[The effect of Lendormin (brotizolam) on the sleep structure and subjective assessment of sleep in insomnia patients].

The influence of Lendormine (diazepine derivative) both on the sleep structure and subjective estimation of sleep was examined in 25 patients at the age of 19-64 years old with neurotic insomnia. The investigation was performed before, during and after 10 days of treatment (0.25 mg before the sleep time). The drug administration resulted in significant improvement of sleep structure that is in sleep duration and efficiency because the drug either promoted falling asleep or increased the second and the forth stages of sleep. Lendormine decreased also the wakening time inside the sleep. The polysomnographic results coincided completely with subjective estimation of sleep by patients themselves after the treatment. The optimal time of therapy didn't exceed 2 weeks. The drug was well tolerated without any complications. The conclusion is made about high effectiveness of Lendormine in the treatment of patients with insomnia.

[The phototherapy of parkinsonism patients].

40 patients with idiopathic parkinsonism were treated by artificial white light (intensity--3300 lux) together with total or partial "drugs holidays". The patients with newly made diagnosis "parkinsonism" were treated too. Both before and after light therapy (LT) (10 light exposures) we performed estimation of the main clinical manifestations of the disease (in rating scales), psychological assessment (Back's depression rating scale), examination of motility (authors' own special computer program "Hand's Movement Test for Windows"). It was observed that LT resulted in a decrease of the severity of either rigidity or bradykinesia (but not the tremor) as well as it also decreased the depression gravity and improved the motor functions. LT facilitated the patients' capacity to tolerate "the drugs holidays". LT permitted to combine it with drug therapy as well as LT decreased the complications of DOPA-containing drugs (on-off-effects and dystonic hyperkinesias). LT may be used as monotherapy in patients with initial manifestations of the disease. LT didn't cause any complications and was quite simple in carrying out. The conclusion is made about the perspectivity of LT application in parkinsonism treatment as important non-drug therapy.

[Effects of sleep deprivation in patients with parkinsonism].

The paper is concerned with a pilot detailed study of a single 24-h sleep deprivation (SD) in parkinsonism patients (n-35). The score estimation of the disease clinical signs demonstrated diminution of rigidity, bradykinesia, gait defects as well as of the total score of the patients condition severity. The clinical observations were verified by computer processing of motor tests. SD proved antidepressant in parkinsonism patients. The combination of drug treatment with SD permits a 2-fold and 50% decrease in the treatment duration and daily dose, respectively. A test is proposed for assessing tonicity of facial muscles.

[Sleep deprivation and brain pathology].

The paper presents a brief review of literature data on clinical effects of sleep deprivation (SD), short-term deprivation (up to 40 h), in particular. SD was studied for effects on patients with defected cerebral dopaminergic systems (parkinsonism, dystonic hyperkinesis, persistent lactorrhea-amenorrhea, acromegaly). The control was made using clinical, electrophysiological, psychological, motor and biochemical tests. SD was compared to L-Dopa, parlodel, lisenil action. SD is shown to have therapeutic and differential diagnostic significance in varying pathological conditions of cerebral dopaminergic systems.

[Autonomic disorders in Parkinson disease].

The authors provide the clinical and laboratory findings regarding the vegetative nervous system in 60 patients with parkinsonism and in 11 healthy volunteers. It has been found that parkinsonism is necessarily associated with vegetative disorders which are pronounced to a greater degree in right-handed patients and bilateral clinical manifestations of the syndrome.

[Lateralized forms of parkinsonism (clinical, electroencephalographic and psychomotor aspects)].

As many as 101 parkinsonism patients were examined. The mean patients' age was 50 years. Of these, 46 patients (12 with left-handed clinical manifestations of parkinsonism, 17 with right-handed, 17 with bilateral ones) were subjected to comprehensive examinations -- clinical, electrophysiological and psychomotor. Ten healthy persons of the respective age and sex made up the control group. The examination made it possible to reveal the clinical and electrophysiological characteristics of the different patterns of parkinsonism. At the same time it allowed the conclusion about the involvement into the pathological process of both hemispheres whatever the primary lateralization of the clinical manifestations; about varying roles played by the right and left hemispheres in the pathogenesis of parkinsonism.

[Psychosomatic and humoral correlations of persons in late middle age].

Comprehensive examination of psychoemotional personality characteristics, electropolygraphic examination during night sleep and in various states of day-time wakefulness, as well as determination of the circadian rhythm of urinary catecholamine excretion were carried out in elderly patients versus control subjects. A correlational analysis of the findings obtained revealed certain changes of the studied indicators in the elderly patients. Nocturnal sleep was associated with elevated functional activity of the ascending activating system of the brain and with increased noradrenaline excretion. In day-time wakefulness the dynamics of physiologic, psychologic and biochemical changes were of the unidirectional nature: a decrease in reactivity of alpha-index was logically combined with decrease in reactive anxiety and in noradrenaline and dopamine secretion.

[Relation between clinico-physiologic indices of sleep and the nature of stress and reflexotherapy application].

The time-course of the structure of nocturnal sleep in normal subjects in conditions of a single sleep deprivation (acute sleep deprivation) and in neurotic patients with chronic sleep deprivation has been studied. The efficacy of acupuncture under different conditions is analyzed. The results obtained suggest a differential nature of the acupuncture effect in conditions of acute and chronic sleep deprivation. In acute sleep deprivation in normal subjects electrical acupuncture activates both synchronizing and desynchronizing mechanisms of the cerebral nonspecific systems. In chronic disturbances of sleep associated with neuroses acupuncture activates the synchronizing systems and inhibits the cerebral deactivating systems. Preventive acupuncture is recommended as a method increasing the activity of people working on night shifts (operators, traffic controllers, etc.).

[Features of the polygraphic picture of natural sleep in patients with subacute sclerosing panencephalitis].

The results of electropolygraphic examinations of 6 patients suffering from subacute sclerosing panencephalitis are presented. The polygraphic examination of sleep revealed a pronounced disorganization of the latter, the degree of which depended on the disease duration (the EEG of awakeness was abnormal, too). An importance of electromyographic changes (within the polygram) for differentiating the states of the sleep and awakeness is demonstrated. The sleep duration, the frequency of Radermaker's complexes, and the heartbeat rate were analyzed. An inverse relationship between the latter two parameters was revealed. A prognostic significance of somnological examinations is demonstrated: a particular importance in this respect is attached to the state of vegetative functions.

[Sleep and sedation: clinical and electrophysiological parallels].

Clinical and electrophysiological studies were performed in 10 volunteers and 45 patients with Classes I and II, as stated by the American Society of Anesthesiologists, to evaluate consciousness during sedation. Prior studies on volunteers showed that there was a clear correlation between N-REM sleep and SEF in 90% of cases. Based on these findings, the authors propose a modified suppressed consciousness scale whose rating was adjusted to the levels of N-REM sleep. Irrespective of the type and technique of sedation (infusion of thiopental and propofol; PCS, midazolam), changes in the levels of drug-depressed consciousness were found to be the same.

[Characteristics of the functioning of the human hypophyseal-gonadal system during 1-time sleep deprivation].

The authors studied indices of the activity of the hypophyseogonadal system under the conditions of single sleep deprivation of 24-36 h duration and in the period of recovery. Eleven healthy men aged 21 to 26 were recruited in the study. Two series of investigations were performed: I--the examinees were studied before, after 24 h sleep deprivation and after the first recovery night; II--the scheme of investigation was identical but it included electroauriculopuncture 5-6 h after the start of sleep deprivation. The 24 h sleep deprivation was accompanied by an increase in ACTH and LH secretion, a decrease in the levels of STH, prolactin, total androgens and progesterone in the examinees' blood. On the whole, electroauriculopuncture optimized the activity of the hypophyseogonadal system expressed in lesser changes in the secretion of the above hormones in response to sleep deprivation.
