[Mediastinal schwannoma originating from the intrathoracic vagal nerve--a case report and review of the literature].

A 31-year-old woman was operated upon with the diagnosis of mediastinal tumor. The resected specimen revealed pathologically benign schwannoma originating from the right intrathoracic vagal nerve. The postoperative course was uneventful with no complication. Mediastinal schwannoma arising from the vagal nerve is rare. We discussed characteristics of this disease and reviewed the Japanese literature.

Accumulation of amyloid beta-protein precursor (APP) in Purkinje cells and increase of amino-terminal fragments of APP in cerebrum and cerebellum of aged rat brain.

In aged rat brain, amyloid beta-protein precursor (APP) is accumulated in dendrites and cell bodies of Purkinje cells as full-length or truncated APP, because dendrites and cell bodies are positively stained by antibodies against both the amino- and carboxy-termini of APP. Western blot analysis of homogenates of brains of aged and young rats showed no apparent differences except for an increase in amino-terminal fragments in cerebrum and cerebellum of aged rat. These results indicate that the expression, transport or metabolism of APP in specific regions of brains may be affected by the aging process.

Early renal effects of occupational exposure to low-level hexavalent chromium.

To detect early renal effects of occupational exposure to hexavalent chromium (Cr), urinary total proteins (U-TP), urinary albumin (U-Alb) and urinary retinol-binding protein (U-RBP) were determined in 166 male Cr platers and 106 male controls. The mean employment time in Cr plating for the platers was 12.6 years. Urinary Cr (U-Cr), which was determined as an index of Cr exposure, ranged from "not detected" to 19.91 micrograms/g creatinine in the platers. The U-Cr level was lower than those in other previous studies. Age-adjusted U-TP, U-Alb or U-RBP levels were not different between the platers and the controls. In the platers, a significant positive correlation was found between age-adjusted U-TP and U-Cr, but U-Cr had no significant relation to age-adjusted U-Alb or U-RBP level. Employment time had no effect on any age-adjusted urinary proteins. The Cr exposure may have been too low to induce definite renal dysfunction. Early renal effects of low-level Cr exposure may be mild, and may not be specific to renal function.

Frequency and operative correction of aortic insufficiency associated with ventricular septal defect.

Frequency and surgical results of aortic cusp prolapse and aortic regurgitation (AR) associated with ventricular septal defect (VSD) were studied. One hundred thirty-six consecutive patients with type I and II VSD according to Kirklin and associates' classification were divided into two groups; group A included 50 patients with type Ia VSD without the conal muscular rim, and group B included 86 patients with type Ib VSD with the conal muscular rim, or with type II VSD. Aortic cusp prolapse was detected in 74% of group A patients and 29% of group B patients. The aortic cusp prolapse correlated negatively with preoperative left-to-right shunt ratio, mean pulmonary artery pressure, and pulmonary-to-systemic pressure ratio in both groups. In group B patients, the smaller the VSD, the higher the frequency of aortic cusp prolapse, especially when less than 4 mm. Aortic regurgitation was observed in 44% of group A patients and 24% of group B patients. All 20 patients with first grade AR underwent VSD closure, 11 with second or third grade AR underwent VSD closure plus valvuloplasty, and 1 with third grade AR underwent aortic valve replacement. Postoperative persistent AR occurred in 8 out of 32 patients, and correlated positively (p < 0.01) with the preoperative grade of AR and the number of plication stitches in both groups.

Diverticulum of the superior vena cava.

A 14-year-old girl underwent operation with the diagnosis of diverticulum of the superior vena cava. Microscopic findings revealed a diverticulum with venous architecture. This represents a rare case of a giant diverticulum of the superior vena cava.

Relationships between serum saturated fatty acids and serum total cholesterol and HDL-cholesterol in humans.

To investigate the effects of serum saturated fatty acids (FAs) on human cholesterol metabolism, total-cholesterol (T-C), HDL-cholesterol (HDL-C), T-C/HDL-C ratio, and FA composition [myristic acid (MA, 14:0), palmitic acid (PA, 16:0) and stearic acid (SA, 18:0)] were determined in serum from 115 men and 120 women (20-70 years old). MA, PA and SA were expressed as percentages of serum total FAs. Using multivariate analysis to account for the confounding effects of age, body mass index, drinking, and smoking it was found that SA was negatively correlated with T-C and T-C/HDL-C ratio in both men and women. In men MA was negatively correlated with HDL-C, and in women SA was positively correlated with HDL-C. Although PA was the major component of serum saturated FAs, PA had no significant relation to any cholesterol indices in either sex. These results suggest that serum MA may aggravate and serum SA may improve cholesterol metabolism, thereby influencing the risk for ischaemic heart disease.

Polysaccharides in Fungi. XXXIV. A polysaccharide from the fruiting bodies of Amanita muscaria and the antitumor activity of its carboxymethylated product.

A water-insoluble, alkali-soluble, glucan (AM-APP), [alpha]D +160 degrees in 0.4 M NaOH, was isolated from the alkaline extract of the fruiting bodies of Amanita muscaria. The results of chemical and spectroscopic investigations indicate that AM-APP is a linear (1 --> 3)-alpha-D-glucan with a molecular weigh estimated by gel chromatography of about 42000. Its carboxymethylated product (AM-APP-CM) showed potent antitumor activity against sarcoma 180 in mice, although the native polysaccharide (AM-APP) had little effect. The distribution of carboxymethyl groups in the molecule was analyzed by gas chromatography and mass spectrometry. The degree of substitution of carboxymethyl groups was 0.95 and the substituents were located at O-2, at O-4, at O-6, at O-2 and O-6, and at O-4 and O-6 on glucose.

Formation of anti-sense RNA to IFN mRNA in poly (rI):(rC) induced HEL cells.

To elucidate the mechanism of super induction of IFN, mRNA and anti-sense RNA were analyzed by using reverse transcription-polymerase chain reaction (RT-PCR) method, and following results were obtained. (1) When human embryonic lung fibroblast cells (HEL cells) were exposed to poly (rI):(rC) for 1 hr, treated with cycloheximide for 3 hr, treated with actinomycin-D for 30 min at the final period of cycloheximide treatment, then washed and replenished with maintenance medium, (HEL-I:C/CH/Act. D), they produced much higher amount of IFN and IFN production continued for longer period when compared to the HEL cells which were not treated with actinomycin-D (HEL-I:C/CH). In agreement with these IFN production kinetics, HuIFN-beta mRNA was stabilized in HEL-I:C/CH/Act. D. In HEL-I:C/CH/Act.D, the mRNA was detected at the end of cloheximide treatment (4 hr after induction) and 6 hr after induction, however, it was not detected at 6 hr after induction in HEL-I:C/CH. (2) Anti-sense RNA was detected in HEL-I:C/CH/Act.D at 4 hr and 6 hr after induction, and also in HEL-I:C/CH at 4 hr after induction, however, it was not detected at 6 hr after induction in HEL-I:C/CH. These results indicated that an anti-sense RNA to HuIFN-beta mRNA was formed in poly (rI):(rC) induced HEL cells, and that aRNA had the same fate as mRNA, suggesting it's role in the regulation of IFN production. Possible role of aRNA in the regulation mechanism of IFN production was discussed.

[Surgical treatment of needles in intracardiac cavity and chest wall--a case report and the review of cases removed under open-heart surgery in Japan].

A 36-year-old woman under the treatment for neurosis stuck five sewing needles into her left anterior chest wall for the purpose of suicide. She was hospitalized with complaints of anterior chest pain and low-grade fever. Chest roentgenogram and computed tomogram showed two needles in the cardiac cavity and three needles in the chest wall. Following extraction of three needles in the chest wall, we removed two needles from the left and the right ventricular cavities under extracorporeal circulation. Preoperative and intraoperative two-dimensional echocardiography was useful for confirming the location of needles.

[Experience with operation for synchronous multiple primary lung cancer].

We presented a 66-year-old man with synchronous double cancers developing in the right upper lobe, which were diagnosed preoperatively. He underwent right upper lobectomy with hilar and mediastinal lymphadenectomy. Post-operative patho-histological study showed two different histologic types of cancer; one was undifferentiated carcinoma of the small cell type, and the other was moderately differentiated adenocarcinoma.

[Lung cancer with pleural dissemination diagnosed by thoracofiberscopy: a case report].

Pleural dissemination was suspected in a 39-year-old woman with pulmonary adenocarcinoma. Thoracofiberscopy provided the preoperative exact diagnosis of pleural dissemination and the following prepare operative procedure. Thoracofiberscopy is effective for the diagnosis of pleural dissemination and the selection of operative methods in patients with pulmonary cancer.

[A concomitant operation of coronary artery bypass grafting and thymectomy: a case report].

A 63-year-old man was successfully operated upon concomitantly for coronary artery disease and invasive thymoma. The chest roentogenogram showed a mass shadow in the anterior mediastinum, which was 6.0 x 4.0 cm in size and irregular. Coronary angiography revealed complete obstruction of the right coronary artery and 90% stenosis of the first diagonal branch (#9). Following thymectomy combined with the resection of the left phrenic nerve and the mediastinal pleura, coronary artery bypass grafting with the major saphenous vein was performed to #4 PD and #9. This is the first report of concomitant surgery for the ischemic heart disease and invasive thymoma in Japan.

[The transverse submammary skin incision for median sternotomy].

Median sternotomy is the incision of choice to allow access to the anterior mediastinum, heart or both lungs. The vertical skin incision is cosmetically unsatisfactory for many female patients. The transverse submammary skin incision with dissection of a flap including the subcutaneous tissue and breasts allow exposure of the sternum for a median sternotomy. Seventy patients with the transverse submammary skin incision was compared with 56 patients with the vertical skin incision in the point of the cosmetic aspect and satisfaction of patients. The transverse submammary skin incision needed a longer wound size and took a longer time for cardiopulmonary bypass, but there were no difficulties in cannulating the ascending aorta for cardiopulmonary bypass. The exposure of the mediastinum and the cosmetic result were excellent. The complications associated with this incision are insignificant if close attention is paid to details.

[Surgical treatment of abdominal aortic aneurysm associated with idiopathic thrombocytopenic purpura and consumption coagulopathy: a case report].

A 68-year-old man who had abdominal aortic aneurysm (AAA) associated with idiopathic thrombocytopenic purpura (ITP) and consumption coagulopathy was successfully operated upon. The count of platelet was improved from 1.4 x 10(4)/microL to 8.0 x 10(4)/microL by the preoperative treatment with gamma-globulin (400mg/kg/day for 5 days) and heparin sodium (10000-15000 unit/day). Abdominal aortic aneurysm was replaced with a Y-shaped knitted Dacron graft, and splenectomy was performed for ITP. No severe hemorrhagic diathesis was encountered during and after the operation. This is the first report of surgical treatment of AAA associated with ITP and consumption coagulopathy.

[Thoracotomy for patients with bronchial asthma].

We discussed the perioperative management in three patients with bronchial asthma who underwent thoracotomy. The management was as follows: 1. theophylline and steroid agents were preoperatively given till wheezing disappeared, 2. just before surgery, theophylline or beta-stimulator and steroid agents were given, 3. during surgery, aminophylline and steroid agents were given and bronchodilative anesthetics were inhaled, 4. steroid agents were postoperatively applied sufficiently so that signs due to bronchial asthma don't appear. Preoperative respiratory conditions improved with disappearance or decrease of wheezing, and resection of giant bulla and lobectomies with radical mediastinal lymph node dissection were safely possible with no evidence of the side effects due to steroid administration. The perioperative sufficient suppression of airway-tract hyperactivity is very important for thoracotomy in patients with bronchial asthma.

Cell fusion-induced quick change in replication time of the inactive mouse X chromosome: an implication for the maintenance mechanism of late replication.

It is unknown how and why the genetically inactivated mammalian X chromosome replicates late in S phase. There are also occasional inactive X chromosomes characterized by an opposite behavior replicating early in S phase. Two clonal cell lines, MTLB3 and MTLH8, isolated from a cultured murine T-cell lymphoma have an allocyclic X chromosome of the latter type. This precociously replicating X chromosome was judged to be genetically inactive as the late replicating one. Immediately after fusion with another cell line, the precociously replicating X chromosome from these cells starts to replicate late in S phase. This finding seems to suggest that late replication characterizing the inactive X chromosome is actively maintained by a trans-acting factor in female somatic cells, and that its lack entails a switch from late replication to precocious replication. It remains unknown whether this presumptive factor also modifies the autosomal replication pattern.

Carbonic anhydrase II deficiency in three unrelated Japanese patients.

Three Japanese patients with carbonic anhydrase II (CAII) deficiency from three families were described. The parents of one patient were unrelated, the parents of each of the other two patients were first cousins. All the patients had renal tubular acidosis, osteopetrosis, symmetrical cerebral calcification and mental retardation. They exhibited poor activity and poor appetite in the neonatal period, and then developed psychomotor retardation. Two of them were diagnosed as having osteopetrosis at 10 months and 36 years of age, respectively, and the other as having osteomalacia at 28 years of age. All patients had recurrent episodes of muscle weakness. The CAII enzyme activity and protein levels in red blood cells in each of the three patients were deficient. Their parents exhibited approximately 50% normal levels of CAII activity and protein. This is the first report of patients with CAII deficiency in the Japanese population.
