The use of polybrene for heparin neutralization in protein C activity assay.

The protein C activity assay of Francis and Patch (Thromb Res 1983; 32: 605-613) is based on the prolongation of the activated partial thromboplastin time in the presence of activated protein C isolated from the test samples. The assay was modified and standardized by Rapaport et al. (Am J Clin Pathol 1987; 87: 491-497), but could still only be used in patients on heparin therapy after chromatographic removal of the heparin. In this study we attempted to eliminate the heparin separation step without losing the advantages of the modified (Rapaport) method. Heparin was added to the isolated protein C to obtain a rapid and complete antithrombin effect after the thrombin activation step and polybrene was subsequently used to neutralize the excess heparin. Using this modified assay protein C activity ranged from 67 to 133% in the normal population, and from 9 to 25% in coumarin-treated patients. Precision of the modified method was acceptable in both normal and pathological PC ranges: within- and between-batch variations were 5.6 and 3.6%, and 8 and 14%, respectively. The assay correlated well (r = 0.84) with the ELISA technique in both healthy donors and non-coumarin-treated patients.

[Wiskott-Aldrich syndrome in a hetrozygous woman].

The authors report a case of a 20-year old woman, with all the characteristics of the Wiskott-Aldrich-syndrome. The main signs of the disease were recurrent infections, eczema, thrombocytopenia and mild mucosal bleeding. The size of platelets was reduced. Storage pool disease was based on the abnormal platelet aggregation and the lack of dense bodies shown by electronmicroscopic examination. The disturbances of the lymphocytes, neutrophils and monocytes, which resulted in immunodeficiency, could be proven by the immunological tests. This X-linked recessive disorder appeared with typical signs in a heterozygous carrier woman. According to our knowledge, no similar case has been reported before.

Congenital factor XIII deficiency with multiple benign breast tumours and successful pregnancy with substitutive therapy. A case report.

A 34-year-old woman with congenital factor XIII (FXIII) deficiency and multiple connective tissue tumours is reported. The subunit a of FXIII was totally absent in her plasma, platelets and histiocytes of breast fibroadenomas and considerably reduced in the monocytes (below 5%). The plasmatic level of subunit b was also reduced (25%). She had a bleeding tendency and habitual abortions. Fresh frozen plasma therapy permitted a successful pregnancy.

Heterogeneous regeneration of motor fibers following union of the ventral and the dorsal roots in frogs.

In frogs, the dorsal and ventral roots of one of the large lumbar segments (8th or 9th) of the spinal cord were severed and the central stumps were united in order to let motor fibers of the ventral root regenerate into the spinal cord via the dorsal root. After a survival period of 7 to 30 weeks, regeneration was tested in stimulation experiments and in histological investigations. Stimulation experiments involved recordings of field potentials and ventral root potentials evoked by stimulation of the regenerated dorsal root. Appreciable responses could be recorded only from a fraction of the animals investigated. Histological investigation following cobalt labelling of the united dorsal root revealed the presence of a significant amount of regenerated fibers. The arborization pattern of some regenerated fibers resembled the muscle end-plate formation. The results indicate that cholinergic motor fibers are able to replace, to a limited extent, dorsal root fibers. This suggests that there is not a strict receptor-nerve specificity in the sense that a synaptic site can be exclusively stimulated by the appropriate mediator, but rather a preferential selection for the appropriate connection. This preference is higher when both the morphology and the chemistry of the presynaptic structure match with the postsynaptic medium.
