the morbidity caused by it.

Psychological distress symptoms in women undergoing medical vs. surgical termination of pregnancy.

OBJECTIVES: The purpose of this study is to compare the baseline psychological distress and symptom profile of women undergoing either medical (with mifepristone) or surgical pregnancy termination and the psychological outcome 2 weeks after the procedure. METHODS: Women (n = 200) given free choice of pregnancy termination method, either medical or surgical, were assessed before pregnancy termination by a demographic questionnaire including questions regarding the choice of the method of pregnancy termination, the Brief Symptom Inventory (BSI), the Spielberger State Anxiety questionnaire and the Rotter Locus of Control Scale. Two weeks after the procedure, the BSI and Spielberger questionnaires were repeated. RESULTS: Women with a smaller number of past pregnancies tended to choose the medical procedure. Reasons for choosing the medical procedure were fear of surgery, anesthesia and of future fertility difficulties. Prior to the procedure, the "medical group" had significantly higher levels of obsessive-compulsive symptoms, guilt and BSI general symptom index score, and a trend for higher interpersonal sensitivity and paranoid ideation. Postprocedure, both groups showed significant decline in anxiety levels and did not differ on most symptom parameters. CONCLUSIONS: Women who chose to have a medical termination are marginally more symptomatic before the procedure than women choosing surgical termination. However, both methods of pregnancy termination resulted in significant reduction in preabortion psychological distress level.

In utero diagnosis of bladder perforation with urinary ascites. A case report.

Prenatal sonographic diagnosis of fetal obstructive uropathy is relatively common. However, the complication of spontaneous rupture of the obstructed bladder in utero causing fetal urinary ascites is extremely rare. This case report describes an unusual case of congenital bladder perforation and urinary ascites diagnosed in utero. The pertinent literature is reviewed.
