Immunological studies of patients with asbestosis. I. Studies of cell-mediated immunity.

A variety of cancers have been documented in patients exposed to asbestos dust. Since a deranged immune system may play a rôle in cancer development, the general level of immunocompetence was studied in a group of twenty-six patients with radiographically defined asbestosis, who might be at risk of developing asbestos-related neoplasms. Statistical comparisons were made with a comparable control group. A disproportionate number of the patients displayed cutaneous energy to certain recall antigens and to 2,4-dinitrochlorobenzene. In vitro studies of cellular immunity, as evaluated by phytohaemagglutinin-induced proliferative and cytotoxicity assays, showed significantly lower values amongst the patient group. Serum inhibitors of mitogen-induced lymphocyte transformation were also detected in several of the patients. The possible significance of these findings is discussed.

The obligations of medical practitioners in relation to the new Mines and Works Act.

The Occupational Diseases in Mines and Works Act No. 78 of 1973 deals with compensation to be paid in cases of pneumoconiosis and other compensatable diseases such as tuberculosis, permanent obstruction of the airways and scleroderma. Any medical practitioner who attends a miner or worker from a 'works' such as Iscor, Amcor and several others at present controlled, is obliged, if death occurs, to remove the cardiorespiratory organs and forward them to the Medical Bureau for Occupational Diseases, unless the near relatives refuse permission for an autopsy. A pathologist who performs a routine autopsy on such a person must follow the same procedure.

The immunology of asbestosis.

Forty-one employees with varying degrees of asbestosis have been tested for any alteration in the immunological profile. Of these, twenty-two employees showed evidence of pleural thickening and nineteen parenchymal asbestotic fibrosis. Those employees showing pleural thickening gave a strong reaction to different skin tests and a few of those with a parenchymal asbestotic fibrosis showed depressed cutaneous reactivity. Lymphocytotoxic antibodies were present in the sera of 60% of those with pleural thickening and 94% of those with parenchymal asbestosis. An immunological screening schedule is suggested for those employees who show pleural thickening.

The ingestion of asbestos fibers.

Feeding of baboons with crocidolite showed small numbers of asbestos needles 0.5-1 mum in ashed tissue of the gut wall, which probably came from iron-containing macrophages. It is suggested that pleural plaques and hyaline nodules in the peritoneum represent a hypersensitivity reaction to ferritin protein coating on asbestos fibers. In South Africa only a few peritoneal mesotheliomas come from the asbestos areas, and the incidence of gastrointestinal carcinomas is no greater than normal. Intrapleural and intraperitoneal injection produces unrealistic situations. Calcium salts are deposited on asbestos cement pipes from hard water and organic material from soft water. It is difficult to envisage asbestos contamination of the water so reticulated.
