Some aspects of phlebographic technique.

When contrast medium is infected into an artery it flows to0wards the branches distally to the injectionsite. If, however, the contrast medium is injected into a vein the flow of the contrast medium usually has several routes to strive toward the right atrium. The flow in the veins may be affected by using e.g. compression, Valsalva manouore or gravity. By taking these aspects in the technique into consideration the diagnostic value of the phlebography can in many cases be improved.

Cineangiographic study of the venous drainage of the femoral neck in children.

The purpose of the study was to examine the venous flow of the femoral neck using cineradiography, paying special attention to various draining routes and their sensitivity to disturbances. The cine technique served to show that changes seen in various diseases are not artefacts caused by changes of the position of the hip. In normal hips the preferred route for contrast medium was the medial circumflex vein. In internal rotation, flow through the gluteal vein was most easily disturbed, but contrast medium did not flow to the diaphysis. In hips with Perthes' disease, drainage through the veins of the neck was disturbed, and contrast medium flowed to the diaphysis. In postoperative hips the dominating draining route was the lateral circumflex vein.

Lymphographical findings in disseminated tuberculosis without radiological lung manifestations.

Lymphographical findings are presented in three patients with verified tuberculosis. The diagnosis of tuberculosis was difficult in these patients because there were no actual changes in chest radiographs and the clinical findings were generally nonspecific. In each patient, lymphographic changes in lumbar lymph nodes were established. These changes corresponded to those described in the literature in connection with tuberculosis. Thus, the lymphographical findings can here be considered to be a further criterion for the diagnosis of tuberculosis. Lymphographical follow-up also proved beneficial in the follow-up of treatment results in one patient. Suspected tuberculosis, in which local findings cannot be made available by other means, can be regarded as one of the indications for a lymphography.

Characteristics of hypernephromas as seen with ultrasound and computed tomography.

The structure of 20 hypernephromas as seen with ultrasound (US) and computed tomography (CT) were compared. The hypervascular and necrotic areas, as well as the areas of normal tumoral enhancement in CT, were seen as markedly, moderately, or minimally echogenic or had mixed areas of minimal and marked echogenicity in US.

Image technique optimization in MR imaging of a titanium alloy joint prosthesis.

The purpose of this study was to explore systematically the effect of the imaging parameters changeable by the user in spin-echo (SE) imaging sequences to minimize image distortion when imaging joint prostheses. A titanium alloy hip joint prosthesis was studied at I.O.T. The SE imaging parameters were bandwidth/pixel(BW/ p), TE, strength of encoding gradients (matrix size), echo train length (ETL), and direction of phase and frequency encoding. The effect of ETL in rapid acquisition relaxation enhanced (RARE) sequences was also evaluated with a turbo-SE sequence using a different ETL with the same TR and an effective TE. It is concluded that an optimized image quality can be achieved in SE imaging by using a high bandwidth/pixel value (at least 130 Hz/pixel), a high resolution matrix (256-512), sequences with multiple refocusing, and a frequency-encoding axis parallel to the long axis of the prosthesis. The degree of distortion is reduced with this optimized technique.

Narcotic smuggling and radiography of the gastrointestinal tract.

The gastrointestinal tract is being used to an increasing extent as a route for smuggling narcotics. Small, swallowed packages overwrapped with condoms or other materials are usually not detected by the customs authorities. Conventional abdominal radiography may indicate foreign bodies surrounded by a characteristic thin layer of gas and located in the gastrointestinal tract. Some short case histories, and the radiologic findings in four heroin 'body packers' are presented.

Ultrasound examination of foreign bodies. An in vitro investigation.

Ultrasound examination of foreign bodies most frequently encountered in the human body was performed when they were contained within a transmission medium acoustically resembling human soft tissues. All the foreign bodies evaluated (lead and plastic pellets, pieces of wire, nails, needles, small fragments of rock and glass, wooden slivers, surgical sponges and surgical threads) were detectable with ultrasound. Strong echoes were reflected from their surface when they were positioned favourably, although in some positions the echoes from a needle, fragment of glass, wooden silver, or surgical thread could not be distinguished from those of the surroundings. Acoustic shadows were also seen behind the foreign bodies. A striking column of reverberation echoes was demonstrated behind lead pellets and nails while behind a glass plate there was an acoustic shadow that was often difficult to detect because it was concealed by a mirror image. With modern grey-scale real-time equipment it is possible to demonstrate objects from various angles, a fact which should facilitate the detection of foreign bodies in vivo circumstances.
