This work represents a special card for finding and expiration documentation in the prosthodonic field, which is subdivided in anamnese, status praesens, definite treatment and recallsystem. The data processing by personal-computer is possible because of the completion with a ledge for the data recording.
The Helkimo index was used on 130 subjects to identify dysfunctions of the stomatognathic system. There were no significant discrepancies between the results obtained by two investigators except in the case of occlusal interference.
