Combined application of heterologous collagen and fibrin sealant for liver injuries.

Hemostasis in complex liver injuries remains a problem despite improvements in operative techniques including debridement, suturing or packing. To evaluate fibrin sealant (FS), a new biodegradable hemostatic agent in combination with porcine collagen for sealing of liver injuries, three series of experiments were performed in 132 rats. In series I, 18 rats had a 10-mm in diameter and 2-mm in depth punch defect to the left lateral lobe. In the FS group (n = 9), bleeding was treated by insertion of an FS-soaked piece of collagen of equal size which was firmly attached to a plastic disk with wire anchor. In the control group (n = 9), collagen alone was inserted. Fifteen minutes after the insertion the lobe was excised and pull-off experiments were performed with simultaneous script chart recording. There was a highly significant difference in the adhesion to the liver surface (85.6 +/- 7.1 in the FS group versus 24.8 +/- 2.6 g/cm2 in the control group, P less than 0.001). In series II, 42 anticoagulated rats (Coumadin, PT 27.5% +/- 1.3) with lobectomy or liver rupture were placed in three groups (n = 14). Group I was treated with FS, group II with FS and collagen, and group III with catgut sutures which served as controls. Fourteen days later 12 rats of group I, 13 of group II, and 7 of group III were alive yielding 85.7, 92.8, and 50% overall survival rates, P less than 0.05 groups I and II versus group III. In series III, 72 non-anticoagulated rats were treated identically to series II and examined morphologically at 1, 7, 28, and 56 days.(ABSTRACT TRUNCATED AT 250 WORDS)

[Microsurgical anastomoses of nerves and arteries using absorbable sutures (author's transl)].

Anastomoses of the carotid artery and the sciatic nerve of the rat were performed by using the microsurgical technique and comparing the nonabsorbable suture material Ethilon and the absorbable Vicryl, both 0,2 metric. No suture insufficiency neither in sutured nerves nor in vessels could be observed. After 70 to 84 days the Vicryl sutures were completely absorbed. In particular nerve sutures using Vicryl proved to be superior to Ethilon, as the displacement of axons in the area of granuloma decreased with progressing absorption of the suture and the degeneration of the axons was less renewed.

[Recent aspects of the treatment of local hydrofluoric acid erosion].

In experimental studies we compared the intraarterial injection of calcium to local infiltration of calcium as treatment of local hydrofluoric acid burns. We showed that intraarterial injection of calcium is the superior therapy in terms of earlier and more effective arrest of destruction. The results are better the earlier the injection is given. There were no local or systemic disturbances due to the intraarterial injection of calcium.

[Microsurgical nerve and blood vessel sutures with resorbable suture material].

An experimental study of the behavior of the absorbable suture Polyglactin 910 (Vicryl) 0,2 metric in the tissue was performed. Microsurgical sutures of nerves and vessels (sciatic nerve and carotid artery) were performed and compared with a non-absorbable suture (Ethilon). The micromorphological studies included a period of 12 weeks after surgery. The absorbable suture showed equal mechanical qualities, with the advantage that the suture material and the tissue reaction disappeared after the 10th week, while the poor visualization of the absorbable suture under the operating microscope was a disadvantage.

[Comparative examinations of hysterectomy clamps (author's transl)].

Conventional and recently developed hysterectomy clamps were examined with respect to sure, nonslip grip and minimum resulting tissue trauma. Slippage was appraised by means of the tenacity testing device, tissue damage on the basis of macroscopic, histologic, and scanning-electron-microscope findings. The new clamp (Zeppelin) comes closest to meeting the demands set for sure grip and gentleness to tissue as compared to the conventional clamps (Wertheim, Rogers, Masterson) and best satisfies requirements for the use in hysterectomies today.

[Activity of plasminogen activators and plasmin inhibitors in the joint capsule in various articular diseases (author's transl)].

The activity of plasminogen activators and plasmin inhibitors in articular capsules of the hip and knee joints in rheumatoid arthritis, chronic traumatic inflammation, arthrosis and aseptic loosening of prosthesis was determined histochemically and correlated with the degree of mobility in osteoarthrosis of the hip. Microscopically unchanged articular capsules were employed as reference sites. In inflammatory articular diseases the plasminogen activator activity was significantly reduced, whereas it remained unchanged in arthrosis and necrosis of the heas of the femur, and significantly enhanced in aseptic loosening of prosthesis. Plasmin inhibitor activity was established in joint capsules with inflammatory changes only; in case of rheumatic inflammation, it was higher than with traumatic inflammation. In osteoarthrosis of the hip there is a positive correlation between joint mobility and plasminogen activator activity. The results point towards involvement of the local fibrinolytic system in joint diseases; however, no definite statement can be made with regard to possible causative linkups.

[Autologous vein grafts as artery replacement in microsurgery. An experimental study of micro- and macromorphologic changes 3 weeks to 12 months postoperatively].

In 45 rabbits we interposed autologous vein grafts of different length in equivalent defects of the femoral artery. After fixed intervals of six weeks to 12 months functional, histological and angiographic studies were done. Long grafts showed the same thrombus rate as short ones. The histological findings showed a marked thickening of the vessel wall with a maximum between six to eight weeks after operation. After this period this thickening was reduced but was after 12 months still noticeable. The angiography showed no signs of stenosis or narrowing of the lumen. Occasionally we saw bone formation or incrustation which might be induced by intima lesions.

[Examination of plasminogen-activators and plasmin-inhibitors in maternal and fetal rat-lung-tissue after administration of DL-carnitine-hydrochloride (author's transl)].

In the formation and resolution of fibrinous pulmonary hyaline membranes of the type occurring in perinatal respiratory distress, the plasminogen activators and plasmin inhibitors contained in the tissue assume central importance. Carnitine does not change plasminogen activator activity and reduces the plasmin inhibitor content in the foetal rat lung. Hence, no inhibiting action on local fibrinolytic processes can be established for carnitine.

Anastomosis of A. carotis and of N. ischiadicus of the rat were carried out by microsurgical technique in comparison to the nonabsorbable suture material Ethilon and the absorbable Vicryl, both 0,2 metric. There occurred no suture insufficiencies with either sutured nerves or vessels. After 70-84 days the Vicryl sutures were completely absorbed. In particular with nerve sutures Vicryl proved to be superior to Ethilon, as the displacement of axons in the area of granuloma decreased with progressing absorption of the suture and thus also resulting less renewed and thus also resulting less renewed degeneration of the axons.

[Experimental investigations of rat ileum enterotomies after application of physiological plasma fractions (author's transl)].

Standardised enterotomies of the rat ileum were performed by conventional enterorrhaphy and in combination with the fibrin adhesion technique. When enterotomy is also sealed with fibrin adhesive, the initial bursting strength is significantly higher. In the postoperative course this is only valid if a successful inhibition of fibrinolysis can be achieved by adding fibrinolytic inhibitors. Histochemical investigations of tissue enzyme activities confirm these findings.

The mechanical, enzymatic and morphological changes in acute pulmonary insufficiency following the production of a haematoma in rabbits.

The production of a haematoma in the hindleg of rabbits represents a simplification of post-traumatic models. The lungs, one of the target organs, show similar micromorphological alterations to those described in traumatised human beings. The lungs of rabbits, bearing a hindleg haematoma for 24 hr, showed a significantly increased incidence of micromorphological lung alterations compared to the control group. Moreover, measurement revealed a significantly decreased lung compliance within the haematoma groups. It was demonstrated that whether the haematoma was subcutaneous or intramuscular, similar alterations were seen in lung histology, and in lung compliance values. Studies on the activity of superoxide dismutase in lung tissue homogenates demonstrated a significant decrease in those rabbits having had a haematoma intramuscularly for 24 hr.

Experimental studies on the use of microwaves for the localized heat treatment of the prostate.

High frequency tumor therapy requests heating of a well defined area, whereas the surroundings should be unaffected. This sort of therapy should be more tolerable than a whole body heating. For this purpose a new developed microwave applicator (433.9 MHz) for local heat application was tested in animals. Using this rectally insertable applicator the prostate of dogs were irradiated. The temperature of the rectum, the urethra and the prostate were measured and recorded using specially developed thermistor probes. The effect of hyperthermia on the prostate and the surrounding tissue was analyzed macroscopically and micromorphologically. It was shown that local heating of the prostate of dogs is possible. The induced damages of the prostate and surrounding tissue is not severe enough to be untolerable.

[Anesthsiological experiences in experimental surgery].

The paper reports experience with methods of anaesthesia in rabbits, rats, mice, guinea-pigs, pigs, sheep, baboons, dogs and cats. Special problems of handling, mode of injection and intubation are discussed. A comprehensive survey of literature provides further information about the proposed patterns of anaesthesia and informs about other anaesthetic methods, not practised by the authors.
