The radiological appearance of appositional new bone on the medial part of the neck of the femur in coxarthrosis.

The X-rays of 448 hip joints with coxarthrosis (representing 304 patients) were examined. The majority (87.5%) showed appositional new bone formation on the medial part of the femoral neck in antero-posterior X-rays. Further studies showed that the extent of new bone formation on the posterior surface of the femoral neck was usually much greater than that found on the medial margin. Well trabeculated cancellous bone lay between the new bone cortex and the persisting cortex of the femoral neck. The new bone formation is considered to be a response to abnormal forces caused by limping. The radiological appearance was clearly defined into four major groups and it is suggested that the pattern observed may be related to the type of limp, the presence of a flexion deformity or anteversion of the femoral neck.

Bone Development

Transposition of the greater trochanter of the femur during growth.

The indications for surgical treatment of coxa vara were laid down in our establishment on the basis of biomechanical considerations and from 1963 to 1979, 52 transpositions of the greater trochanter were carried out during growth, i.e. at the age of 7-16 years. We treated only those patients where transposition of the greater trochanter was carried out as the only intervention on a hip, regardless of previous hip operations. Apart from the mechanical elements the conditions of enchondral ossification of the femoral head and the greater trochanter were taken into consideration in the indications. In our material stability of the pelvis was achieved in 73.07% of the cases operated on, and in preserved enchondral ossification of the femoral head we succeeded in enlarging the collodiaphyseal angle in 86.53% of all cases. According to our results, we consider the optimal age for the operation to be between the age of 8 and 11. We do not recommend transposition of the greater trochanter in cases of dysplastic acetabulum. It was our wish to include in our observations the most important biomechanical elements. The results of functional and morphological conditions following surgery were considered basing the indications on the causative factor of the origin of coxa vara.

Adolescent

Experiences with total hip replacement arthroplasty ad modum ring.

The results of total replacement of the hip joint ad modum Ring are presented. The review includes 63 hips (from among the total number of cases operated by the various methods) followed up for periods ranging from 6 months to 5 years. The assessment has shown excellent results in 33.3 per cent, good results in 50.8 per cent, fair in 9.5 per cent and poor results in 6.4 per cent. Advantages and disadvantages of the method are discussed.

[Modified Küntscher's nail for the proximal femur fixation after corrective osteotomies in children (author's transl)].

The authors describe the development in the design of the modified Küntscher's nail with two fins on its upper part, assigned for the fixation of proximal femur after corrective osteotomies in children. Detailed explanation about the nails construction and its biomechanical characteristics are presented. The fixation of the nail in the proximal and distal femoral part, as well as on the site of osteotomy is particularly analyzed. On the base 423 operated cases, where the modified Küntscher's nail was applied, the authors conclusion is that the nail is very suitable for the correction of proximal femur deformities in children.
