Studies of the structure and organization of cationic lipid bilayer membranes: calorimetric, spectroscopic, and x-ray diffraction studies of linear saturated P-O-ethyl phosphatidylcholines.

Differential scanning calorimetry, x-ray diffraction, and infrared and (31)P-nuclear magnetic resonance ((31)P-NMR) spectroscopy were used to examine the thermotropic phase behavior and organization of cationic model membranes composed of the P-O-ethyl esters of a homologous series of n-saturated 1,2-diacyl phosphatidylcholines (Et-PCs). Differential scanning calorimetry studies indicate that on heating, these lipids exhibit single highly energetic and cooperative endothermic transitions whose temperatures and enthalpies are higher than those of the corresponding phosphatidylcholines (PCs). Upon cooling, these Et-PCs exhibit two exothermic transitions at temperatures slightly below the single endotherm observed upon heating. These cooling exotherms have both been assigned to transitions between the liquid-crystalline and gel phases of these lipids by x-ray diffraction. The x-ray diffraction data also show that unlike the parent PCs, the chain-melting phase transition of these Et-PCs involves a direct transformation of a chain-interdigitated gel phase to the lamellar liquid-crystalline phase for the homologous series of n > or = 14. Our (31)P-NMR spectroscopic studies indicate that the rates of phosphate headgroup reorientation in both gel and liquid-crystalline phases of these lipids are comparable to those of the corresponding PC bilayers. However, the shape of the (31)P-NMR spectra observed in the interdigitated gel phase indicates that phosphate headgroup reorientation is subject to constraints that are not encountered in the non-interdigitated gel phases of parent PCs. The infrared spectroscopic data indicate that the Et-PCs adopt a very compact form of hydrocarbon chain packing in the interdigitated gel phase and that the polar/apolar interfacial regions of these bilayers are less hydrated than those of corresponding PC bilayers in both the gel and liquid-crystalline phases. Our results indicate that esterification of PC phosphate headgroups results in many alterations of bilayer physical properties aside from the endowment of a positively charged surface. This fact should be considered in assessing the interactions of these compounds with naturally occurring lipids and with other biological materials.

Refined structure of 1,2-diacyl-P-O-ethylphosphatidylcholine bilayer membranes.

1,2-diacyl-P-O-ethylphosphatidylcholines are synthetic cationic lipids that show some promising properties as nonviral DNA transfection agents. To gain further insight in the effects of the additional ethyl group in the headgroup region on the bilayer structure we systematically investigated a homologous series of fully hydrated ethylphosphatidylcholines with linear saturated chains (C14:0, C16:0, and C18:0) by small- and wide-angle X-ray diffraction. Our data show that all of them form multilamellar vesicles with chain interdigitated gel phases. Paying regard to the very importance of the liquid-crystalline phase in gene transfection, we applied the novel MCG method on high resolution X-ray diffraction data of the C16:0 derivative to be able to gain structural information on this phase. Comparison of this ethylphosphatidylcholine with its parent compound, the unesterified phosphatidylcholine, revealed that the major difference in the liquid-crystalline phase is the significantly reduced water layer between the bilayers for the cationic lipid. This may be one factor that contributes to the comparatively good DNA transfection efficiency of ethylphosphatidylcholines.

[Effectiveness of partial inpatient rehabilitation from the patient's viewpoint. Results of an evaluation of a partial hospitalization model project at the Ichenhausen specialty clinic].

Partial-hospitalization rehabilitation is a relatively recent form of rehabilitation, which, as opposed to the ambulatory approach, comprises the entire programme available for inpatient medical rehabilitation except for accommodation and full board. Great expectations are attached to partial hospitalization: it is supposed to cost less than a comparable in-patient measure but matching in terms of quality. However, hardly any empirical data exist so far concerning the effects partial hospitalization rehabilitation is having in these respects. The present article seeks to answer the question of whether from a patient perspective, the outcome quality of this new model is comparable to that of the classical full-hospitalization programmes. In terms of methodology, questionnaires were used to raise the necessary data from two independent samples (i.e., partial vs. full hospitalization) of former orthopaedic rehabilitees of Fachklinik Ichenhausen (Bavaria). Data analysis included some 205 patient questionnaires (57% return rate). High agreement was found among the two groups concerning outcome evaluation, subjective health and patient satisfaction. From a patient perspective, rehabilitation outcome on the majority of variables considered, even is judged slightly better for partial hospitalization patients than for the inpatient comparison group. As none of these results reach statistical significance, these minor differences should however not be overrated. While partial-hospitalization, rehabilitation, hence, has turned out an option which matches traditional full-hospitalization programmes in terms of quality, too great expectations nevertheless are unwarranted: Partial-hospitalization rehabilitation is no general alternative to inpatient programmes as it will realistically be suitable for a small percentage of orthopaedic patients only, hence be unable to replace full-hospitalization programmes to any major extent. Undoubtedly, however, it is apt to contribute to greater flexibility and needs orientation among the services and programmes available in rehabilitation, and may thus be considered a valuable approach for complementing inpatient and ambulatory forms of rehabilitation.

[A model of integrated partial inpatient rehabilitation].

This article deals with a new form of rehabilitation in Germany: the partly inpatient rehabilitation. This kind of rehabilitation comprises an offer for regular inpatient rehabilitation excluding overnight stay, breakfast and evening meal. In total the quasi inpatient rehabilitation is qualitatively equal to regular inpatient rehabilitation in respect of rehabilitative outcome. This new kind of rehabilitation contributes to a flexible and requirements-oriented health service in the German rehabilitation system. In this respect, "quasi inpatient rehabilitation" seems to be a valuable complement to traditional inpatient and out-patient rehabilitation, though quasi inpatient rehabilitation cannot generally replace traditional rehabilitation.

Effects of hexadecylphosphocholine on fatty acid metabolism: relation to cytotoxicity.

The cytotoxicity of the antineoplastic drug hexadecylphosphocholine (HePC) was determined in a human monocytic tumor cell line, THP1, and in primary cultures of rat mesangial cells. Both cell types were susceptible to HePC toxicity, the concentrations producing 50% inhibition of cell viability (LD50 values) being 7 micrograms/ml for THP1 cells and 19 micrograms/ml for mesangial cells. The degree of toxicity was dependent on the culture conditions. In the absence of serum, HePC was highly toxic independent of cell proliferation. As a potential molecular mechanism, the effect of HePC on long-chain fatty acyl metabolism was investigated. HePC had no effect on fatty acid incorporation into cellular lipids or on release of fatty acids from lipid stores. The distribution of labeled fatty acids, however, was shifted from the phospholipid fraction to the triacylglycerol fraction. This effect was in accordance with an inhibition of the activity of the reacylating enzyme lysophosphatide acyltransferase. There was, however, no correlation between the interference with fatty acid distribution and HePC cytotoxicity in vitro. The data argue against interference with membrane fatty acid metabolism as a necessary prerequisite of HePC toxicity, the mechanism of which remains to be elucidated.

Autoradiographic localization and effects of exogenous radioactively labelled surfactant in the lung of the prematurely delivered fetal rabbit.

Adult rabbit lung surfactant was radioactively labelled with [3H]palmitate and isolated by centrifugation. This material was instilled into the trachea of fetal rabbits prematurely delivered on the 27th gestational day. A similar preparation of unlabelled surfactant was used to measure the effects on pressure-volume characteristics in lungs of 27th day fetuses. Tissue sections were prepared from the lungs of all animals and morphometric and autoradiographic determinations made. Surfactant instillation improved pressure-volume relationships in fetal rabbit lungs. Histologically, although only the middle right lobe seemed to show significant qualitative improvement in expansion after surfactant treatment, quantitative assessment indicated that the surfactant preparation had significantly increased the mean alveolar cross-sectional areas in all three lobes of right lungs. In addition, distribution of autoradiographic grains indicated that 8-25% were located over the alveolar spaces while approximately half this percentage was present over tissue at the level of the alveolus. These results indicate that intratracheal instillation of surfactant supplements the endogenous surfactant at the level of the alveolus.

[New developments in the work environment--consequences for occupational preventive health].

The introduction of new technologies has made partly severe inroads into conventional types of work and has substantially changed the working tools and their handling, involving new forms of stress and demands made on the operators. The ultimate effects of these developments are largely hidden from sight and pose new theoretical and practical problems in occupational medicine. This report deals with these problems and describes on the basis of several examples how the officers who are responsible for occupational safety and industrial health try to cope with changed circumstances and demands.

[Health education within the scope of task development for occupational medicine].

Company doctors occupy a key position when translating into reality health policy demands for occupational health promotion. Although general health education measures have been realised for a long time within the overall framework of occupational safety and health protection, industrial health officers will now have to cope with many new problems. Two examples are given to show how the demand for extending the conventional patterns of medical care by the discipline of health education is approached and translated into reality from two angles representing two basic positions.

[Smoking in industry--a controversial topic of prevention].

The realization that smoking is not only harmful to the health of the smoker but also to that of his environment, polarizes smokers and non-smokers. This can give rise to an explosive atmosphere of possible conflicts especially at the place of work. Non-smokers expect from the company doctor--not without some justification--to support their interests. However, it cannot be his obligation to make things worse by going all out in favour of the non-smokers, because he also wants to be listened to by the smokers. The article deals with this delicate problem. It also discusses as to whether the company can be enlisted for support in case of a wish to stop smoking. In this regard it is essential to find out how company-sponsored courses to stop smoking are assessed.

[Wholesome occupational food services--an area of prevention with high acceptance].

The general public is increasingly interested in health-promoting food, an interest that has its effects on the food situation in a commercial enterprise or industrial company. Canteen food is of central importance for two reasons: first of all, it is possible to supply salubrious food that is well-balanced according to modern guidelines worked out by nutritionists; secondly, modern canteen catering can exercise an influence on the general attitude to food. An increasing number of companies are now offering their employees courses in nutrition. In this manner, companies are fulfilling an important task of health education that raises relatively fewer conflicts than other topics. Multiplicators are therefore of the opinion that this is a most promising area of possible development towards general health consciousness in the long run.

[Stress and stress prevention in occupational practice from the viewpoint of occupational experts].

Stress is a common and widespread phenomenon in today's life and work. In a company it is present in all divisions and at all levels of activity. How to tackle stress is a difficult and by no means uniform problem. Some companies offer their employees possibilities to prevent or reduce stress. It is shown what this can look like and what are the pre-requisites and general conditions to achieve it.

[Responsibilities of occupational social counseling in health promotion].

As the modern world of labour changes, so do the areas of activity of the social advisory service in a company change. They are increasingly less concerned with traditional welfare work and are now including broad perspectives of preventive health measures and prophylaxis in general. There is a trend to professionalization and hence to changing the professional image: experts in socio-educational matters and persons who have undergone training in psychology are being increasingly employed. Social services are an effective complement to medical health care and hence also important cooperation partners for company doctors. Depending on the interests and aims of the company concerned abandon its structural employment pattern and long-standing sociopolitical traditions, as well as on new company policy guidelines, the social services differ in respect of their aims from company to company. Three examples are cited to illustrate this.

Endothelium- and sydnonimine-induced responses of native and cultured aortic smooth muscle cells are not impaired by nitroglycerin tolerance.

Tolerance to the cyclic GMP-mediated vasodilator action of nitroglycerin develops with prolonged exposure and may be mediated either by formation of less nitric oxide from nitroglycerin or by desensitization of soluble guanylate cyclase to activation with nitric oxide. In the latter case, smooth muscle cells tolerant to nitroglycerin should show cross-tolerance to nitric oxide released from sydnonimines and endothelial cells (endothelium-derived relaxing factor). Therefore cultured smooth muscle cells from rabbit aorta were pretreated for 1 h with vehicle or high concentrations (0.55 mM) of nitroglycerin or the sydnonimine SIN-1. The formation of cyclic GMP induced by subsequent small doses of nitroglycerin, sydnonimine SIN-1 and endothelium-derived relaxing factor (released from cultured endothelial cells) was compared with the changes in activation of soluble guanylate cyclase, cyclic GMP formation and vasodilation in response to the same stimuli in similarly pretreated segments from rabbit thoracic aortae. Both cultured and native smooth muscle cells remained responsive to stimulation with sydnonimine SIN-1 and endothelium-derived relaxing factor after pretreatment with nitroglycerin, vehicle, or sydnonimine SIN-1, even though they were tolerant to nitroglycerin after pretreatment with nitroglycerin. In contrast, activation of soluble guanylate cyclase by nitroglycerin and sydnonimine SIN-1 was attenuated in homogenates of nitrate-tolerant aortae. The findings suggest that nitroglycerin tolerance in intact cells does not involve desensitization of soluble guanylate cyclase, because in intact cells nitrate tolerance can be overcome by direct activators of soluble guanylate cyclase.

Fendiline and calmidazolium enhance the release of endothelium-derived relaxant factor and of prostacyclin from cultured endothelial cells.

We investigated the effects of fendiline, calmidazolium and trifluoperazine, compounds described as calmodulin antagonists, on the release of the endothelial autacoids prostacyclin (PGI2) and endothelium-derived relaxant factor (EDRF). Cultured bovine aortic endothelial cells were grown on microcarrier beads and continuously superfused with Tyrode's solution. Samples collected from the superfusate were assayed for PGI2 concentration (6-keto PGF1 alpha radioimmunoassay) and for EDRF activity (stimulation of soluble guanylate cyclase in vitro). Stimulation of endothelial cells by ATP (3 microM) resulted in a 6.9 +/- 1.4-fold increase of PGI2 concentration in the superfusate (p less than 0.01) and an 8.6 +/- 3.4-fold enhanced guanylate cyclase activity (p less than 0.01). In the presence of calmidazolium (10 microM), the basal values of PGI2 concentration increased 28-fold (p less than 0.01) and the guanylate cyclase activity 10-fold (p less than 0.01). Further enhancement of both was observed after additional administration of ATP. Fendiline (30 microM) did not affect autacoid release by non-stimulated cells. However, the ATP-induced release of PGI2 and EDRF was more than doubled (p less than 0.01) in the presence of this drug compared to ATP-stimulation alone. Trifluoperazine (10 microM) had no enhancing effect on EDRF release, and the ATP-induced release of PGI2 was even significantly attenuated by 84 +/- 12% (p less than 0.01). Calmidazolium and fendiline were also applied to endothelial cells loaded with the fluorescent indicator of free calcium concentration (Ca2i+), indo-1. However, effects of calmidazolium on Ca2i+ could not be quantified since calmidazolium caused some leakage of indo-1 out of the cells. A smaller leakage was observed during the combined application of fendiline and ATP.(ABSTRACT TRUNCATED AT 250 WORDS)
