Ethane exhalation and vitamin E/ubiquinol status as markers of lipid peroxidation in ferrocene iron-loaded rats.

Organ damage caused by iron overload has been mostly attributed to iron-induced peroxidation of membrane lipids. Using the ferrocene iron-loaded rat model, we studied ethane exhalation as a direct marker of in vivo lipid peroxidation, as well as concentrations of alpha-tocopherol and ubiquinol 9/10 in liver and plasma as indirect markers of this process. The feeding of a diet enriched with 0.5% TMH-ferrocene up to 31 weeks resulted in a large increase in liver iron concentration to about 25 mg/g wet weight (w wt). At lower, predominantly hepatocellular liver siderosis, the breath ethane exhalation was dependent on dietary vitamin E (VitE) supplements (onset of ethane exhalation at liver-Fe > 2 mg/g w wt on vitE-restricted diet; > 5 mg Fe per gram on VitE-replete diet). At severe liver siderosis, breath ethane exhalation reached a maximum of approximately 8 nmol/kg/hr independent of VitE supplementation. Plasma as well as hepatic alpha-tocopherol decreased with progressive iron loading. In addition, a significant depletion in hepatic ubiquinol 9 and 10 was noted.

[Health policy goals in child and adolescent health reporting--evaluation with reference to an intermediate-term evaluation].

As a result of the first report on the health of children in Hamburg (1990) 14 primary health policy objectives were identified and related to specific intervention fields. The health information system group at Hamburg has prepared an interim evaluative statement for 1994 on whether and how far each objective has been achieved compared to the targets formulated for the years 1995 and 2000 respectively. 8 out of 14 objectives are considered remaining unchanged as before (i.e. unintentional conception), either proceeding towards the target (i.e. low birth weight, dental health) or having reached the target for 1995 (i.e. infant mortality, age-appropriate vaccination). With 4 objectives the trend deteriorated (i.e. body weight of teenagers, accidents). One objective was monitored on the basis of a new data source (violence against children) and for one objective information could not be proved or updated (breast feeding). In traditional areas of local health policy (i.e. dental health, infant mortality) the health information system and the health targets may be sufficient to help improve the health of the city. In non-lobbied fields (i.e. breast feeding) or those without a consensus on responsibilities (i.e. violence, health and social discrimination) health reports on their own do not invoke achievements, but need to be supported by additional measures.

[Use of a new fixation device--the intraosseal compression loop --in the management of articular (osteochondral) fractures of the hand].

Author describes a new method, the intraosseal compression loop, unknown until now in this country for the treatment of intraarticular, osteochondral fractures. The advantages of the new method are analysed, attention is called however to the fact, that it can be used only in the therapy of certain types of fractures and it can be only one from the great variety of procedures for the treatment of osteochondrial fractures. It is demonstrated with a few examples that the method can be used to treat unstable diaphyseal fractures too.

Cefodizime stimulates subpopulations of cells mediating spontaneous or antibody-dependent cytotoxicity in patients with bacterial infections.

The influence of cefodizime (CDZ) on spontaneous cell-mediated cytotoxicity (SCMC) and antibody-dependent cell-mediated cytotoxicity (ADCC) was investigated in nine patients with conditions predisposing to infection (T-cell deficiency, humoral immune deficiency, myeloproliferative syndrome, kidney or liver damage, or chronic pulmonary disease). SCMC, using the K562 and LIK cell lines as targets, and ADCC, using the LIK cell line, were measured at baseline and after ten days of therapy with CDZ for lower respiratory tract infections. Six patients were cured; clinical outcome was not evaluable in the other three. SCMC lysis of K562 cells did not change significantly. SCMC and ADCC lysis of LIK (lymphoblastoid) cells both increased significantly. CDZ selectively stimulated a subpopulation of NK cells in this population of immunocompromised patients with lower respiratory tract infections.

[Selective denervation of the wrist joint].

The long-lasting pains of the wrist cause many times problems in the everyday trauma treatment. They are also frequently encountered by orthopedists, internists and rheumatologists. The pain is most frequently the characteristic symptom of osteoarthritis of the wrist, however it may appear without an accident (as a part of destructive and systemic disease, damaging the joints) too. Based on the paper published by WILHELM in 1966 we have performed in this Institute selective denervation of the wrist in 77 cases, with good results in 45 per cent. Our material is analysed in etiologic groups, giving light to the field of indication of this operation, that can be considered most generally as an alternative solution versus radiocarpal and intercarpal arthrodesis. This paper reports beside the description of the method on our favourable experiences, that are published in hope of a more wide spreading of the method.

Treatment of severe chronic bronchitis with ceftazidime.

The aim of this study was to determine whether ceftazidime is more effective than other antibiotics in the treatment of severe acute exacerbation of chronic bronchitis, particularly when other antibiotics have previously failed. Our investigations showed that ceftazidime is effective and well tolerated in patients with severe chronic bronchitis and purulent sputum. The effectiveness of ceftazidime was seen in a reduction of the volume of sputum produced daily and in a colour change from yellow-green to clear-white. The absence of an effect on parameters of lung function was not unexpected since our patients had long-lasting chronic bronchitis.

Controlled prospective study of oral amoxycillin/clavulanate vs ciprofloxacin in acute exacerbations of chronic bronchitis.

This investigation compared the efficacy of oral formulations of amoxycillin/clavulanate and ciprofloxacin in acute exacerbations of chronic bronchitis. Forty patients were randomized to receive either Augmentin (1,000 mg amoxycillin +250 mg clavulanate) tds or ciprofloxacin (500 mg) bd. During and before therapy sputum samples were taken for bacteriology, sputum volume measurement and histamine determination. Lung function was also monitored. From sputum, 143 bacterial isolates and 15 yeast strains were obtained before therapy. During therapy with amoxycillin/clavulanate the incidence of Gram-positive isolates decreased significantly whereas ciprofloxacin left their frequency unchanged. On the other hand, ciprofloxacin more effectively diminished the incidence of Gram-negative isolates. Yeasts were grown from the sputum of several patients before and during therapy. Their number did not increase during amoxycillin/clavulanate therapy while it increased under ciprofloxacin. The groups showed no significant differences with regard to sputum production, histamine concentration or lung function. The majority of patients (28/40) acknowledged improvement of their symptoms. There was no significant difference between the groups. On the basis of these results both amoxycillin/clavulanate and ciprofloxacin appear of similar value for treatment of patients with exacerbations of chronic bronchitis.

Influence of picumast dihydrochloride on the ex vivo histamine release from leucocytes of allergic asthmatic patients.

In a pilot study 8 patients with allergic obstructive airway disease were treated with the new compound picumast dihydrochloride (3,4-dimethyl-7-[4-(4-chlorobenzyl)piperazine-1-yl]propoxycoumarine++ + dihydrochloride) 2 x 2 mg daily orally during a period of 8 days additionally to a usual therapy of bronchodilators and corticoids in a steady state. It was demonstrated that the compound inhibited the allergen induced histamine liberation of patients basophils significantly. Compared with a collective of patients without an additional picumast dihydrochloride treatment the effect of the substance was shown more clearly. From reasons of these experimental data picumast dihydrochloride may be of a considerable value as a prophylactic agent in allergic bronchial asthma when given orally.

Simple pA2 estimation of partial agonists: comparison with the Kaumann-Blinks method.

Estimation of KB and pA2 of partial agonists as antagonists of full agonists is hindered by their stimulant component of action. This reflects competitive synergism, i.e., an additive interaction with a full agonist. The antagonistic component of a partial agonist is seen as a parallel shift of the agonist curve to the right from a constructed additive dose-response curve, from which a dose ratio (DR) was determined for calculation of graphic estimation of KB or pA2 of partial agonists. Dose-response curves from eight studies were analyzed in this way, in comparison with the method of Kaumann and Blinks, and showed excellent agreement between pA2 values determined by either method (r = 0.994, slope = 0.999, n = 17).

Oral corticosteroids and histamine releasability of basophil granulocytes.

The influence of oral corticosteroid therapy (7.5 and 15 mg prednisolon/day for 8 days) on basophil releasability by antigens was investigated in a series of 16 patients with chronic obstructive airway disease. A significantly smaller histamine release was observed after the treatment. Different types of releasing curves were observed which mostly persisted after treatment.

[Histamine content of the sputum of patients with obstructive bronchitis].

High histamine concentrations were measured in the sputum of patients with chronic obstructive bronchitis. The histamine concentration doubles or triples when the sputum is incubated for 24 and 72 h, respectively at 37 degrees C. The antibiotics therapy with doxycycline significantly inhibited the histamine formation in 8 patients, which was mainly caused by bacteria. In the sputum of one patient the histamine formation was reinforced in spite of the doxycycline therapy. The clinical importance of these findings is discussed.

Phage adsorption and productive lysis in stable protoplast type L-forms of Bacillus subtilis and Streptomyces hygroscopicus.

Transferable productive lysis in stable protoplast type L-form cells of Bacillus subtilis was produced by 6 phages out of 14 strains virulent for the parent B. subtilis 170 and 1997. Most of these phages lytic for L-forms show the phi 29 morphology characteristic for the smallest B. subtilis phages containing double-stranded DNA. Among 31 actinophages, 23 of which were virulent for Streptomyces hygroscopicus, only SLE 109 and phi c 31 gave productive infection of the stable protoplast type L-form of S. hygroscopicus NG 33--354. Electron microscopic investigation and treatment by DNAse demonstrated that infection of L-form cells is an adsorption-injection process, and that it is not caused by transfection of free phage DNA or endocytotic uptake of phage particles. Because in both stable L-forms cell wall biosynthesis is blocked irreversibly the results allow the conclusion that specific receptors must be localized in the cytoplasmic membrane for those phages producing transferable lysis in protoplast type L-forms. Localization of receptors for certain phages in the cytoplasmic membrane seems to occur in many Gram-positive bacteria, but not in Gram-negative bacteria.

Cloverleaf skull associated with Pfeiffer syndrome: pathology and management.

The clinical and radiological findings in an infant with severe cloverleaf skull associated with a Pfeiffer syndrome are presented. The skull anomaly resulted in raised intracranial pressure and proptosis of the right eye within several weeks of birth. The child died at 3 months of age after subtotal craniectomy. At autopsy a detailed macroscopical and histological analysis of the skull base was conducted. The timing and value of surgery are discussed as well as the pathology of the cloverleaf skull anomaly.

The role of vagus nerve on histamine- and acetylcholine-induced bronchoconstriction (experiments on cats).

During administration of highly concentrated acetyclcholine and histamine aerosol (20-40 min), different effects on tidal volume, respiratory rate, and bronchial tone could be demonstrated. Inhalation after vagotomy neither showed changes in respiratory rate nor in tidal volume. These results point out the importance of the nervus vagus in case of bronchoconstriction caused by mediators. It is discussed to which degree vagus activity is required for the direct effect of different mediators in living animals.
