Simultaneous bilateral compared with unilateral percutaneous nephrolithotomy.

OBJECTIVE: To compare simultaneous bilateral percutaneous nephrolithotomy (SBPCNL) and unilateral PCNL in separate sessions in patients with bilateral renal stones for several variables before and after surgery. PATIENTS AND METHODS: The results from SBPCNL carried out at two centres in different countries on 198 patients (aged 1.25-70 years) were compared with those from 300 patients undergoing unilateral PCNL in separate sessions. At one centre where extracorporeal shockwave lithotripsy (ESWL) was available SBPCNL was used for stones where at least two sessions of ESWL would have been required on each side. Other associated procedures were also used wherever required. RESULTS: The stones were cleared bilaterally in one session of SBPCNL in 190 patients; in eight, fragments of < 3 mm remained but were not clinically significant. A second session was required in six patients on one side only. Variables assessed before and after treatment (e.g. hospital stay, analgesia requirements and complications) were not significantly different between SBPCNL and PCNL. The mean (range) total operative duration for SBPCNL was 46 (20-100) min and the hospital stay 4.3 (3-8) days. CONCLUSION: From this experience, SBPCNL is a cost-effective and beneficial solution for selected patients, with clear advantages over separate unilateral PCNL in patients with bilateral stones.

Some neurohistochemical properties of nerve elements in myenteric plexus of rabbit ileum: similarities and dissimilarities to the rodent pattern.

Enteric neurons have distinct neurochemical codings in each species. The basal tone of the gastrointestinal tract of the rabbit is low and produces neurally evoked pendular movements. Therefore, it might have an innervation pattern different from that of other laboratory animals. We have characterised myenteric neuron populations in rabbit ileum with neurochemical markers that are known to be associated with distinct cell types and/or fibre systems in the myenteric plexus. The density of nerve cells estimated with the NADH-diaphorase technique was about 2500 cells/cm2 and most, if not all, neurons contained microtubule-associated protein 2. NADPH-diaphorase-positive cells were numerous. One cell type was large and emitted long straight processes, whereas small cells bore thin filamentous dendrites. Neurons immunoreactive for 28-kDa calcium-binding protein were rare. Over 70% of them had very strongly labelled lamellar dendrites. Their axons were beaded and formed pericellular baskets around unstained somata. We found very few small tyrosine-hydroxylase-positive cells. The fibre network in the plexus was very strong; the axons formed many pericellular baskets. In double labelling studies, no co-localisation was revealed between the 28-kDa calcium-binding protein and NADPH-diaphorase. Some fibres containing 28-kDa calcium-binding protein formed only a few contacts on somata of NADPH-diaphorase-positive cells. None of the NADPH-diaphorase-labelled cells were found to be stained for tyrosine hydroxylase. Tyrosine-hydroxylase-positive fibres rarely made pericellular baskets on the surface of NADPH-diaphorase-positive somata. Strongly immunolabelled pericellular baskets were never observed around NADPH-diaphorase-positive cell somata. The results suggest that myenteric neurons in rabbit comprise distinct and characteristic neurochemical properties that are different from the rodent pattern. Therefore, the explanation of the motility pattern of rabbit intestine can be approached on a chemical neuroanatomical basis.

NADPH-diaphorase positive myenteric neurons in the ileum of guinea-pig, rat, rabbit and cat: a comparative study.

The nitrergic system of the ileal myenteric plexus of four mammalian species was studied by means of NADPH diaphorase histochemistry which in aldehyde-fixed tissue stains only those cells that contain the constitutive, calcium-dependent nitrogen monoxide synthase isoenzyme. Since previous studies assumed minor species-specific anatomical variations, we sought similarities and differences in the nitrergic innervation pattern of the ileal musculature. In rat and guinea-pig, the ratio of nitrergic cells slightly exceeds 20%, in rabbit it is close to this number (16%), and it is lowest in cat (about 10%). The nitrergic neurons target the circular muscle layer in all investigated species where they participate in inhibitory motoric transmission. Apart from this, some elements of the sensory innervation of the circular musculature may derive from nitrergic neurons in rat and rabbit. The tertiary plexus (longitudinal muscles) is strongly supplied by NADPH diaphorase positive fibres in rat, moderately in guinea-pig and cat and not at all in rabbit. The rest of the nitrergic neurons may serve as inhibitory interneurons and control other (probably excitatory motor) neurons in the myenteric plexus. We conclude that the diverse physiological and pharmacological properties of the nitrergic system described in the small intestine in different species can be connected with the anatomical heterogeneity of its elements.

Endopyelotomy: experience with 320 cases.

We were the first to initiate endopyelotomy in Hungary (in 1986) and in Yemen and Pakistan (in 1993). Through the end of 1995, 320 cases of ureteropelvic junction (UPJ) stenosis have been operated upon. The procedure was performed under local anesthesia in adult patients and general anesthesia in children. The minimum age of the patient was 4 years, while the oldest patient was 80 years of age. The UPJ was incised longitudinally at the posterolateral aspect until the perinephric fat was seen, and a drain of 8F to 12F was inserted transrenally into the ureter through a nephroscope. Patients soon left the hospital and were able to start working in 5 days (average). The drain was removed after 6 weeks. During the follow-up period, ultrasonic examination was performed each 3 months. The success rate (mean of three centers) came out to be 87%. Open pyeloplasty was performed in cases where the symptoms of pyelectasia were persisting and caused complaints and stenosis was present even after 6 months. In our opinion, endopyelotomy should be the procedure of first choice for UPJ stenosis, because it is less troublesome for the patient than open pyeloplasty, and the results are real encouraging.

Rare case of the utero-vesical fistula caused by intrauterine contraceptive device.

The vesico-uterine fistula is a very rare disease. There have only been 150-200 causes. We are reporting on a case in which the chronic fistula was caused by an IUD having been placed 4 years ago and it "wandered" through the bladder. A 30 year-old patient in 1992 and IUD was inserted. She had gynecological controls twice, in 1993 last time. She has problem of urination very often. A cyclical bladder bleeding drew the attention to the disease. In ambulanterely performed cystoscopy we found an IUD perforating towards the interior of bladder in the borderline of its bottom and back wall and was situated in the bladder with its 3/4. We have removed it with forceps. After six weeks of expectation and strict observation did we want to manage the fistula after having consolidated the symptoms of the inflamed surroundings. During the operation we have noticed a wallment size mass of scar between the uterus and the bladder expanding to the height of the orifice of the uterus. The scarily fixed bladder has been separated from the cervix and the scarry wall of the fistula has been cut out. We have brained the cervix towards the vagina and then we've sutured the cervix and the bladder with Dexon 'O' treat, as well. We have interposed a surgical net between the cervix and the bladder followed by blood-clotting and peritonisation. We should take the follows into consideration: careful separation, fine operative technique, and strong well absorbing thread as well as trying to keep the organ. In our opinion the bioplast--interpositum used on our case maker the efficiency of the operation higher.

[Percutaneous nephropexy].

We present a new, minimally invasive method for the treatment of nephroptosis. We are suggesting the more spreaded use of this method after the good results we found during the long term observation of our 40 patients with percutaneous nephropexy.

Laparoscopically controlled and assisted percutaneous transperitoneal nephrolithotomy in a pelvic dystopic kidney.

Two renal pelvic stones were removed percutaneously from a pelvic dystopic kidney using a transabdominal, transperitoneal approach. The puncture was made antegrade and controlled partly fluoroscopically and partly laparoscopically. Where the kidney was covered by bowels, these were displaced with forceps inserted through another laparoscopic trocar. The dilation and stone removal were performed traditionally. As the kidney was just behind the uterus, the authors considered extracorporeal lithotripsy contraindicated in this case, so the only minimally invasive resolution was percutaneous nephrolithotomy, which could be performed only with the aid of laparoscopy.
