[Conventional roentgen functional diagnosis--videodensitometry. 4. Conventional roentgen videodensitometry exemplified with peri-cardiac vessels].

It is possible to record movement-parameters by means of roentgen videodensitometry of the great central vessels as well the pulmonary artery as the thoracic aorta visible in fluoroscopic image (provided by videorecorder). We could noninvasively measure pre-ejection periods (PEP) for the right heart (PEP f) as well for the left heart (PEP g) by synchronous electrocardiography for each cardiac action and for each examination process. It is important to form a quotient between PEP f and PEP g in the individual case. First findings in patients with cardiac septal defects (with and without right heart block and pulmonary hypertension) show characteristic quotients and figures of the curves. In cases with electrocardiographic left heart block the elongated PEP g can be stated in graduated form. Further investigations on the mechanic interaction between the right and left heart, especially in cases of disorders in the heart conduction system, might be of interest.

[Conventional roentgen functional diagnosis--videodensitometry. 1. Densitometric and topometric measurements of roentgen video images: technique and method of measuring].

It is possible to measure movements in the human body with the help of X-ray television systems. A great number of different techniques has been published in the scientific literature. They all based on only two principles with qualitative differences in signal acquisition. The densitometric and topometric measurements are assessed and compared. Neither the densitometric nor the topometric principle seems to be superior. To support the experimental work the videoanalyser VIANA MP was developed on the Ilmenau Institute of Technology.

[Conventional roentgen functional diagnosis--videodensitometry. 2. Clinical imaging site for noninvasive diagnosis of organ motion].

By means of the described system it is possible to measure and as well analog as digital process the organ movement, needing a X-ray television system for flouroscopy and a videotape for continuously recording. Using the densitometric principle measurements of time and a quantitative form analysis of the curves are performed. After border recognition and using the topometric principle, the local organ motion can be evaluated. The levels of signal and noise in the measured curves are computed by a fourier transformation. In the field of functional cardiology the video signal is recorded and processed synchronously with the electrocardiogram.

It is possible to record movement parameters video densitometrically and synchronously by means of a video signal analysator at various points of the organ surface visible via fluoroscopic images (provided by videorecorder). Movement parameters of the heart surface were signed just in the same cardiac action by synchronous electrocardiography. There are some difficulties in this method according to evaluation with reference to laevocardiography in consequence of functional variety of the heart and the different exercise conditions. Small aneurysms of the cardiac apex were only seen when they were separated and located at the cardiac margin. Extended cardiac aneurysms were video densitometrically recorded as a paradoxical movement at the cardiac surface. A paradoxical movement was never seen with negative findings in laevocardiography.

[Serological diagnosis of cytomegalovirus-induced interstitial pneumonia in patients following bone marrow transplantation].

For diagnosis of the interstitial pneumonia conditioned by cytomegalovirus (CMV) an indirect immunofluorescence test was used to measure IgM and IgG antibodies to CMV in 369 serum specimens from 41 patients after bone marrow transplantation (BMT). An interstitial pneumonia conditioned by CMV was diagnosed in 5 patients either serological (4 patients) or by detection of cytomegaloviral infection typical inclusion bodies in lung cells (1 patient). The detection of antibodies to CMV for diagnosis of interstitial pneumonia conditioned by CMV is problematical in the phase immediately after BMT because bone marrow recipients are severely immunosuppressed by radiation and cytotoxic drugs received before transplantation. New method for detection of CMV-specific antigens are necessary.

[The diagnosis of functional epigastric syndrome].

The radiological examination of the stomach and the duodenum under functional aspects resulted in references of an irritative stomach in patients with epigastric complaints without conceivable organic findings lasting for many years. On the basis of simultaneously established peculiarities in the behaviour and the social situation of the patients concerned the disturbance of kinetics demonstrated is realized with great probability and explained as pathological by the patient. The results of the examination may be points of influence for the therapy of such disturbances.

[Videodensitometry analysis of stomach wall motility--presentation of a new method].

The videodensitometry is a suitable method for recording the gastric wall motility. It provides biosignals amenable by computer. There are no special demands to any organic form or kind of contraction. The asymmetry of the stomach makes no difference, therefore. The hitherto usual manometry will not be superseded. Both methods complete themselves in an ingenious manner. The radiation exposure by videodensitometry can be diminished by modifying the system for image causation.

[Development of body weight in males of the Wurzen district 1961-1983].

In 1961, 1971 and 1983 additional medical parameters, among them body length and body weight, were established during the mass radiographies in the district of Wurzen. From the data of 7,670 males from the year 1983 for two 10-year age groups regression equations between body weight and body length were determined. The calculation of relative weights was performed on the basis of the optimum weights after Ott. The longitudinal comparison from 1961 to 1971 resulted in an increase of the number of persons with overweight by 17.7% during 10 years. The results of the examinations of the year 1983 showed that the trend to the increase of body weight continues in the male inhabitants of the district of Wurzen.

[Cardiovascular radiography, changes in the thoracic aorta, ECG and clinical parameters in hypertension. 2. Changes in the thoracic aorta in hypertension].

In 142 hypertensives and a control group of 230 normotensives the measure of the aorta after Kreuzfuchs was estimated. In the two groups a significant increase of the width of the aorta was found with growing age. Whilst the width of the aorta concerning the degree of severity I of hypertension on an average were slightly below those of normotensives, they were increased in the degrees of severity II and III, but a further increase could not be ascertained at the transition from degree of severity II to III. On the other hand, the duration of hypertension is of particular importance when a dilation of the aorta is present. Also between deformation of the heart and measure of the aorta close relations which may conclude to a homogenous development are to be seen particularly in the younger patients. The influence of the sclerosis of the aorta in the sense of an additional dilation of the aorta could not be clearly confirmed on the basis of our material. On the other hand, the frequency of calcium depositions significantly increased with growing age in the region of the aortic arce and in the group of younger patients with the duration of hypertension, whereas the different degrees of severity II and III had a smaller influence. It is interesting that several patients showed normal widths of the aorta despite a duration of the hypertension of more than 5 years and a degree of severity III.

[Cardiovascular x-ray findings, changes of the thoracic aorta, electrocardiographic and clinical parameters in hypertension. 1. Cardiovascular x-ray findings in hypertension].

We examined 142 hypertensive patients of special consulting hours for hypertension. In a subdivision into 3 degrees of severity of hypertension and into the age groups less than or equal to 50 years and greater than 50 years X-ray findings on the heart and on the large vessels near the heart were made. Patients with blood pressure values RRs greater than or equal to 160 Torr and RRd greater than or equal to 95 Torr had these findings in 78%. 56% of the test persons with a heart affected in the left ventricle (R1) were found and 23% of the test persons with left-prevailing heart affected on the two sides (R3). The positive correlation between age of the patients with hypertension, duration of hypertension and frequency of the heart deformation in the sense of the left-ventricularly affected heart and the left-prevailing heart affected on the two sides could statistically be ascertained. 62% of the patients less than or equal to 50 years and 89% of the patients greater than 50 years had these findings. In a duration of hypertension less than or equal to 5 years 64% had these findings, in a duration of hypertension greater than 5 years 64%. The total number of findings increases with the degree of severity of hypertension. In test persons with degree II of severity of hypertension (RRs 160-200 Torr, RRd greater than or equal to 95 Torr) 73% had these findings, in test persons with degree III (RRs greater than Torr, RRd greater than or equal to Torr) 86%. The relation of the heart deformation in the sense of the left-prevailing heart affected on the two sides between degree II and III of the severity of hypertension was contradictory. In hypertension II and duration of hypertension greater than 5 years the percentage of the test persons with left-prevailing heart affected on the two sides was 42%, in hypertension III and duration of hypertension greater than 5 years 19%.

[Behavior of hypertension and cardiovascular findings in hypertension within an observation period of 10 years (the Wurzen study 1961-1971)].

In concert with literary data during a mass X-ray examination in 1961 we established in 5,517 men of the district Wurzen a frequency of hypertension of 13.6% and in 1971 in the same group of test persons a frequency of hypertension of 19.4%. The percentage of the pathological cardiovascular X-ray findings was in normotonus 10.4% in 1961 and 11.2% in 1971, in hypertension 22.3% in 1961 and 27.5% in 1971. The dependence on the age of the test persons, on the degree of severity of hypertension, and particularly on the duration of the hypertension is clear. For prevention of a hypertensive heart disease the early recognition of a hypertension and an early therapy are necessary. On the basis of our studies the making of cardiovascular X-ray findings from photofluorograms is not suitable as screening test for the hypertension. The value of blood pressure controls in mass X-ray examinations must be emphasized. Important is the collection of cardiovascular X-ray findings from photofluorograms of the thorax for the establishment of a hypertensive heart disease, of not recognized vitia and age-conditioned cardiac and vascular changes. It implies an optimum use of the material collected in the mass X-ray examination.

[X-ray findings in bones of patients with chronic renal insufficiency under chronic hemodialysis program].

In the chronic dialysis program, taking into consideration the duration of the dialysis, we sought in 25 patients (average age 34 years) for the radiological signs of the uraemic osteopathy, of the ectosteal calcification and the vascular calcification of the type Mönkeberg. In our patients a radiologically visible affection was to be proved at a duration of the dialysis of more than 3 months. Size and kind of the alterations depended on the age of the patients. In patients younger than 40 years we established an accumulation of isolated soft tissue or osseous processes, in patients older than 40 years the combination of multilocular osseous changes with calcification of the soft tissue or vascular calcium was of interest. Calcifications of the vessels of type Mönkeberg we could not find in patients younger than 40 years also at a duration of the dialysis of more than 1 year. This work shall be starting point of further observations on patients with chronic renal insufficiency during the course of dialysis, especially after changing of the calcium content of the dialysis solution.
