[Clinicodiagnostic significance of gastroesophageal reflux and structural changes in the esophagus and stomach in bronchial asthma].

Four hundred patients with bronchial asthma (BA) were questioned. Complex clinical and instrumental examination of 60 patients with BA and endoscopic-positive gastroesophageal reflux disease was carried out; the control groups included 30 patients with BA alone and 30 patients with GERD alone. Clinical symptoms of GERD were revealed in 58% of BA patients; endoscopic-positive GERD was diagnosed in 48.6% of BA patients. The study proved the connection between the degree of respiratory disturbances, on the one hand, and the degree of esophagitis and the level of intraesophageal pH, on the other, in patients with BA plus GERD. The study also established the role of esophageal and gastric epitheliocytes, producing endothelin-1, melatonin, NO-synthase, and calretinin, in the occurrence of mutual exacerbation syndrome.

[Prognostic factors of efficacy of eradication therapy in patients with duodenal ulcer].

The aim of the study was to determine microorganism-associated predictors of efficacy of eradication therapy in patients with Helicobacter pylori-associated duodenal ulcer. The subjects were 129 such patients. Clinical, endoscopic, microbiological, and immunological examination revealed differences in the initial immune status, the structure of gastroduodenal zone mucosa, and large bowel biocenosis, which predict efficacy of eradication therapy.

[Gastroesophageal reflux and the degree of esophagitis in patients with coronary heart disease: impact on myocardial repolarization parameters and cardiac rhythm variability].

The specific features of myocardial repolarizing processes and heart rate variability (HRV) were studied in 67 patients with coronary heart disease (CHD) concurrent with gastroesophageal reflux disease (GERD) (a study group), in 72 patients with CHD and in 46 patients with GERD (comparison groups). Patients with concomitant diseases showed a significant asynchronism of repolarizing processes and decreased HRV. A relationship was established between the presence and magnitude of inflammatory lower esophageal third changes, and the severity of myocardial repolarizing processes, and decreased HRV in patients with CHD + GERD.

[Features of the clinical course and some electrophysiological characteristics of the myocardium in patients with combined gastroesophageal reflux and ischemic heart disease].

We investigated the course of myocardial repolarization processes and cardiac rate disturbances in patients with gastroesophageal reflux disease (GERD) combined with ischemic heart disease (IHD). It was established that among patients with IHD GERD is revealed more often in patients with unstable angina. Asynchronism of myocardial repolarization processes is much higher in patients with combined pathology than in patients with GERD or IHD only, which indicates a higher risk of fatal arrhythmias and cardiac deaths in patients of this group.

[Apudocytes and mast cells in chronic inflammation of colon: clinicomorphological correlations].

AIM: To study functional morphology of a total population of endocrine cells of colon mucosa, mast and enterochromaffine cells. MATERIALS AND METHODS: Light and electrone microscopy, immunohistochemical methods, morphometry were used to study endocrine and mast cells of the sigmoid colon in inflammation. RESULTS: Changes of functional morphology and size of endocrine and mast cell population as well as apudocytes producing serotonin, melatonin, vasointestinal peptides were stated. Apudocyte and mass cell functional morphology, clinical symptoms and mucosal structural changes correlated. Specificity of some parameters in Chron's disease is shown. CONCLUSION: The results may provide additional criteria in diagnosis of different variants of chronic colitis.

[Melatonin and serotonin in inflammatory colon diseases and colorectal neoplasms].

Patients with inflammatory intestinal diseases and colorectal adenocarcinoma in sigmoid mucosa were examined using light and electron microscopy, immunohistochemical tests, radioimmunoassay and morphometry. Functional morphology of the general population of the endocrine cells and apudocytes producing melatonin and serotonin as well as urine excretion of 6-sulfatoxymelatonin changed in quality and quantity. The parameters showed specificity depending on clinicomorphological variant of colorectal pathology. The findings provide additional criteria in diagnosis and prediction of the course of different types of inflammatory and tumor affections of the colon.

[Changes in the APUD system of the large intestine as a risk factor for colorectal cancer].

A common population of endocrine cells, apudocytes producing serotonin, melatonin, vaso-intestinal peptides and mast cells were examined on unaffected mucous membranes of 27 colorectal cancer patients. Light and electron microscopy, immunohistochemical and morphometric methods were used. A marked decrease in total number of apudocytes, hypoplasia of cells producing serotonin, melatonin, vaso-intestinal peptides were revealed at early stages of cancer irrespective of the tumor structure. Mast cells in premetastatic period outnumbered those after emergence of metastases. The results obtained may serve as additional criteria of early colorectal cancer diagnosis.

[Analysis of genetic variability of strains of tick-borne encephalitis virus by primary structure of a fragment of the membrane protein E gene].

Primary structures of gene fragments of E protein (160 n.b.) have been determined for 29 tick-borne encephalitis (TBE) strains isolated from different parts of a territory. Analysis of homology of nucleotide sequences of these strains and data on 6 TBE strains published by other authors showed that they can be divided into 6 groups (genotypes) by the following gene typing criteria: strain structure within the genotypes differing by no more than 9%, differences between strains of different genotypes are at least 12%. Based on these criteria, the prototype strains of the Far Eastern antigenic variant (Sofyin), Central European antigenic variant (Neudoerfle), and Vergina strain form different genotypes 1, 2, and 6, respectively. East Siberian strain Aina and Ural Siberian strain Lesopark-II belong to the same TBE virus genotype 3; two-thirds of analyzed strains belong to this genotype. Genotype 4 is represented by one strain 178-79, and genotype 5 by strain 886-84, both isolated in East Siberia.

[Specific determination of flaviviruses by molecular hybridization with synthetic deoxyoligonucleotide probes].

Molecular probes were designed for the purpose of specific determination of flavioviruses transmitted by the ticks of tick-borne encephalitis (TBE) and Omsk hemorrhagic fever (OHV) as well as by mosquitoes of Japanese encephalitis (JE), North Nile (NN), Murrey Valley encephalitis (MVE), Saint-Lois encephalitis (SLE), dengue 1-4 and of yellow fever (YF). The probes are synthetic deoxyoligonucleotides with the 18-20 long basis and complementary for the RNA fragments defined by computer analysis. The thus obtained probes, which specifically hybridize themselves with the sets of the TBE virus or of the OHV virus and do not hybridize themselves with other TBE viruses' sets. Group-specific probes for YE and dengue viruses as well as virus-specific probes, which are able to detect each of the above viruses without any cross effects, were suggested for indexing and identifying the flaviviruses transmitted by mosquitoes.

[The geographical distribution of genetic variants of the tick-borne encephalitis virus].

Geographic distribution of 185 tick-borne encephalitis (TBE) virus strains isolated in 8 physico-geographic areas and classified into six genetic variants was analysed. The strains of genetic variant I homologous to the Sophyin prototype strain were found to occur predominantly in the Far East and also frequently found in Western and North-Western parts of the East European plain. The vast territories from lake Baikal in the East to Ukraine in the West harbor mostly the strains significantly different from the Far-Eastern Sophyin strain. Hybridization experiments with oligonucleotide probes specific for the Neudorffle strain showed that the strains genetically similar to the virus of central European encephalitis occurred also in Eastern Europe and Western Siberia. It is concluded that a relationship exists between genetic types of TBE virus and their geographic origin.

[2. Diagnostic resources and clinical indications].

Preliminary investigations, dedicated to the diagnostic application of three-dimension ultrasonic angioreconstruction, confirmed the feasibility of using the above method in ophthalmologic practice in cases of suspected intraocular and orbital neoplasms. 32 patients (37 eyes) with various pathologies of the eye and orbital cavity, including choroidal melanoma, facial angiomatosis, retinal detachment, chronic uveitis etc.) were examined. The results are indicative of that the three-dimension ultrasonic angioreconstruction makes it possible to detect the latent vascular neoplasms, which cannot be defined by the standard visual and ultrasonic examinations, and the dislocation of structural elements of the eyeball tunics. Besides, it provides for evaluating their special localization and their interrelations with the neighboring anatomic elements.

[Spatial ultrasound visualization of the eye and orbit].

The paper presents data on the diagnostic use of three-dimensional (3-D) ultrasound visualization, analyzes earlier existing and new ultrasound diagnostic technologies, and shows it expedient to apply them in ophthalmologic practice. A hundred and forty-six healthy individuals and 52 patients (60 eyes) with different diseases of the eye and orbital cavity (chorioidal melanoma, facial angiomatosis, retinal detachment, vitreoretinal changes, etc.) were examined. The results of the studies and the analysis of the data available in the literature on this problem allow the authors to state that 3-D ultrasound diagnostic technologies can detect occult masses undetectable during conventional visual and ultrasound studies and define the topography of the dislocation of structurally complicated changes in the tunics of the eyeball.

[Clinical and laboratory characterization of Ixodes tick-borne borreliosis in the Baikal area].

Materials on Ixodes tick-borne borreliosis (ITB) for the period of 2000 - 2003 (i.e. 1,222 blood serum samples from patients with acute ITB and 629 patients with chronic ITB) were analyzed. The proportion of seropositive samples among those obtained from patients with acute and chronic ITB was determined with the use of the indirect immunofluorescence test. In addition, data on different clinical forms of the disease were presented. A high percentage of neurological manifestations in the early period of ITB (31%) and in the late period of the disease (72.8%) was noted. The conclusion was made concerning the necessity of using several diagnostic methods for more exact determination of the structure of the disease.

[Morphological rationale for autografting of musculus rectus ocularis fibers at revascular optic nerve decompression surgery].

The authors made a morphological study of the early proposed and clinically tested revascular optic nerve decompression operation developed to improve blood supply to the initial portion of the optic nerve in glaucoma-induced optic neuropathy. An experiment was carried out on 11 rabbits. The operated and control eyes were enucleated 1, 4, and 6 months after surgery. The experiment has established that muscle autografting into the retroscleral and paraneural space of the optic nerve is followed by a number of cellular reactions with the active participation of satellite cells, some of which may be associated with regenerative processes and presumably regarded as one of the cell autotherapy modalities.

[Clinical observation periods for diphyllobothriasis patients depending on the invasion period].

Discrepancy between the terms of clinical observation and normalization of gastric functions was established on the basis of a time-course study of gastric secretory function in 189 diphylobothriasis patients examined before the treatment and 1-2, 5-6, 10-12 months after it. Recovery of the secretory function took less than 5-6 months in people recovered from short-term invasion (less than a year), while in patients recovered from long-term invasion (lasting more than a year) it took 10-12 months, and reduced secretory activity in the latter group was still observed in many cases 10-12 months after treatment. Results of the study may be used in optimization of terms and forms of clinical observation.
