Vesico-ureteral reflux and uninhibited neurogenic bladder.

Ureteral reflux in female patients who also have latent uninhibited neurogenic bladder occurs synchronously with uninhibited contraction, is blocked by buscopan and is aggravated by besacholine. Intrinsic neuromuscular antireflux mechanism of the vesicoureteral junction (VUJ) appears to be affected by unknown ways with uninhibited contraction. Urethrovesical reflux observed synchronously with uninhibited contraction seems to play an additional role in the genesis of urinary infection. A different look is required in the management of these refluxes, in that concomitant use of the anticholinergic drugs is recommended in addition to the conventional approach for the control of infection, reflux and voiding symptoms.

A spontaneous vesico-ectopic ureterovaginal fistula in a girl.

A case of a spontaneous vesicovaginal fistula through an ectopic ureteral stump in an 8-year-old Japanese girl is reported. A simple nephrectomy had been done when she was 3 years old. The genesis of this fistula is discussed and the diagnostic significance of vaginoscopy is emphasized.

The mechanism of urinary continence after prostatectomy.

An anatomical and neuropharmacological study of the urethra was performed in 19 patients after prostatetectomy to investigate the mechanism of continence. Anatomically, the urethral segment responsible for the postprostatectomy continence was located distal to the open prostatic fossa in the external sphincter which is comprised of intrinsic striated muscle and is called arbitrarily rhabdosphincter by the author. This unique striated muscle in the rhabdosphinecter appears to be primarily under the alpha-adrenergic influence in view of the change in the urethral pressure profile observed following alpha-adrenergic drugs.

Synthesis of alpha-fetoprotein and some other serum proteins in testicular tumors.

Serum levels of alpha-fetoprotein were determined in 33 patients with testicular germ cell tumors and were normal in 11 patients with seminoma and in one patient with matured teratoma; high levels were observed in 19 of 21 patients with embryonal carcinoma, teratocarcinoma, or a mixed type of these germ cell tumors. Tissues from the testicular germ cell tumors were cultivated with 14C-labeled leucine. After incubation, the culture media were subjected to immunoelectrophoretic and autoradiographic analyses. The results were: (i) Radioactive alpha-fetoprotein, albumin, transferrin, and alpha1-globulin appeared in the culture media of embryonal carcinomas obtained from two infants. (ii) Radioactive albumin and alpha1-globulin appeared in the culture media of a mixed type tumor metastasized from testis to retroperitoneal region. (iii) No such radioactive proteins appeared in the culture media of primary seminomas.

Alpha-fetoprotein synthesis in tissue culture of human testicular tumors and an examination of experimental yolk sac tumors in the rat.

The slices of testicular tumors isolated from patients with high serum alpha-fetoprotein (AFP) levels were cultivated in vitro, and the synthesis of some serum proteins, including AFP, was demonstrated. The tumors were produced from yolk sac in rats by fetectomy. The tumors consisted of the various histological structures, such as of teratoma, yolk sac tumor, and very immature tumor. The tumors with yolk sac histology were able to produce AFP. The tumors induced in rats were transplantable, but the well-differentiated histology disappeared after transplantation. The capacity of AFP synthesis in transplanted tumors seemed to be maintained for generations in female rats but not in male rats.

Transitional cell carcinoma of renal pelvis in an infant.

A case of transitional cell carcinoma of the renal pelvis in a 4-month-old japanese boy is reported. This patient is the youngest one described in the literature with a tumor of this type. The differential points of Wilms tumor and other types of neonatal renal tumors are discussed.
