[Surgical treatment of severe lower lid ectropion with a transplant of skin and perichondrium].

A surgical procedure is described for severe ectropions of the lower lid which cannot be treated successfully by conservative measures or minor surgery. This technique, employing a perichondrocutaneous graft, is superior to traditional methods in terms of both the functional and esthetic results. The composite graft is obtained from the anterior surface of the auricular choncha. In about 50% of the patients the postoperative biopsy showed a chondrogenic activity of the transplanted perichondrium substituting the tarsus. In the other cases, though no cartilaginous growth was noted, there was an intense proliferation of collagen fibers stabilizing the lid and the tarsal region. This procedure leads to both functional and natural esthetic rehabilitation and satisfies the criteria which techniques in plastic surgery have to meet today.

[Abdominoplasty--personal surgical guidelines].

An account is given by the authors of their own guidelines for surgical treatment of deformities of the anterior abdominal wall. The procedures described are based on experience obtained from 866 surgical interventions at the first author's clinic, between 1959 and 1987. Indications, techniques, and complications are discussed in some detail.

[Facial burns--treatment of sequelae].

The treatment of the sequelae of facial burns is still a controversial subject. The authors present a retrospective study of 1516 cases between 1967 and 1987. They suggest a more conservative approach of the therapy in purely aesthetic lesions. Therefore, they wait for the "softening" of the scars and start an operative treatment after the maturation of the collagen connective tissue. Functional sequelae have to be treated earlier according to their dimension and according to the aesthetic units affected.

[Reflections on the use of fluid dimethylpolysiloxan in plastic surgery of the head and neck].

The authors present retrospectively results and complications of 47 treatments with fluid dimethylpolysiloxane (silicone). The injections were not administered in the author's surgical department. The history of fluid silicone, its tissue reactions and the aesthetic results obtained are described. Different types of complications and their management are discussed. According to the authors' experience fluid dimethylpolysiloxane has not proved to be a proper material for plastic reconstructive and aesthetic treatment in the head and neck region.

[Rhytidoplasty: perioperative guidelines. Particular technical basic details].

The authors present their philosophy of rhytidoplasty and the basic technical details of this kind of operation. The aging face often causes psychological as well as social problems that make patients go to a facial plastic surgeon. The authors stress the importance of an intensive preoperative information. Basic details of preoperative preparation, surgical technique and postoperative care are emphasised because they represent decisive steps in terms of the aesthetic results obtained. These details proved to be mostly important in the course of about 5000 rhytidoplasties.

Augmentation mentoplasty: a critical analysis.

An evaluation of the senior author's mentoplasty technique by use of inorganic silicone and acrylic implants is presented, encompassing 16 years of experience. The evaluation was chiefly concerned with the silicone implant and long-term followup observations of its survival and some of its extrusions and complications.

Clinical evaluation and surgical treatment of hemangiomata.

We report on the histologic and anatomopathologic aspects of cutaneous hemangiomas, stressing the involutive and noninvolutive aspects of these malformations. We analyze its clinical evolution, approaching the propedeutic and therapeutic methods, and present a detailed commentary on the conservative methodology of a series of 473 consecutive patients, encompassing a follow-up of 1 to 18 years.

Treatment of abdominal wall eventrations associated with abdominoplasty techniques.

An experimental and clinical study using alloplastic materials (Marlex and Prolene mesh) for repairing large defects of the abdominal wall is reported. A defect was created in the experimental model to simulate an anatomical disarrangement of the musculo-aponeuroticfascial complex of the abdominal wall and the surgical correction of this deformity. Macroscopic and microscopic observations of the operated area were done to demonstrate the good integration of the surrounding tissues with the mesh. In a clinical study, 16 patients, presenting large eventrations of the abdominal wall, were surgically treated using Marlex or Prolene mesh to reinforce the abdominal wall. Two techniques of abdominoplasty, vertical and horizontal, were performed on these patients, according to the location of previous scars. The advantages of using alloplastic materials to reinforce the abdominal wall and the cosmetic results of both abdominoplasty techniques are discussed.

Our experience with combined procedures in aesthetic plastic surgery.

The authors report on the possibility of carrying out combined procedures for aesthetic surgery. They pinpoint the conditions necessary for the success of these techniques: surgeon, surgical team, anesthesiologist, and patient should fulfill some requirements that are indispensable. Preoperative and postoperative technical details are presented together with the methodology used for the most important associated procedures. The advantages of these techniques are a more complete patient satisfaction owing to the simultaneous correction of her or his multiple aesthetic problems, decreased hospitalization length and costs (if compared with multistaged surgical corrections), and decreased doctor and patient time investment. No important difference as far as general clinical complications or the aesthetic results were noticed after these procedures. The importance of a qualified team approach is underlined as a condition sine qua non. Otherwise, these techniques would be extremely difficult and risky. Data on personal experience are given, and finally, the authors emphasize the fact that combined procedures for aesthetic surgery are only a possibility that can be performed when all the basic principles and conditions are strictly respected, not a treatment of choice.

Surgical management of congenital giant hairy pigmented nevi.

The author describes his experience with surgical ablation of truly giant hairy pigmented nevi in 26 cases, three in detail of which two were on the face and one on the back. In a comprehensive table, salient clinical, histopathologic, and surgical data on the 26 cases are given.

Indications for and treatment of frontal and glabellar wrinkles in an analysis of 3,404 consecutive cases of rhytidectomy.

In recent years, there has been an increase in the percentage of forehead lifts in patients undergoing face lift surgery on our service. Also we find that younger patients are having the surgery. We describe our present technique and choices of incisions. We avoid incisions anterior to the hairline whenever possible, since they may produce conspicuous scars. Our treatment of the aponeurotic expansion by vertical and horizontal incisions has produced very satisfying results (Figs. 11 through 15). No postoperative complications are reported in our series.
