Changes in cell shapes and cytokeratins of epithelial cells during the infiltration of lymphocytes in the human palatine tonsils.

By the immunohistochemical method using anti-keratin antibodies (KL1-antigen, 56 KD keratin; PKK1-antigen, 40-52.5 KD keratin) and electron microscopy, the changes in cell shapes and the cytokeratin components of the epithelial cells during the infiltration of lymphocytes were studied. In the surface epithelium, PKK1 reacts with only the keratinocytes in the basal layer, while KL1 stains in the spinous layer. In the neck portion of the crypt, transformed keratinocytes scattered in the spinous layer react intensely with PKK1. These cells issue prolonged cytoplasmic processes which surround the cavities filled with infiltrating lymphocytes. In the deep portion of the crypt, PKK1-positive cells interconnect with one another constituting the network of the star-shaped reticular cells. The enlarged intercellular spaces of the reticular cells are filled with many lymphocytes. Our observations suggest that the infiltration of lymphocytes into the crypt epithelium induces the change in keratin expression of the epithelial cells in the human palatine tonsils, and the change of keratin molecules makes the stratified flattened epithelium to transform into the network of star-shaped reticular cells.

Clinical evaluation of medical treatment for Menière's disease, using a double-blind controlled study.

There are great variations in the clinical evaluations of the effectiveness of antivertiginous drugs for Meniere's disease. In our clinical evaluation, subjective and objective signs and symptoms of 128 patients with Meniere's disease and of 98 with other peripheral vestibular disorders were analyzed in a double-blind controlled study comparing adenosine triphosphate (ATP) (300 mg) and betahistine (36 mg) each given daily for 4 weeks. The method of administration was the matched-pair-group-method for this double-blind controlled trial. The attending physicians concluded that ATP was significantly more effective than betahistine in the treatment of Meniere's disease and other peripheral vestibular disorders.

[Four cases of mitral valve prolapse associated with ventricular tachycardia].

Four patients with mitral valve prolapse and ventricular tachycardia or fibrillation were described. Case 1 was a 46-year-old man with syncope due to ventricular fibrillation. Case 2 was a 36-year-old woman with palpitation and dizziness due to ventricular tachycardia. Case 3 was a 47-year-old man with palpitation and dizziness due to ventricular tachycardia. Case 4 was a 38-year-old man with syncope due to torsade de pointes. Left ventriculograms showed mitral valve prolapse in all cases. Coronary arteriograms were also normal, supporting a noncoronary etiology of the arrhythmia. Patients with unexplained ventricular arrhythmias should be screened for mitral valve prolapse.

Systemic talc granulomatosis associated with disseminated histoplasmosis in a drug abuser.

A 32-year-old intravenous drug abuser was found to have systemic talc granulomatosis and disseminated histoplasmosis. The clinicopathologic findings of this case support the hypothesis that the patient was predisposed to disseminated histoplasmosis by repeated intravenous talc administration. The effects of silica, a close relative of talc, on macrophages and the role of macrophages in recovery from Histoplasma capsulatum infection are described.

Ultrastructural and immunohistochemical investigations of the giant cells in nodular fasciitis.

The giant cells of histologically typical cases of nodular fasciitis were studied by electron microscopy and immunohistochemistry. Ultrastructurally, these cells were found to be similar to the adjacent spindle and large fibroblast-like cells in many ways. These giant cells exhibited numerous intracytoplasmic, well-formed longitudinally oriented bundles of myofilaments and hemi-desmosome-like structures with associated basement membrane-like material, abundant endoplasmic reticulum, and numerous dense bodies. The observed myofilaments did not show beading or Z-line formation. Immunohistochemical staining by means of the indirect peroxidase-antiperoxidase technique failed to localize the proven antigenic markers of histiocytes within the giant cells. The results support the concept that the giant cells in nodular fasciitis are of myofibroblastic origin. It is thus proposed that these giant cells are neither histiocytes nor rhabdomyoblasts as previously thought, but are rather modified fibroblasts ("myofibroblasts").

Stage I and II Waldeyer's ring and oral-sinonasal non-Hodgkin's lymphoma.

Sixty-six patients with Ann Arbor Stage I and II Waldeyer's ring and oral-sinonasal non-Hodgkin's lymphoma are presented. Ten-year survival was better for the 32 patients with Waldeyer's ring non-Hodgkin's lymphoma (Stage I, 83%; Stage II, 75%) than for the 34 with oral-sinonasal non-Hodgkin's lymphoma (Stage I, 47%; Stage II, 50%). Diffuse large cell lymphomas were common in patients with Waldeyer's ring involvement (59%). In those affected in the oral-sinonasal region, 38% had high-grade lymphoma. There was a high incidence of extranodal relapses outside of the gastrointestinal tract in patients with oral-sinonasal lymphoma (10 cases). Gastrointestinal tract relapse occurred commonly in patients with Waldeyer's ring lymphoma and was found in five cases.

Cholinesterase activity of lamellated sensory corpuscles in the rat lip.

Non-specific cholinesterase (ChE) activity was demonstrated in lamellated sensory corpuscles of the rat lip by light and electron microscopy using Karnovsky and Roots' method. ChE activity was present in the interlamellar spaces between neighbouring lamellae as well as in the peri-axonal space between axon terminals and their adjacent lamellae. Reaction products were also deposited in some caveolae of the lamellar cell plasma membrane, and in the cisternae of the rough endoplasmic reticulum as well as in the nuclear envelope of lamellar cell bodies. No reaction products were detected within the axon terminals. The findings show that the lamellated corpuscles in the rat lip, like other mechanoreceptors, have intense ChE activity which is probably synthesized in lamellar cells and released into the intercellular spaces. That ChE activity is particularly strong in mechanoreceptors composed of lamellar cells suggests that this enzyme would play an important role in function and/or maintenance of such mechanoreceptive corpuscles.

Histologic characteristics of gonadotropin-producing hepatoblastoma: a survey of seven cases from Japan.

Histologic studies of the human chorionic gonadotropin (hCG) producing hepatoblastoma were made for seven cases encountered in Japan. It was found in all cases that multinucleated tumor giant cells in several figures were admixed in usual hepatoblastoma tissue and one of them was confirmed to be hCG positive immunohistochemically. Round and clear cells adjacent to or separate from the giant cells showed squamoid metaplasia with occasional pearl formation. These histologic elements were highly suspected to be derivatives of choriocarcinoma and were ascertained to be characteristic of the hCG-producing hepatoblastomas. Only one among seven cases survived long after hepatic lobectomy. At the time of recurrence, none showed elevation of serum or urinary hCG level, while serum alpha-fetoprotein (AFP) level became high again. That was because recurrent tumors contained none of the characteristic elements that had been found in preceding tumors.
