Field surgery on a future conventional battlefield: strategy and wound management.

Most papers appearing in the surgical literature dealing with wound ballistics concern themselves with wound management in the civilian setting. The pathophysiology of modern war wounds is contrasted with ballistic wounds commonly encountered in peacetime, but it should be noted that even in peacetime the modern terrorist may have access to sophisticated military weaponry, and that patients injured by them may fall within the catchment area of any civilian hospital. Management problems associated with both wound types are highlighted; areas of controversy are discussed. The orthodox military surgical approach to ballistic wounds is expounded and defended.

Animal cadaveric models for advanced trauma life support training.

'Hands on' practical workshop teaching is a fundamental part of advanced trauma life support training which has now been introduced into the United Kingdom. The use of live large animals, which is standard for the original courses developed in the United States of America, is prohibited in the United Kingdom and, therefore, models for upper airway bypass procedures and thoracic drainage have been developed. Their preparation and use is described, and contrasted with alternatives.

The natural history of the post anal (pilonidal) sinus.

Forty-two military hospital patients with post anal (pilonidal) sinus disease who had returned a questionnaire were studied retrospectively. Overall recurrence rate after various treatments was high, and it is suggested this be considered a chronic disease with a natural regression. Seldom this point seems to be after the 30th birthday. However, there appears to be a small group that develop the condition for the first time in their fourth decade. Sitting down at work appears to be a significant (p = less than 0.05) factor in aetiology.

Extra-cranial aneurysm of the internal carotid artery treated by proximal ligation.

A case of aneurysm of the extra-cranial section of the internal carotid artery extending to the base of the skull is described. Treatment comprised proximal vessel ligation, the tolerance of the patient's cerebral circulation to this procedure first being assessed by measuring the back pressure across the circle of Willis after temporary carotid artery occlusion. The value of this technique is discussed.
