Diagnostic value of thyrotrophin releasing hormone tests in elderly patients with atrial fibrillation.

A prospective study was carried out to compare clinical and biochemical thyroid states with responses of thyroid stimulating hormone (TSH) to thyrotrophin releasing hormone (TRH) in elderly patients with either atrial fibrillation (n = 75; mean age (SD) 79.3 (6.0) years) or sinus rhythm (n = 73; mean age 78.4 (5.6) years) admitted consecutively to the department of geriatric medicine. No patient in either group had symptoms or signs of hyperthyroidism. Overall, the TSH responses to TRH did not differ significantly between the two groups. Ten (13%) of the patients with atrial fibrillation (of whom four had raised thyroid hormone concentrations) and five (7%) of the patients with sinus rhythm showed no TSH response to TRH while 26% of each group (20 and 19 patients, respectively) showed a much reduced response. Only one of 13 patients with apparently isolated atrial fibrillation showed no TSH response to TRH, and none of these 13 patients was hyperthyroid. In particular, three patients (two with atrial fibrillation and one with sinus rhythm) who showed no TSH response to TRH at presentation exhibited a return of TSH response to TRH at follow up six weeks later. In conclusion, reduced or absent TSH responses to TRH are common in sick elderly patients whether they have atrial fibrillation or sinus rhythm and whether they are euthyroid or hyperthyroid biochemically. An absence of response is therefore an uncertain marker of hyperthyroidism in these groups of patients, and diagnosis and ablative treatment should be based at least on the presence of raised circulating free triiodothyronine or free thyroxine concentrations, or both.

Sexual development and behavior in black preadolescents.

As part of a longitudinal study of the sociosexual development of black preadolescents, sexual maturation and sociosexual behaviors were assessed and the relationships between these variables determined in a sample of 101 nine- to eleven-year-old middle- and low-income boys and girls. Sexual maturation was measured by Tanner's staging criteria of specific secondary sex characteristics. Involvement in heterosexual behaviors was elicited via self reports and was classified on a five-point heterosexual physical activity scale (HPA). The data corroborate other studies in demonstrating that girls were more advanced than boys in the process of sexual maturation. Considerable variation in stages of maturation for chronological age existed in both boys and girls, but was more pronounced for girls. In girls, there was no significant association between HPA and degree of biologic maturation. However, genital development in boys was significantly related to their sexual behavior. Income level was not significantly associated with the HPA score. A baseline from which to chart the progress of sociosexual behaviors in these developing preadolescents was established.

Controlled trial of homoeopathic treatment of osteoarthritis.

In a double-blind, placebo-controlled crossover study to compare the homoeopathic remedy Rhus tox. 6X with fenoprofen in osteoarthritis of the hip and knee, fenoprofen was shown to have beneficial analgesic and anti-inflammatory effects which differed significantly from those of placebo. The effects of Rhus tox. 6X and placebo did not differ significantly. Patient preference was for fenoprofen. Side-effects were not severe but were seen more frequently with fenoprofen. Similar results were seen in all patients regardless of whether they had been referred to and assessed by a homoeopathic physician or a rheumatologist.

Aetiology of hyperthyroidism in Canada and Wales.

A retrospective, comparative review of 100 consecutive new outpatients presenting with hyperthyroidism in Cardiff, South Wales, and in Toronto, Canada, was performed. The aim was to quantify the causes of hyperthyroidism with particular emphasis on the prevalence of viral thyroiditis and "silent" thyroiditis. The proportional morbidity of Graves' disease (approximately 70%) was similar in the two groups. Toxic multinodular goitre and toxic adenoma (Plummers' disease) occurred significantly more frequently in Cardiff (25% v 8%), whereas thyroiditis predominated in Toronto (17% v 1%).

The identification of 'casein' in human breast cancer.

A retrospective study of ninety-six cases of breast cancer was carried out to determine the prognostic values of a casein immunolocalization technique. This was performed using an indirect immunofluorescence method with antisera raised in rabbits to pooled human casein. Fluorescence positivity was graded according to its intensity and distribution. The pathology of each tumour was studied and the tumour type, histological grade and tissue response assessed. The relationship between these observations and the casein content of the tumour was studied. In addition the correlation between casein content and age, menopausal state, clinical staging and survival was investigated. The incidence of casein positivity in our series was 50% with approximately half of the positive cases showing strong fluorescence. There was a relationship between casein content and the age of the patients, with casein being more frequently found in tumours from younger patients. Tumours with a high casein content, in general, show a poorer survival than the group as a whole. This difference in survival was most marked in the first eight years after operation and restricted to those tumours in clinical stage I and histological grade I. The presence of casein in the tumours did not appear to be related to the other factors examined.

Diagnosis of gastric cancer.

A prospective comparison was made of the accuracy of different diagnostic methods for gastric cancer. The basis of the study was a consecutive series of 113 patients thought to have gastric pathology; cancer was the final diagnosis in 32. Endoscopy and radiology were the most accurate investigations, whereas biopsy, cytology, and clinical examination gave disappointing results. A wide range of clinical features and laboratory investigations were studied in all patients in an attempt to identify criteria suggestive of malignancy. Multifactorial computer analysis of these investigations failed to improve upon the radiological diagnosis. A systemic approach designed to make optimal use of limited endoscopic and histopathological resources in the diagnosis of gastric lesions is presented.
