[Problems of the organization of outpatient cardiac consultation services for patients with cardiovascular diseases in a large city].

The experience gained in the study of outpatient consultative cardiac care in a big city such as Leningrad permits one to single out the following stages in the delivery of these services: a polyclinic-based cardiologist--a cardiologist in the consultative-diagnostic polyclinic and (or) an out-patient department at the Research Institute of Cardiology (follow-up centre)--urban specialized centres of consultative care. The main issues requiring improvement in this system are: the necessity of efficient regulation of indications for consultative care at all stages, determination of functional responsibilities of consulting physicians, their role in the system of dispensarization, the drafting of optimum documentation and criteria of performance.

[Cardiological service: essential components and functions].

A system of cardiological service consisting of five essential components is described: profile of diseases (the object of care and the subject of distributing labour between physicians); qualification of a cardiologist; continuity of care; qualification of a therapist; management. Four main groups of typical targets for cardiological service units are identified. For the purpose of evaluating the performance of the unit of this medical service in dynamics it is suggested to consider it as consisting of three elements (therapist-patient-cardiologist), and the quality of work of each physician as consisting of four interrelated blocks of operations: to assess the health status and diagnose the disease, to make a diagnosis and register it officially, to choose and prescribe treatment, to determine the place for further treatment and follow-up. The data provided could be used for promoting the structure of cardiological service and its components and for choosing the unified method for their further investigation.

[Experience in the organization of heart surgery services to patients with disorders of heart rhythm and conduction in large cities].

The article generalizes 17-year experience in the organization of surgical service for patients with disturbed rhythm and conductivity of the heart in Leningrad and the northwest region of the USSR. The work of the city center of surgery of arrhythmias is analysed. Prospective calculations were made. Basically new approaches to determining the regular number of persons on the staff and the number and type or operative interventions are suggested.

[Evaluation of factors influencing the formation of personal qualities in nurses].

The article provides a socio-psychological evaluation of factors influencing the formation of personal qualities of nurses. For the purpose of screening diagnosis of professionally conditioned deformation of personal qualities the following semantic variables were chosen: a capacity for initiative, discipline, honesty, sympathy, communicativeness, discretion. The influence of nurses' age, specialization, place of work, family status on self-evaluations is traced. Factors are identified which influence the working stability of medium-level personnel under conditions of major curative and preventive institutions.

[Organization of the outpatient referral service for patients with disorders of cardiac rhythm and conductivity requiring surgical treatment].

Under analysis were results of the outpatient aid to patients in the center of surgery during 5 years. Every year the consultative and ambulatory aid was given to 1200 patients. A conclusion was made that organization of the city consultative-diagnostic center for patients requiring surgical treatment of arrhythmia with a special staff and equipment for performing electrophysiological and other investigations, organization of the zone consultation units in large polyclinics for electrophysiological investigations will improve the quality and efficiency of treatment of patients with complex disorders of the heart rhythm and conduction.

[Hospital aid to patients with bronchial asthma in big cities].

This paper provides the results of the analysis of 3013 hospital admissions for bronchial asthma. The hospitalized incidence of bronchial asthma under conditions of big industrial cities is 5.5 per 10,000 adult population. Among measures of primary and secondary prevention of bronchial asthma in adult population-residents of big cities, priority should be given to the adequate treatment of pre-asthmatic conditions. The number of beds utilized for hospitalization of patients with bronchial asthma is 0.6 per 10,000 adult population per year.
