[Laparoscopic fenestration of the ovaries in sterility and the Stein-Leventhal syndrome--is it a modern fad or correct procedure].

The treatment of the sterility due to the Stein-Leventhal syndrome has been always object of many discussions. Sometimes, the approach towards this disease in the different departments is diametrically different. Some offer only conservative therapy other-only surgical interventions and some-combination between the two. The authors of this study present the results of operative laparoscopy in the treatment of sterility and polycystic ovary syndrome, in the department of gynaecology to the Higher Medical Institute, Pleven. Object of observation were 33 women in whom was performed laparoscopic fenestration of the ovaries and 20 women in whom was made resectio cuneiformis ovariorum. Comparative analysis of the results for one year period after the surgical interventions was made. The conclusion of the authors is that after failure of the conservative approach, the laparoscopic fenestration of the ovaries remains the only right therapeutic intervention.

[A rare case of generalized echinococcosis and pregnancy].

The authors describe a rare case of combination of generalized echinococcosis and gravidity. It concerns about a 23-years-old patient in 10 lunar month in whom 10 days before the term, accidentally through ultrasound is found an Echinococcus cyst in the liver. The delivery performed with planned caesarean section without disturbance of the cyst, ended successfully. Two days after the surgery the patient complained of troubled respiration and oppression. The X-ray photograph showed 3 Echinococcus cysts more--one in the right end two in the left lungs. The problem was solved with three more operations after 2, 6 and 10 months, with which the Echinococcus cysts were eliminated.

[A multicenter study of the antimicrobial effect of Macmiror and Macmiror Complex in the treatment of vaginal infections].

UNLABELLED: The aim of the present multicentre study was to examine the therapeutic possibilities of the wide-spectrum medicament MACMIROR & MACMIROR COMPLEX for the treatment of the vaginal infections. MATERIALS AND METHODS: The study included 159 nonpregnant women among 15 and 54 years (middle age 35.6) with different by kind and intensity colpitis complaints. The following microbiological characteristic was established: in 26 cases Gardnerella vaginalis, in 46 Candida spp., and in the rest 87-mixed aerobic bacterial flora, with a combination of Gardnerella, yeast and Trichomonas. The treatment of the patients was done in combined scheme: peroral and vaginal administration, simultaneously with local treatment of the partner. The control examination was performed bistagely: on 7-10 day and on 30-40 day. RESULTS: The good clinical and microbiological influence of the treated patients was established, for the first control examination the effect was found in 88.1% and 86.8% and for the second--respectively in 81.1% and 82.4%. CONCLUSIONS: The received results give us a cause to approve, that the combination "Nifuratel and Nystatin" (Macmiror & Macmimor complex) has the good possibilities to influence the mixed forms of vaginal infection.

[Tocolysis and its place in premature labor].

Premature labour ranges from 6 to 8 per cents from all of the deliveries but it causes 75 80 per cents of the perinatal infant mortality. The purposes of the present research are: to prove the benefit of the tocolytic therapy in case of progressing premature labour (cervical dilatation 3 and more cm and distinguishable uterine contractions--at least 2 in 10 min for more that 1 hour); to compare the efficiency of the medicines used in tocolysis, as well as their influence on the maternal and infant morbidity and mortality; A successful tocolysis is considered to be the one, which delays the delivery for at least 48 hours. The authors concluded, that there is not any significant statistic difference in the tocolysis success with beta-adrenomymethics and the use aquatic solution of Magnesium sulfate. Better results were achieved in the cases when Indomethacin was added to the basic tocolytic medicine. No significant harms were observed in the maternal and infant organisms in result of the tocolytics application. It has been found that after the tocolytic therapy application the newborns were with greater birth body weight and the percentage of the respiratory distress was lower, compared to the group with expectant behaviour at hospitalization.

[Postnatal screening for congenital anomalies--the possibility of detecting families at high genetic risk].

The aim of the study was to present out experience with the registration of congenital anomalies (CA) and to assess the effect of the preventive genetic-consultative activities in affected families. In the period 1990-1996, 19174 infants born or hospitalized at the Clinic of Obstetrics, Pleven were screened for CA, showing frequency of 26.1%. Structural analysis of the CA is presented. 226 out of 500 (45%) families with and affected child were consulted by a geneticist. Data an prenatal diagnosis (PD) offered to 142 families at high risk and their reproductive decision are submitted. The low rate of families made use of invasive PD is pointed out; the real benefits of ultrasonography as a screening test for detection of fetal anomalies has been recommended.

[Disorders in sexual function following hysterectomy].

The authors aims to examine the sexual function among patients prior to hysterectomy. Four groups were examined: group one--total hysterectomy with bilateral adnexectomy; group two--total hysterectomy without adnexectomy; group three--supracervical hysterectomy with bilateral adnexectomy and group four--supracervical hysterectomy without adnexectomy. A questionnaire aimed at Learning the sexual function and psychologic state of the patients and more importantly the effects of the hysterectomy on then was passed out. From the completed questionnaires we found that the most affected group was the one of total hysterectomy with bilateral adnexectomy. In all respects sexual function is disturbed in this group. Least affected are patients in groups three and four. In some of them we even found a slight increase in sexual function, which is regarded compensatory for eliminating the infertility complex arising from surgery of the most important female organs.

[Antibiotic prophylaxis in cesarean section with cefoxitin (Mefoxin)].

The ain of this presentation is to show the advantages and disadvantages of a single prophylactic dose of Mefoxin, in comparison to the classical antibiotic prophylaxis with penicillin and gentamicin. The authors conclude that: single dose prophylaxis with Mefoxin significantly reduces the rate of infection morbidity after caesarean section; a single application of 2 g Mefoxin after clamping of the umbilicus is enough for prophylaxis of infectious complications after caesarean section.

[The diagnostic and therapeutic problems in genital infections caused by Chlamydia trachomatis].

There have been examined 28 married couples from 25 to 35 years old with fertility and 55 patients from 25 to 40 years old with symptoms of genital infection and promiscuity. Chlamydia-antigens in urethral and endocervical specimens and antibodies in blood were searched by the Microimmunofluorescence method. Only 7.1% of the married couples and 1.8% of the patients had positive results for antigens. 53.6% of the married couples and 54.25 of the patients had positive results for antibodies, at rates from 1:32 to 1:512, confirming a profound Chlamydia trachomatis-genital infection. Outline laboratory analysis are needed for solving the diagnostic and therapeutic problems of Chlamydia trachomatis-genital infections.

[Multiple pregnancy--ultrasonic biometry after the 28th gestational week--nomograms].

The authors aimed at working out normograms for BPD, FL, FAC and EFBW among twins in multiple gestation. They find a delay in the rate of increase of sonographic indices in multiple gestation in comparison to singleton pregnancies, as follows: in BPD--after 26-28 wks; FL--after 34-35 wks; and FAC--after 34-36 wks. Lower EFBW was found after 32-33 wks. The differences in the biometric indices in the first and second twins were also noted. In conclusion it is stated that it is mandatory to monitor by sonography BPD in the 26th wk, and FL and FAC in the 34th-35th wk.

Androgen level variations, clinical response to LHRH agonists and changes in the quality of life subscales in metastatic prostate cancer--speculations about possible role of the monoamine system.

The aim of this study was to investigate the effect of goserelin-acetat (Zoladex) on testosterone suppression, to compare achieved suppression with clinical effects in patients with prostate cancer with bone metastases and consequent painful syndrome, to study the behavior of adiol during treatment and to assess life quality with emphases on the physical and psychological domain in relation to clinical and biological treatment effects. Fifteen patients were treated by Zoladex in one dose every 28 days, and followed-up for 12 months. All patients had several metastatic localizations in the bones, initial high prostate specific antigen (PSA), and high acid (AP) and alkaline phosphatase (ALP). PSA, testosterone, adiol (delta-5-androstenediol), luteinizing hormone (LH), foliculostimulating hormone (FSH), ALP and AP were also measured before every cycle. For evaluation of the life quality Rotterdam Symptom Checklist was used. Clinical progression was not registered during follow-up, with drop of PSA, ALP and AP. Testosterone and adiol displayed mainly inverse trends during treatment. The complete testosterone suppression was never achieved. It seems that Zoladex has quite different influence on LH and FSH, as levels of those hormones have shown opposite trend. Some of the observed hormonal effects could be attributed to stimulation of the monoamine system. Suppression of LH level provoked by administration of LHRH agonists increases level of dopamine in hypothalamus which inhibits releasing of its hormones. By inhibition of corticotropic releasing factor and ACTH, and by its influence on adrenal gland, we could explain drop of adiol levels in the first months of administration of LHRH agonists. Testosterone increase and adiol drop in the first months, and adiol increase following testosterone level drop in the fourth to eight month, may be explained by negative feed back mechanism between different androgens which could be stimulated or provoked by LHRH therapy. The question of effects which are results of LHRH agonists modulation of the monoamine system and consequent activation of other central mechanisms of hormonal regulation is still open. Patients' quality of life under therapy was improved for about 30% in psychological and functional domains. There were no significant changes on physical subscale, during treatment. It seems that the obtained positive psychological treatment effect is not only a consequence of pain decrease, but it could be the result of the change in the level of monoamines in CNS under Zoladex.

[The use of the antibiotic cefoxitin (Mefoxin) in gynecological practice].

The aim of this presentation is to study the advantages and disadvantages of perioperative prophylaxis with the antibiotic Mefoxin against the classical postoperative antibiotic prophylaxis with penicillin and gentamicine, as well as to analyse the place of this antibiotic in modern treatment of different forms of pelvic inflammatory disease. The authors conclude that the qualities of Mefoxin (high resistant wide spectrum antibiotic, covering aerobes and anaerobes), make it an ideal antibiotic for perioperative prophylaxis in gynaecologic and oncologic surgery; the clinical effectiveness of Mefoxin in the treatment of inflammatory diseases of the female pelvis precludes the need for a combined parenteral antibiotic therapy.

Modulation of cisplatin antitumor activity by short infusional high-dose cytosine arabinoside in advanced pretreated head and neck carcinoma: a phase II study.

The aim of the present study, which included 24 pretreated patients (both with radiotherapy and/or cisplatin-containing chemotherapy) with histological grade I/II head and neck cancer, was to investigate whether the enhancing action of cytosine arabinoside on cisplatin antitumor activity might be evidenced without excessive hematological toxicity with the administration of short infusional high-dose cytosine arabinoside, applied before cisplatin. Cytosine arabinoside was administered on day 1 of the treatment cycle at 0h and 12h at the dosage of 500 mg/m2 (1 hour infusion), and cisplatin at 6h and 18h of the same day with a dose of 20 mg/m2. On days 2, 3 and 4, only cisplatin 30 mg/m2/24h was administered. 19 patients were evaluable for activity including 14 patients pretreated with cisplatin-containing chemotherapy. 2/19 achieved complete response (CR), 2/19 a partial response (PR), 8/19 had stable disease and 7/19 progressive disease. Objective responses (2 CR and 1 PR) were evidenced in 3/14 patients previously resistant to cisplatin at the same dosage as in the present regimen. WHO grade IV toxicity for granulocytes was recorded in only one patient.

Pilot study of enhancement of cisplatin activity by high dose cytarabine in advanced gastric and colorectal (G3/G4) cancer.

The response rate of advanced gastric cancer to cisplatin monotherapy averages 20% and in colorectal cancer no activity of cisplatin monotherapy has been detected in initial studies. Cytarabine is ineffective in gastric cancer and displays borderline activity in colorectal cancer. In vitro studies on cell lines from human digestive cancers have demonstrated a dose and timing dependent enhancing effect of cytarabine on cisplatin antitumor activity. The aim of the present study was to investigate whether this enhancing activity can also be demonstrated in vivo. We have treated 37 patients with advanced gastrointestinal cancer (21 gastric and 16 colorectal), poorly differentiated G3-G4. The treatment included-Day 1: cytarabine 500 mg/m2 0 h and 12 h, cisplatin 15 mg/m2 6 h and 18 h. Day 2-4:cisplatin 30 mg/m2. Cycles were repeated every 4 weeks. Thirty four patients were evaluable for objective response. The overall response rate was 16.7% (CR 2/18, PR 1/18, SD 5/18, PD 10/18) for patients with gastric cancer and 25% (CR 1/16, PR 3/16, SD 7/16, PD 5/16) for patients with colorectal cancer. Grade 4 anemia (WHO criteria) occurred in 1/37 and thrombocytopenia in 1/37 patients. These patients had previous adjuvant chemotherapy. Vomiting grade 3 occurred in 4/37 and hepatotoxicity grade 3 in 1/37 patients. There were no toxic deaths. Our study did not demonstrate any enhancement of cisplatin activity by high dose cytarabine in advanced gastric cancer. There appears to be an enhancing activity on colorectal cancer although true synergism can not be ruled out since cytarabine has a borderline activity in this type of human cancer.

[Epidemiological studies of patients with genital chlamydiosis in Pleven District].

The authors aimed at conducting an epidemiological analysis of genital chlamydial infection among married couples with infertility problems, women with pelvic inflammatory disease and among patients with promiscuity and clinical symptoms of genital infection in the region of Pleven. They performed a prospective study for a six month period of time. 36 married couples with infertility problems and 66 patients with promiscuity were included in the study. The authors conclude that genital chlamydiosis is a widely spread sexually transmitted disease among a large group of patients. Poor personal hygiene, poor education, young age, some professions which are a predisposition to promiscuity increase the risk of sexually transmitted diseases.

[Struma ovarii--a rare tumor in gynecological practice].

The authors present four cases of struma ovarii, observed a period of five years (1990-1995) in the Chair of Gynaecology and Obstetrics--Pleven. The literature data concerning this interesting problem was discussed in detail.
