Immune complex induced hematogenic cell infiltration of the aortic luminal border layer in rabbits. Light microscopic studies of surface preparations.

Interendothelial and subendothelial monocytes/macrophages are extremely rare in normal rabbit aortas compared with humans and swine, as has been shown by the present study and previous investigations of surface preparations. Light microscopy was used to study the development of immune complex-induced hematogenic macrophage infiltrations in 26 rabbits from the 4th to the 28th day after the last antigen administration. Of particular interest was the intensity, localization and duration of the hematogenic cell infiltrates with a view to establishing the role of immunological stimuli for subendothelial monocyte and lymphocyte accumulation. It was found that immune complex-induced hematogenic macrophage infiltrates are very short-lived and disappear after 20-28 days following immune complex deposition. They concentrate in the ostium regions of the thoracic and abdominal aorta and are subject to flow dynamics similar to those of cholesterol-induced sudanophile lipid deposition. Subendothelial macrophage infiltrates increase the permeability of the aorta for low-molecular but not for macromolecular plasma particles, do not affect the endothelial silver line picture and do not lead to lipid accumulation.

Disseminated testicular cancer with bulky disease: results of a phase-II study with cisplatin ultra high dose/VP-16/bleomycin.

This is a preliminary analysis of the AIO-Testicular Tumour Study Group trial in patients with disseminated bulky testicular cancer. Treatment plan: cisplatin 35 mg/m2 days 1-5, VP-16 120 mg/m2 days 1-5 (two daily divided doses), bleomycin 15 mg/m2 days 1, 8, 15. Of 98 patients at present evaluable 63% had complete remission or have no evidence of disease (CR/NED), 30% had partial remission (PR) and 7% had no change or progressive disease (NC/P). Relapse-free survival is 93% for the CR/NED group after a median follow up of 2.2 years: the overall survival for the entire patient population is 70%. Toxicity included predominantly granulocytopenic fever and infection with septicaemia, thrombocytopenia, nausea, vomiting, neurotoxicity and lung toxicity, with 7% fatal toxicity. A prospective randomized trial is warranted to evaluate the apparent superior activity of ultra high dose cisplatin in combination with VP-16 and bleomycin.

G and/or C-bands in plant chromosomes?

Similarities and differences become evident from comparisons of centromeric and non-centromeric banding patterns in plant and animal chromosomes. Similar to C and G-banding in animals (at least most of the reptiles, birds and mammals), centromeric and nucleolus-organizing region bands as well as interstitially and/or terminally located non-centromeric bands may occur in plants, depending on the kind and strength of pretreatment procedures. The last group of bands may sometimes be subdivided into broad regularly occurring 'marker' bands and thinner bands of more variable appearance. Non-centromeric bands in plants often correspond to blocks of constitutive heterochromatin that are rich in simple sequence DNA and sometimes show polymorphism; they thus resemble C-bands. However, most of these bands contain late-replicating DNA. Also they are sometimes rich A X T base-pairs, closely adjacent to each other and positionally identical to Feulgen+ and Q+ bands, thus being comparable to mammalian G-bands. Although banding that is reverse to the non-centromeric bands after Giemsa staining is still uncertain in plants, reverse banding patterns can be obtained with Feulgen or with pairs of A X T versus G X C-specific fluorochromes. It is therefore concluded that not all of the plant Giemsa banding patterns correspond to C-banding of mammalian chromosomes. Before the degree of homology between different Giemsa banding patterns in plants and G and/or C-bands in mammals is finally elucidated, the use of the neutral term 'Giemsa band', specified by position (e.g. centromeric, proximal, interstitial, terminal), is suggested to avoid confusion.

Sister chromatid exchanges and heterochromatin.

The inter- and intrachromosomal distribution patterns of SCEs obtained with or without mutagen treatment are reviewed and compared, with each other as to their relation to heterochromatin and with the distribution patterns of chromatid aberrations that occurred either "spontaneously" in chromosomes of repair-defective human syndromes or after treatment with the mutagens (BrdU, ethylalcohol, DMBA, TMBA, maleic hydrazide, MMS, MMC). The conclusions are: No general rule is detectable for nonrandom involvement of heterochromatin in spontaneous SCEs. Mutagen-induced SCEs show the same or very similar distribution patterns as the spontaneous ones and are in no case as preferentially located as chromatid aberrations (which involve mainly the junctions between eu- and heterochromatin or other special regions). Therefore, a specific mutagen sensitivity of heterochromatin-aberrations does not exist (or is less pronounced) for SCEs. This supports the inference that different mechanisms underlie the origins of the two phenomena.

[On the possible meproscillarin accumulation in patients with impaired renal function (author's transl)].

26 patients with impaired renal function and 7 healthy subjects received an oral dose of 0.5 mg 14-Hydroxy-3-beta-[(4-O-methyl-alpha-L-rhamnopyranosyl)oxy]-14beta-bufa-4,20,22-trienolide (meproscillarin, Clift) per day for 2 weeks. Plasma levels of meproscillarin were measured during and after this period by means of the 86Rb-erythrocyte method. No statistically significant differences were found between the 2 groups. The results are consistent with extrarenal elimination of the substance. Contrary to digoxin, e.g., dose reduction is unnecessary in chronic renal failure. Mild gastrointestinal symtpoms were the only side effects seen in 6 patients.

On the expressivity of aberration hot spots after treatment with mutagens showing delayed or non-delayed effects.

The intrachromosomal distribution patterns of chromatid aberrations induced by agents with delayed effects (as exemplified by ethanol and maleic hydrazide) were compared with those produced by agents with non-delayed effects (as exemplified by fast neutrons, X-rays and bleomycin). Despite nonrandomness of aberration distribution in all cases, the mutagens with nondelayed effects generally showed up with much less pronounced aberration hot spots than the mutagens with delayed effects. From the results obtained it is concluded that hot-spot expressivity is a characteristic "group-specific" feature of the two classes of mutagen and that aberration production during DNA replication (S-phase) by agents with delayed effects strongly favours a very pronounced aberration clustering, which is partly mutagen-specific. Possible causes of these differences with respect to hot-spot expressivity after treatment with mutagens showing non-delayed and delayed effects, respectively, are discussed.

Heat-shocks prior to treatment of Vicia faba root-tip meristems with maleic hydrazide or TEM reduce the yield of chromatid aberrations.

Heat-shocks (10 and 30 min at 40 degrees C) prior to treatment with MH or TEM significantly reduced the yield of metaphases with chromatid aberrations. No such effect was observed when ethanol was used for aberration induction. The 'heat-shock effect' on aberration induction by MH and TEM is comparable to 'clastogenic adaptation' observed after pretreatment ('conditioning') with low clastogen concentrations prior to 'challenging' with high clastogen concentrations; both require unimpaired protein synthesis.

[Sample survey of persons insured in statutory health insurance institutions in Hessen--concept and realisation of person-related data base].

Statutory health insurance data are being increasingly used for secondary data research. Longitudinal data can be prepared for research in health care, epidemiology or demand planning, in particular through the person-related nature of the data which is a precondition for the creation of inter-sector and inter-period data sets. This application possibility was introduced in a method study "person-related sampling of statutory health insurance data" and is now translated into practice on a larger scale for the first time in the regional sample "Versichertenstichprobe AOK Hessen/KV Hessen". For the collection and use of these data, model procedures were designed which take account of organisational (data access, contractual agreement, advisory board), technical (sampling, collection and storage of data) and confidentiality (data protection concept, pseudonymisation) aspects. The insured person-related sample may thus serve as a basis for the data pool planned for the national health system (Social Security Regulation 303 a-SGB V).

[Prescribed drugs and diagnoses in over 60-year-old persons exemplified by cardiovascular diseases. Follow-up studies using federal health insurance data].

In Germany, there have been few investigations into prevalence and medical-therapy for cardiovascular disease (especially regarding cardio infarction) among the elderly. Our aim was to investigate the frequency of cardiovascular disease among over 60 year olds and to show their therapy as population related. The basis of our data was the documents of a representative study group of AOK (insurance) members from a North Rhine Westphalian Metropolis. The prevalence of cardiovascular problems was with 90% very high. 56% showed symptoms of at least two further illnesses from the three categories: the musculo-sceletal system, gastro-intestinal system, and their state of mind. All cardiovascular patients were treated with cardiovascular drugs; 2/3 continuously over a 4-quarter period. 43.9% of the aged population had hypertonia, women (47.1%) being slightly more affected than men (35.4%). The prevalence increased to 50% among 80 year olds. The degree of treatment in the case of female hypertensives was 90% and 82% in the case of males. The prevalence of cardiac infarction came to 3.5% among the elderly, and was notably higher for men than for women (6.7%/2.3%). Almost 90% of the cardiac infarction patients received on average anti-anginal vasodilators with 523 DDD per year. The vast majority of patients received three types of medicines simultaneously. The 1-year mortality of the elderly cardiac infarction patients was, when compared to a random sample of the same age, 7 times higher. Apoplexy among the aged had a prevalence of 2.8% and increased rapidly with age (60-69 years/1% to 5% > or = 70 years). The increase affected, above all, men, whose year's mortality was 15 times higher than that of a random sample of the same age. The apoplexy patients had a large amount of cardiovascular medications prescribed to them.
