[Radiological anatomy of the anterior cerebral vein (author's transl)].

The anatomy of the anterior cerebral vein is described with injection specimens. Radiological appearance of the anterior cerebral vein is illustrated with conventional angiograms. Stereoscopic films are indispensable to identify the anterior cerebral vein accurately. The anterior communicating vein or the distal end of the anterior cerebral vein indicates the anterior wall of the lower portion of the third ventricle. The anterior cerebral vein is very useful in defining the suprasellar extension of an intrasellar mass.

Serum dopamine beta-hydroxylase activity in essential hypertension.

1. Serum dopamine beta-hydroxylase activity was determined in normotensive control subjects and patients with labile or established essential hypertension. The enzyme activity was 25-9 +/- 1-9 (SEM) 29-6 +/- 2-5 and 25-1 +/- 1-9 micronmol min-1 1-1, for control, labile and established hypertensive subjects respectively. 2. Neither blood pressure nor serum dopamine beta-hydroxylase activity was changed in normotensive control subjects by administration of phentolamine; however, in patients with essential hypertension blood pressure was significantly decreased (P is less than 0-01) and serum dopamine beta-hydroxylase activity was slightly increased. With propranolol administration, blood pressure and the serum enzyme activity were not significantly changed in normotensive or hypertensive subjects. 3. Our results suggest that there is no correlation between serum dopamine beta-hydroxylase activity and blood pressure.

System analysis of heart rate control in man.

The dynamic property of the heart rate response to exercise was determined and expressed in the frequency domain to establish a method of examining cardiovascular control function. The response of heart rate to a stimulus was measured at 5-s intervals in nine healthy young volunteers. The stimulus consisted of several runs of two-step exercise practiced in semirandom sequence for 19 min. The weight function of the system was estimated from autocorrelation function of the input signal and cross-correlation function between the input and output signals. The weight function was transformed into a transfer function and its Bode plot diagram was drawn. From the diagram, four dynamic parameters were determined. These parameters are as follows: K is a constant showing the theoretical steady-state increment of heart rate, and T1, T2, T3 are time constants. The values obtained in the present experiment with the healthy young males were: K 46.0 +/- 14.6 beats, T1, 2.12 +/- 0.44, T2, 1.12 +/- 0.16, and T3 0.70 +/- 0.07 min.

Hypotensive action and increased plasma renin activity by Ca2+ antagonist (Nifedipine) in hypertensive patients.

The Ca2+ antagonistic coronary vasodilator, Nifedipine, was sublingually administered by a dose of 30 mg to 19 patients with hypertension. Blood pressure of patients with with essential hypertension (n=14) decreased from 177 +/- 24 to 123 +/- 13 mmHg systolic and from 108 +/- 12 to 80 +/- 11 mmHg diastolic (mean +/- SD) (p less than 0.01). Plasma renin activity (PRA) increased significantly from 0.73 +/- 0.62 to 1.50 +/- 1.02 ng/ml/h (p less than 0.05). The same tendency was observed in malignant and renovascular hypertension. In primary aldosteronism (n = 2), blood pressure decreased but PRA did not increase. Hypotensive action and increased plasma renin activity by Ca2+ antagonist, Nifedipine, were clearly demonstrated in patients with hypertension.

An IgG M-component reacting with both anti-kappa and anti-gamma light chains in multiple myeloma.

A patient with multiple myeloma who has an IgG-IM-component in serum reacting with both kappa and lambda light chains in immunoelectrophoresis is presented. The possibility of "hybrid molecule (H2LkappaLlambda)" is strongly suspected. The absorption studies of the patient's serum employing anti-kappa and anti-lambda light chains, however, support the view that this apparently single M-component consists of two M-components, IgG 1-kappa and IgG 1-lambda. The reason why these two M-components possessed the identical electrophoretic mobility is unclear. The possibility is high that these two M-components are produced in the same cell.

An evoked electromyographic test for peripheral facial palsy.

The purpose of the present study was to determine the degree of nerve degeneration in patients with facial palsy from the number of spikes in M-wave which were induced from the facial muscle by means of a bipolar concentric needle electrode when the facial nerve was stimulated. Ten normal cases between 14 and 72 years of age and 46 cases with facial palsy between 12 and 73 years of age were examined. Histopathological findings of ten denervated nerves were studied to determine to what extent this method may reflect the state of nerve degeneration.

Changes of pH-values during storage of fetal blood samples.

In recent years increased attention has been given to the degree of acidity of fetal blood during the course of labor or immediately after delivery. As it is not always possible to measure the blood pH immediately after sampling, it is important to know whether or not the pH is likely to change if the blood sample is stored for some time. 30 heparinized whole blood samples were collected from the umbilical artery of 30 newborn infants immediately after delivery and were stored at room temperature. pH -measurements were made every 5 minutes up to 80 minutes. In a second group the same number of samples were stored in a refrigator. pH-measurements were performed after 30 minutes and then every hour for up to 7 hours, and finally after 24 hours. The pHact-values did not change at room temperature (about 24 degrees C) for up to 25 minutes; pHqu40-values decreased initially more rapidly but at 50 minutes the decrease was the same as in pHact-values namely 0.02 units. Afterwards the pHact-values decreased at a mena rate of about 0.03 units per hour and pHqu40-values at a mean rate of about 0.017 units per hour. If the storage temperature was lowered by refrigeration (about 6 degrees C) the pHact-values remained between 0 and -0.02 units for 6 hours and pHqu40-values for 3 hours. From these experiments it can be concluded that fetal blood samples must be kept in a refrigerator in order to inhibit autoxidation, if they have to be stored for longer than 50 minutes.

Treatment of multiple myeloma and macroglobulinemai Waldenström: A long-term follow-up study.

Fifteen cases of multiple myeloma and 6 cases of macroglobulin emia Waldenstrom were followed up from 1957 to 1974. As for administration of drugs a low continuous dose regimen was mainly employed instead of a high intermittent dose regimen. 50% survival time from the onset of the disease was 18 months for multiple myeloma and 25 months for macroglobulinemia Waldenstrom. 3 cases of multiple myeloma are still living 44 months after the onset of symptoms. Cyclophosphamide and melphalan seem to have contributed much to the prolonged survival of these patients as well as improved supportive care.
