More than 1,000 cases of total parathyroidectomy with forearm autograft for renal hyperparathyroidism.

Between March 1981 and December 2000, we performed 1,053 total parathyroidectomies with forearm autograft for advanced renal hyperparathyroidism (HPT). Based on histopathologic and pathophysiologic investigations, surgical treatment should be considered when parathyroid glands show nodular hyperplasia. Measuring parathyroid volume by ultrasonography was useful to detect nodular glands and to determine surgical indications. The clinical effect of parathyroidectomy on the symptoms and biochemical variables was striking. Skeletal deformity, progressive bone loss, and vessel calcification leading to high mortality risk could not be alleviated by even successful surgery, however. To prevent cardiovascular complications, parathyroidectomy should be performed in the relatively early stage of renal HPT. Total parathyroidectomy with forearm autograft is a suitable procedure for renal HPT, especially in patients who require long-term hemodialysis. For surgeons, it is important to remove all parathyroid glands, including supernumerary glands, at the initial operation and to choose adequate parathyroid tissue for the autograft to prevent persistent and recurrent HPT. Although the risk of graft-dependent recurrent HPT is not negligible, enlarged transplanted parathyroid tissue can be removed easily and noninvasively from the forearm under local anesthesia. There is no risk of hypofunction of the autograft.

Osteoprotegerin levels before and after renal transplantation.

Osteoprotegerin (OPG) is a newly identified glycoprotein that belongs to the tumor necrosis factor receptor superfamily and regulates bone mass by inhibiting osteoclastic bone resorption. The regulatory mechanism of OPG is still unclear after successful renal transplantation (RTX), however, resulting in resolution of uremia. The present study was designed to clarify the potential role of OPG in uremia and after RTX under immunosuppressive therapy. We evaluated circulating OPG levels by measuring them before and after RTX (postoperative days 2, 14, and 28). Our protocol of immunosuppressive drugs was dual therapy using cyclosporine and steroids. Serum OPG was quantitated using enzyme-linked immunosorbent assay. After successful RTX, serum OPG levels decreased significantly on day 14 and day 28 compared with the baseline level (P < 0.05). Creatinine clearance dramatically increased until day 14 and decreased thereafter. Serum OPG declines for the first 2 weeks after RTX owing to functioning allograft and decreases again for the next 2 weeks because of steroids and possible immunosuppressive agents.

Inhibitory effects of barbiturates on nicotinic acetylcholine receptors in rat central nervous system neurons.

BACKGROUND: Neuronal nicotinic acetylcholine receptors (nAChRs) are widely expressed in the central and autonomic nervous systems. The authors have previously shown that depressant and convulsant barbiturates both inhibit the ganglion-type nAchRs in PC12 cells. However, the central and gangliontype receptors have different subunit composition and pharmacologic properties. In this study, the authors investigated the effects of thiopental, depressant [R(-)] and convulsant [S(+)] stereoisomers of 1-methyl-5 phenyl-5-propyl barbituric acid (MPPB) on neuronal nAChRs in the rat central nervous system to explore significance of these effects in barbiturate anesthesia. METHODS: Whole-cell currents were measured in acutely dissociated rat medial habenula (MHb) neurons by applying 10 or 100 microM nicotine in the absence or presence of thiopental 3-100 microM. Effects of R(-)- and S(+)-MPPB on the nicotine-induced current were also studied. RESULTS: Thiopental suppressed the nicotine-elicited inward current and accelerated the current decay dose-dependently at the clinical relevant concentrations. R(-)- and S(+)-MPPB both inhibited the nicotine-induced current dose-dependently without augmenting the current decay. There was no significant difference in the magnitudes of inhibition by R(-)- and S(+)-MPPB. CONCLUSIONS: Although thiopental suppressed the current mediated through native nAchRs in rat MHb neurons at the clinically relevant concentrations, the depressant and convulsant stereoisomers of MPPB both inhibited the current in the same extent. These findings are consistent with the results previously obtained in the ganglion-type receptors of PC12 cells and suggest that inhibition of nAChRs in MHb neurons is not directly relevant to the hypnotic or anticonvulsive actions of barbiturates.

Mechanisms of modulation of neuronal nicotinic receptors by substance P and OAG.

Substance P is known to modulate neuronal nicotinic acetylcholine receptors (nAChRs) in the sympathetic nervous system. There are two conflicting proposals for the mechanism of this effect, an indirect action mediated by protein kinase C (PKC) and a direct interaction with receptor subunits. We studied the mechanisms of this effect in PC-12 cells. Substance P enhanced the decay of the nicotine-induced whole cell current. This effect was fast in its onset and was not antagonized by guanosine 5'-O-(2-thiodiphosphate), a G protein blocker, or staurosporine, a nonselective PKC blocker. Staurosporine failed to reverse the inhibition by 1-oleoyl-2-acetyl-sn-glycerol (OAG), a synthetic diacylglycerol analog known to activate PKC. The inhibitory effects of the peptide and OAG were preserved in excised patches, but substance P applied to the extra patch membrane was ineffective in the cell-attached patch configuration. We conclude that substance P modulates neuronal nAChRs most likely by direct interactions with the receptors but independently from activation of PKC or G proteins and that PKC does not participate in modulation by OAG.

Effects of heat treatments on mechanical strength of laser-welded equi-atomic AuCu-6at%Ga alloy.

There is little information available on the mechanical strengthening of laser-welded gold alloys to achieve reliable dental prostheses. This study examined the hypothesis that heat treatments increase the mechanical strength of a laser-welded equi-atomic AuCu-6at%Ga alloy with age "hardenability" at intra-oral temperature. Cut cast gold alloy plates were laser-welded. The specimens were given one of three heat treatments: (1) solution treatment, (2) high-temperature aging after solution treatment, and (3) simulated intra-oral aging after solution treatment. As-cast and uncut specimens were also prepared. Tensile testing was conducted, and the breaking stress and yield strength were recorded. The yield strength values of all the heat-treated specimens nearly reached the values of the corresponding heat-treated control specimens. The results of this study indicated that, for high mechanical strength to be achieved, the laser-welded alloy tested should be aged at a high temperature or be intraorally aged after being laser-welded.

Frequency-dependent electrophysiological effects of flecainide, nifekalant and d,l-sotalol on the human atrium.

To compare the effects of class Ic and III antiarrhythmic agents on the termination and prevention of atrial fibrillation, the present study investigated the use-dependent electrophysiological effects of flecainide, nifekalant and d,l-sotalol on the human atrium. Flecainide significantly prolonged effective refractory period (ERP), intra-atrial conduction time (IACT) and monophasic action potential duration (MAPD), and its effects on ERP and IACT were use-dependent. Nifekalalant significantly prolonged ERP and MAPD, and these effects were reverse use-dependent; however, there was no significant change in IACT. d,l-Sotalol significantly prolonged MAPD and the effect was reverse use-dependent. It significantly prolonged ERP, but the effect was not reverse use-dependent. d,l-Sotalol increased IACT in a use-dependent manner. Thus, for atrial fibrillation, class Ic antiarrhythmic agents might be more effective in termination and class III antiarrhythmic agents might be more effective in prevention.

High-level secretory production of phospholipase A1 by Saccharomyces cerevisiae and Aspergillus oryzae.

Phospholipase A1 (PLA1) is a hydrolytic enzyme that catalyzes the removal of the acyl group from position 1 of lecithin to form lysolecithin. The PLA1 gene, which had been cloned from Aspergillus oryzae, was expressed in Saccharomyces cerevisiae and A. oryzae. Through the modification of the medium composition and the feeding conditions of substrate, the production level of PLA1 by S. cerevisiae was increased to a level fivefold higher than that indicated in a previous report. In the case of A. oryzae, introduction of multicopies of PLA1 expression units, and the morphological change from the pellet form to the filamentous form were effective for the enhancement of PLA1 production. We succeeded in producing 3,500 U/ml of PLA1 using an industrial-scale fermentor.

Age and sex differences in the P-wave signal-averaged electrocardiogram in a Japanese study population.

Normal P wave signal-averaged electrocardiogram (SAE) values were determined in 120 healthy Japanese adults (56 men, 64 women), aged 44.5+/-10.2 years (mean+/-SD). The P wave trigger method was used with a Fukuda FDX6500 recorder. We used bipolar Frank leads (X,Y,Z), and recordings were made with forward and backward digital Butterworth filters [40 Hz (18 dB / oct) - 300 Hz (12 dB / oct)]. The recordings were taken for the following five parameters: forward and backward filtered P wave duration [fPd (F); tPd (B)]; bidirectionally corrected fPd [tPd (C)]; and 20 ms of the terminal portions of voltage at forward and backward filtering (RMS20). Overall, fPd (F) was 117.8-136.4 ms, fPd (B) 116.4-134.4 ms, fPd (C) 97.4-115.2 ms, RMS20 (F) 1.6-3.6 microV, and RMS20 (B) was 2.2-5.4 microV. Between the sexes, there were significant differences in fPd (F) (p<0.001) and fPd (B) (p<0.01) and in RMS20 (F) (p<0.05) and RMS20 (B) (p<0.05). Weak positive correlations were observed between fPd (F) and body surface area, fPd (F) and age, fPd (B) and body surface area, fPd (B) and age, fPd (C) and body surface area, and fPd (C) and age. There was no evident correlation, however, between either forward or backward RMS20 and body surface area or between forward or backward RMS20 and age. Differences in the normal P wave values between the sexes and age groups were evaluated in this study.

Trace element accumulation in hawksbill turtles (Eretmochelys imbricata) and green turtles (Chelonia mydas) from Yaeyama Islands, Japan.

Concentrations of 18 trace elements (V, Cr, Mn, Co, Cu, Zn, Se, Rb, Sr, Zr, Mo, Ag, Cd, Sb, Ba, Hg, Tl, and Pb) were determined in the liver, kidney, and muscle of green turtles (Chelonia mydas) and hawksbill turtles (Eretmochelys imbricata) from Yaeyama Islands, Okinawa, Japan. Accumulation features of trace elements in the three tissues were similar between green and hawksbill turtles. No gender differences in trace element accumulation in liver and kidney were found for most of the elements. Significant growth-dependent variations were found in concentrations of some elements in tissues of green and hawksbill turtles. Significant negative correlations (p < 0.05) were found between standard carapace length (SCL) and the concentrations of Cu, Zn, and Se in the kidney and V in muscle of green turtles and Mn in the liver, Rb and Ag in kidney, and Hg in muscle of hawksbill turtles. Concentrations of Sr, Mo, Ag, Sb, and Tl in the liver, Sb in kidney, and Sb and Ba in muscle of green turtles and Se and Hg in the liver and Co, Se, and Hg in kidney of hawksbill turtles increased with an increase in SCL (p < 0.05). Green and hawksbill turtles accumulated extremely high concentrations of Cu in the liver and Cd in kidney, whereas the levels of Hg in liver were low in comparison with those of other higher-trophic-level marine animals. High accumulation of Ag in the liver of green turtles was also observed. To evaluate the trophic transfer of trace elements, concentrations of trace elements were determined in stomach contents of green and hawksbill turtles. A remarkably high trophic transfer coefficient was found for Ag and Cd in green turtles and for Cd and Hg in hawksbill turtles.

Changes in gait stability induced by alteration of mandibular position.

The purpose of this study was to investigate the effect of different mandibular positions on the body equilibrium by measuring the gait rhythm. Twelve young healthy subjects volunteered to participate in this study. Subjects were instructed to walk for a 18m distance with the mandible guided at six different positions by occlusal splints. The gait was recorded at fast, ordinary and slow walking speeds under occluded condition. Gait cycle, coefficient of variation for gait cycle and gait velocity were calculated during the 10m-walk. One-way ANOVA with repeated measures demonstrated significant differences among the six mandibular positions for the gait cycle at all gait speeds, for the coefficient of variation at fast speed and ordinary speed and for the gait velocity at fast speed (P<0.05). Regarding these variables, multiple comparison tests were also performed between the mandibular position without instruction for occlusion and other five mandibular positions. Gait cycle, coefficient of variation and gait velocity changed significantly in 5mm opening position from the intercuspal position, and in 5 mm left and 5mm right position from the 3mm opening position at fast speed (P<0.05). The result of this study suggests that the change of mandibular position could affect the gait stability.

Simulation of Night Myopia in Pseudophakic Eyes.

Purpose: Eyes with an intraocular lens (IOL) implanted that have no accommodative ability are likely to have night myopia caused by Purkinje's shift and chromatic aberration. We evaluated the changes in retinal images caused by night myopia in various IOL-implanted eyes by simulation using model eyes.Methods: A polymethylmethacrylate (PMMA) IOL, soft acrylic IOLs, and high-refractive-index silicone IOLs were prepared, and inserted into the model eye. The image plane of the model eye was determined as the line image at which a slit light of 560 nm was best focused, which was regarded as emmetropic condition under photopic vision. A slit light of 505 nm was then directed into the model eye. Its line image on the image plane was blurred, which was regarded as myopic condition under scotopic vision. Under each condition. The modulation transfer functions (MTF) were calculated and the line images were photographed.Results: The degree of reduction of MTF and blurring of the line image under the night myopic condition was in the order of PMMA<soft acrylic<high refractive index silicone IOL.Conclusion: Eyes with a soft acrylic or high-refractive-index silicone IOL may have more intense night myopia caused by chromatic aberration.

Shear bond strengths of polymethyl methacrylate to cast titanium and cobalt-chromium frameworks using five metal primers.

STATEMENT OF PROBLEM: Poor chemical bonding of a denture base resin to cast titanium frameworks often introduces adhesive failure and increases microleakage. PURPOSE: This study examined the shear bond strengths of a denture base resin to cast pure titanium, Ti-6Al-4V, and a cobalt-chromium alloy using various adhesive primers. MATERIAL AND METHODS: Disks (6.0 mm diameter, 2.5 mm thick) were cast of the 3 alloys. The disk surfaces were grit-blasted with 50 microm alumina and treated with 5 different metal primers (Metal Primer II ¿MP]; Cesead Opaque primer ¿OP]; Meta Base ¿MB]; experimental primer ¿EP]; Siloc bonding system ¿SI]). A denture base resin (Palapress Vario) was then applied on the disks with hole-punched sticky tape (bonding area of 5.0 mm) and a Teflon (PTFE, New Age Industries Inc, Willow Grove, Pa.) ring (6.0 mm diameter x 2.0 mm thick). Specimens without primer were also prepared as controls. All specimens were immersed in 37 degrees C water and thermocycled up to 2,000 cycles. Shear bond strength values were determined at a crosshead speed of 0.5 mm/min. Data were statistically analyzed using 3-way ANOVA, followed by 1-way ANOVA and the Scheffé multiple range test. RESULTS: Primers significantly (P <.05) improved shear bond strengths of denture base resin to all metals, among which no significant differences were found. Specimens primed with OP, MP, and EP showed higher bond strengths than did those primed with MB. After thermocycling, the bond strengths of MB and SI decreased substantially; MB showed the least durability (22.8% to 35.5% decrease) among the primers. CONCLUSION: The application of 5 primers significantly improved the shear bond strengths of a denture base resin to cast CP titanium, Ti-6Al-4V, and Co-Cr alloy. OP and MP primers exhibited greater bond strength and durability than did MB and SI.

Cutting efficiency of air-turbine burs on cast titanium and dental casting alloys.

OBJECTIVE: The purpose of this study was to investigate the cutting efficiency of air-turbine burs on cast free-machining titanium alloy (DT2F) and to compare the results with those for cast commercially pure (CP) Ti, Ti-6Al-4V alloy, and dental casting alloys. METHODS: The cast metal (DT2F, CP Ti, Ti-6Al-4V, Type IV gold alloy and Co-Cr alloy) specimens were cut with air-turbine burs (carbide burs and diamond points) at air pressures of 138 or 207 kPa and a cutting force of 0.784 N. The cutting efficiency of each bur was evaluated as volume loss calculated from the weight loss cut for 5 s and the density of each metal. The bulk microhardness was measured to correlate the machinability and the hardness of each metal. RESULTS: The amounts of DT2F cut with the carbide burs were significantly (p < 0.05) greater than for the other titanium specimens at either 138 or 207 kPa. The diamond points exhibited similar machining efficiency among all metals except for Type IV gold alloy. The increase in the volume loss of Co-Cr alloy (Vitallium) cut with the diamond points showed a negative value (-29%) with an increase in air pressure from 138 to 207 kPa. There was a negative correlation between the amounts of metal removed (volume loss) and the hardness (r2 = 0.689) when the carbide burs were used. SIGNIFICANCE: The results of this study indicated that a free-machining titanium alloy (DT2F) exhibited better machinability compared to CP Ti and Ti-6Al-4V alloy when using carbide fissure burs. When machining cast CP Ti and its alloys, carbide fissure burs possessed a greater machining efficiency than the diamond points and are recommended for titanium dental prostheses.

The machinability of cast titanium and Ti-6Al-4V.

This study investigated the machinability (ease of metal removal) of commercially pure (CP) titanium and Ti-6Al-4V alloy. Both CP Ti and Ti-6Al-4V were cast into magnesia molds. Two types of specimens (with alpha-case and without alpha-case) were made for CP Ti and Ti-6Al-4V. Machinability (n = 5) was evaluated as volume loss (mm3) by cutting/grinding the 3.0 mm surface using fissure burs and silicon carbide (SiC) under two machining conditions: (1) two machining forces (100 or 300 gf) at two rotational speeds (15000 or 30000 rpm) for 1 min, and (2) constant machining force of 100 gf and rotational speed of 15000 rpm for 1, 2, 5, 10, and 30 min. As controls, conventionally cast Co-Cr and Type IV gold alloys were evaluated in the same manner as the titanium. When fissure burs were used, there was a significant difference in the machinability between CP titanium with alpha-case and without alpha-case. On the other hand, there was no appreciable difference in the amount of metal removed for each tested metal when using the SiC points.

Cardiopulmonary cerebral resuscitation using emergency cardiopulmonary bypass, coronary reperfusion therapy and mild hypothermia in patients with cardiac arrest outside the hospital.

OBJECTIVES: The purpose of this study was to evaluate the efficacy of an alternative cardiopulmonary cerebral resuscitation (CPCR) using emergency cardiopulmonary bypass (CPB), coronary reperfusion therapy and mild hypothermia. BACKGROUND: Good recovery of patients with out-of-hospital cardiac arrest is still inadequate. An alternative therapeutic method for patients who do not respond to conventional CPCR is required. METHODS: A prospective preliminary study was performed in 50 patients with out-of-hospital cardiac arrest meeting the inclusion criteria. Patients were treated with standard CPCR and, if there was no response, by emergency CPB plus intra-aortic balloon pumping. Immediate coronary angiography for coronary reperfusion therapy was performed in patients with suspected acute coronary syndrome. Subsequently, in patients with systolic blood pressure above 90 mm Hg and Glasgow coma scale score of 3 to 5, mild hypothermia (34 C for at least two days) was induced by coil cooling. Neurologic outcome was assessed by cerebral performance categories at hospital discharge. RESULTS: Thirty-six of the 50 patients were treated with emergency CPB, and 30 of 39 patients who underwent angiography suffered acute coronary artery occlusion. Return of spontaneous circulation and successful coronary reperfusion were achieved in 92% and 87%, respectively. Mild hypothermia could be induced in 23 patients, and 12 (52%) of them showed good recovery. Factors related to a good recovery were cardiac index in hypothermia and the presence of serious complications with hypothermia or CPB. CONCLUSIONS: The alternative CPCR demonstrated an improvement in the incidence of good recovery. Based upon these findings, randomized studies of this hypothermia are needed.

Corrosion casts of young rabbit palatine mucosa angioarchitecture.

The microvascular system arrangement of the palatine mucosa in young rabbits was studied using vascular corrosion casts. Specimens were obtained by injecting low viscosity resin. Superficial microvessels were observed mainly by scanning electron microscopy (SEM). The blood microvessels showed differing features in the areas considered palatine rugae, inter-rugal areas and posterior region of the palatine raphe. Larger vessels were noted in deeper layers. Several capillary loops were distributed all over the palatine rugal surface. In spite of being a young animal, the rabbit's palatine mucosa demonstrated a complex angioarchitecture.

Infrared and XAFS study on structure and transition behavior of zinc stearate.

Structure and transition behavior of zinc(II) stearate crystal were investigated by infrared and XAFS spectroscopies. Structure of zinc stearate at room temperature was estimated as follows. From XAFS analysis, the coordination number of the carboxylate groups around the zinc atom was evaluated as 4 and the Zn-O distance as 1.95 A. Based on the infrared spectrum and a normal mode analysis, the conformation of the alkyl chain was confirmed as all-trans and the sub-cell packing was considered as parallel type, and also the coordination form of the carboxylate groups was determined as bridging bidentate type. As increasing temperature, zinc stearate has a solid liquid phase transition at 130 degrees C. At the transition, the alkyl chains goes into liquid like state as reported by Mesubi but the coordination structure was confirmed to be maintained.

Long-lasting effect evoked by tonic muscle pain on parietal EEG activity in humans.

OBJECTIVE: To explore EEG changes evoked by tonic experimental muscle pain compared to a non-painful vibratory stimulus. METHODS: Thirty-one EEG channels were recorded before, during and after painful and non-painful stimulation. Pain was induced in the left brachioradialis muscle by injection of hypertonic (5%) saline. The vibratory stimulus was applied to the skin area overlying the brachioradialis muscle. The power of the major frequency components of the EEG activity (FFT, fast Fourier transform) was quantified and t-maps between the different experimental conditions were evaluated in frequency domain. RESULTS: The main effect of muscle pain, compared to non-painful stimulation, was a significant and long-lasting increase of delta (1-3 Hz) power and an alpha-1 (9-11 Hz) power increase over the contralateral parietal locus. This finding could suggest a decreased excitability of the primary somatosensory cortex during muscle pain. The main effect of vibration, compared to its unstimulated baseline, consisted in an increase of beta-1 (14-20 Hz) power in the right frontal region. CONCLUSIONS: Our data demonstrate significant and specific topographic EEG changes during tonic muscle pain. Since these modifications differ from those produced by an unstimulated baseline and during non-painful tonic stimulation, they might reflect mechanisms involved in the processing of nociceptive and adverse tonic stimuli.
