[Individualization of operative procedures for uterine prolapse based on categorization using X-ray urethrocystohysterography and postoperative outcomes evaluated with a scoring system].

A urethrocystohysterography (UCHG) and a prolapse scoring system (PSS) have been used to assess the types of uterine prolapse and postoperative outcomes since 1979. UCHG was useful in identifying the type of uterine prolapse and in selecting operative procedure. UCHG was done by injecting contrast medium into the bladder and uterine cavity and inserting a metallic bead chain into the urethra. A lateral pelvic X-ray was then taken at rest and during straining. The length of the uterus (UL), distance from the pelvic outlet (PO) to the bladder base (BB), distance from PO to the uterine fundus (UF), and distance from the ischial spine (IS) to UF were measured on the UCHG. We found that there were three types of uterine prolapse on the UCHG findings, type 1: cervical elongation without descent of uterine fundus and cystocele, type 2: uterine prolapse with moderate descent of uterine fundus and cystocele, and type 3: giant vaginal eversion including completely prolapsed uterus, marked cystocele, enterocele and rectocele. The operative time of vaginal hysterectomy with anterior and posterior colporthaphy (VH with AP repair) correlated well with UL and PO-UF distance on UCHG, and blood loss. Operating time was significantly shorter and amount of blood loss was significantly smaller in cases of Machester operation (cervical amputation, fixation of cardinal ligament stumps to the anterior wall of the remaining cervix and AP repair) than in those of VH with AP repair.(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of opioid peptides on the tumoricidal activity of spleen cells from nude mice with or without tumors.

The present study was designed to explore the effects of opioid peptides on the immune systems of intact nude mice and nude mice bearing human ovarian cancer cells (KF). When spleen cells from intact nude mice were incubated with medium alone, a significant ability of spleen cells to lyse the KF cells was not observed. However, incubation of the spleen cells with 1 microM beta-endorphin or 1 microM alpha-endorphin induced a significant lytic activity on the KF cells. The control-level lytic activity was increased significantly to about 4.5-fold by 1 microM beta-endorphin and about 3.7-fold by 10 microM met-enkephalin. These results suggest that opioid peptides play a crucial role in cellular immunity. Thus, we examined plasma levels of beta-endorphin in patients with ovarian or uterine carcinoma. The plasma beta-endorphin levels in patients with ovarian or uterine carcinoma were significantly higher (more than twofold) than those of age-matched healthy women.

1,25-Dihydroxyvitamin D3 inhibits thromboxane release from activated macrophages.

1,25-Dihydroxyvitamin D3 (1,25-(OH)2D3) in concentrations from 0.1 to 10 nM suppressed immunoreactive thromboxane B2 (iTXB2) release from Propionibacterium acnes (P. acnes)-elicited liver adherent cells stimulated with lipopolysaccharide (LPS, 1 microgram/ml). These suppressive effects of 1,25-(OH)2D3 were also observed in oyster glycogen-elicited peritoneal macrophages. On the contrary, it did not inhibit iTXB2 release from both resident Kupffer cells and peritoneal macrophages stimulated with the same concentration of LPS. Furthermore, 1,24(R)-dihydroxyvitamin D3 (1,24(R)-(OH)2D3), a vitamin D3 analogue, also inhibited iTXB2 release from liver adherent cells, but, another synthesized vitamin D3 analogue, 1 alpha-hydroxyvitamin D3 (1 alpha-OH-D3) tended to decrease iTXB2 release only at higher concentrations. These results suggest that active vitamin D3 analogues inhibit iTXB2 release from activated macrophages.

[In vitro and in vivo effects of ginsenoside Rh2 on the proliferation of serous cystadenocarcinoma of the human ovary].

Inhibition of human ovarian cancer cell proliferation in vitro and in vivo by Ginsenoside Rh2(Rh2) isolated from Red Ginseng was examined by using a cell line (HRA) derived from ascites of a patient with serous cystadenocarcinoma of the ovary. The HRA cell proliferation in vitro was inhibited in a dose-dependent manner with between 10 and 100 microM of Rh2. The uptake of radiolabeled precursors (3H-thymidine, 3H-uridine, and 3H-varine) by HRA cells was also inhibited in a dose-dependent manner with between 30 and 100 microM of Rh2. The growth of HRA cells transplanted into nude mice was not significantly inhibited by Rh2 alone. On the other hand, when cisplatin was administered together with 10 microM, 30 microM, or 50 microM Rh2, the growth of HRA cells was inhibited 31 and 35 days after tumor inoculation. Survival was also prolonged when cisplatin was administered with 10 microM and 30 microM Rh2. These results suggest that there is a synergistic effect between cisplatin and Rh2.

Protective effect of vitamin D3 analogues on endotoxin shock in mice.

The effect of vitamin D3 analogues on endotoxin shock in mice was investigated. Male ICR mice were orally administered vitamin D3 analogues or vehicle, accompanied by an intraperitoneal injection of endotoxin (E. Coli lipopolysaccharide, LPS, 20 mg/kg). Endotoxin caused a decrease in survival rate in a time-dependent manner. Increases in plasma immunoreactive (i) eicosanoid and hepatic malondialdehyde (MDA) levels were also observed. Administration of 1 alpha-hydroxyvitamin D3 (1 alpha-OH-D3) improved the survival rate 24 to 48 h after endotoxin treatment. The effects were markedly observed at a dose of 20 ng/kg. In addition, 1 alpha-OH-D3 restored the plasma iTXB2 and hepatic MDA levels 8 h after endotoxin injection. However, it did not affect plasma iPGE2, i6-keto-PGF1 alpha and blood iLTB4 levels. At a dose of 20 ng/kg, both 1,25-dihydroxyvitamin D3 (1,25-(OH)2D3) and 1,24(R)-dihydroxyvitamin D3 (1,24(R)-(OH)2D3) restored the survival rate, the plasma iTXB2 and hepatic MDA levels. These results suggest that vitamin D3 analogues may inhibit endotoxemia through regulation of the formation of TXA2 and free radicals.

Calcium-regulating hormones and osteocalcin levels during pregnancy: a longitudinal study.

We measured serum concentrations of calcium-regulating hormones and osteocalcin in 20 women longitudinally throughout pregnancy, 1,25-Dihydroxyvitamin D levels were high early in pregnancy and increased with advancing gestation. Parathyroid hormone and osteocalcin levels were low in early pregnancy. They declined toward the middle of pregnancy, but increased in late pregnancy. The serum osteocalcin level correlated with the parathyroid hormone level. The synthesis of osteocalcin by osteoblasts is stimulated by the action of 1,25-dihydroxyvitamin D, and serum osteocalcin levels are also related to the levels of parathyroid hormone. During early and mid pregnancy, the stimulatory effect of 1,25-dihydroxyvitamin D on the synthesis of osteocalcin may be overridden by the inhibitory effect of declining parathyroid hormone levels. The increase in osteocalcin level in late pregnancy may be a consequence of increasing levels of both parathyroid hormone and 1,25-dihydroxyvitamin D.

Inhibition of human ovarian cancer cell proliferation in vitro by ginsenoside Rh2 and adjuvant effects to cisplatin in vivo.

In vitro and in vivo effects of ginsenoside Rh2 on human ovarian tumor growth were examined by using a cell line (HRA) derived from ascites of a patient with serous cystadenocarcinoma of the ovary. The HRA cell proliferation in vitro was inhibited in a dose-dependent manner with dosages of 10-100 microM of ginsenoside RH2. DNA, RNA and protein synthesis by the HRA cells was inhibited in a dose-dependent manner at more than 15 microM of ginsenoside RH2. However, the growth of HRA cells transplanted in nude mice was not significantly inhibited by ginsenoside RH2. On the contrary, when cisplatin was administered together with 10 microM (but not 1 microM or 100 microM) ginsenoside RH2, the tumor growth was significantly inhibited 31 days after inoculation and the survival was also significantly prolonged, compared with not only the untreated group but also the groups given cisplatin alone or ginsenoside RH2 alone. This indicates synergistic effects between cisplatin and ginsenoside RH2. From monitoring of body weight and hematocrit, concentrations of ginsenoside RH2 used in this study did not seem to cause any adverse effect.

Total lactate dehydrogenase and its isozymes in serum from patients with primary carcinoma of the ovary.

Serum lactate dehydrogenase (LDH), its isozymes and alpha-hydroxybutyrate dehydrogenase (HBD) were examined in 57 patients with primary carcinoma of the ovary and compared to those in 220 patients with benign ovarian tumor. Serum LDH, LDH-4, LDH-5 and HBD in patients with ovarian carcinoma were significantly higher than those in patients with benign ovarian tumor. Except for the percent fraction of LDH-5, LDH activity, LDH-4 activity, percent fraction of LDH-4, LDH-5 activity and HBD activity in patients with ovarian carcinoma correlated significantly with stage of disease. The positive result of ovarian carcinoma was highest at LDH-4 activity (43.6%), followed by LDH activity (42.1%), HBD activity (34.6%), LDH-5 activity (32.7%), percent fractions of LDH-4 and LDH-5 (20.0%). On the other hand, the false-positive rate in patients with benign ovarian tumor was highest at LDH activity (12.7%), followed by HBD activity (10.0%), LDH-5 activity (8.6%), percent fraction of LDH-5 (8.2%), LDH-4 activity (6.8%) and percent fraction LDH-4 (3.6%). The detection rate for early ovarian carcinoma (stages I and II) was elevated from 4.5% by HBD activity alone to 50.0% by a combination assay of LDH, LDH-4, LDH-5 and HBD, while that for advanced ovarian carcinoma (stages III and IV) was elevated from 50.0% by HBD alone to 79.4% by the combination assay.(ABSTRACT TRUNCATED AT 250 WORDS)

Levels of glucose and insulin during twenty-four hours in hyperprolactinemic women with pituitary microadenoma.

Excursions of plasma glucose and insulin throughout the 24-hour metabolic clock were evaluated in hyperprolactinemic women with pituitary microadenoma. The glucose response to food intake and 24-hour excursion of plasma glucose were not altered in hyperprolactinemic women compared to normal women. Premeal and 24-hour insulin levels were significantly increased in hyperprolactinemic women, though fasting insulin levels were not. Postprandial insulin increments appeared to be more sustained in these patients compared to normal women, though the difference was not statistically significant. And therefore the increased 24-hour concentration of insulin in hyperprolactinemic women appears to be due to the sustained insulin response to meals and increased premeal insulin levels. A slight resistance to insulin was observed in hyperprolactinemic women. That the 24-hour excursion of plasma glucose is not altered in hyperprolactinemic women in the face of decreased sensitivity to insulin may be accounted for by the increased insulin secretion.

Rodent CNS neuroblasts exhibit both perpendicular and parallel contact guidance on the aligned parallel neurite bundle.

Mouse cerebellar granule cells showed two types of migration behavior in microexplant cultures. They first migrated along their neurites, showing the typical contact guidance, and then oriented themselves at right angles to the parallel neurites, thus exhibiting the 'perpendicular contact guidance' (Nakatsuji, N. and Nagata, I. 1989 Development, 106, 441-447). To study whether other neurons have the capacity to show similar 'perpendicular contact guidance', we cultured dissociated neuroblasts from various parts of CNS or PNS on parallel neurite bundles. The PNS neuroblasts always extended their processes parallel to the neurite bundle. In contrast, almost all kinds of CNS neuroblasts tested oriented their processes both perpendicular and parallel to the neurite bundles that were all free of glia. Time-lapse video recording revealed that neuroblasts migrated in both directions. Thus, CNS neuroblasts possess the capacity to migrate and extend their processes at right angles to the substratum of heterotypic neurite bundles, which may play an important role in histogenesis of the CNS during development.

Characterization of immunomodulating action of TI-31 on antibody response in mice.

TI-31, at concentrations of 10 and 100 microM, suppressed the antibody response to T cell-dependent (sheep red blood cells, SRBC) and -independent antigens (trinitrophenyl-lipopolysaccharide, TNP-LPS and TNP-Ficoll). The anti-SRBC response was suppressed by TI-31 when T cells were replaced by a supernatant of concanavalin A (Con A)-stimulated spleen cells, indicating the drug had influence on T lymphocytes, B lymphocytes and macrophages. TI-31 augmented the Con A-induced suppressor T cell (Ts) assessed by anti-SRBC plaque-forming cells response in vitro. The drug (10 mg/kg, p.o.) potentiated antigen specific Ts induction in vitro after injection of a high dose of SRBC. The results demonstrate that TI-31 enhanced Ts induction, which resulted in a diminution of antibody formation.

Possible roles of basic fibroblast growth factor in the pathogenesis of moyamoya disease: an immunohistochemical study.

Prominent features of moyamoya disease are fibrocellular thickening of the intima and enhanced angiogenesis. The pathogenesis of moyamoya disease is, however, unknown. Basic fibroblast growth factor (FGF) is an angiogenic factor as well as a potent mitogen for a number of cell types including vascular endothelial and smooth-muscle cells. In order to test the possibility that basic FGF takes part in the pathogenesis of moyamoya disease, the authors tested for the presence of this factor using a mouse monoclonal antibody against human recombinant basic FGF. The surgical specimens studied included two sections of the superficial temporal artery (STA) and four samples of dura mater from four patients with moyamoya disease. Surgical specimens were obtained from three patients with other diseases as control tissue. Sections of the STA obtained from the patients with moyamoya disease showed strong basic FGF immunoreactivity in endothelial and smooth-muscle cells, while control sections had only faint and scattered immunoreactivity. All sections of the dura mater obtained from the patients with moyamoya disease also revealed more intense immunohistochemical staining of basic FGF in meningeal and vascular cells than did control sections. These observations indicate that the amount of basic FGF is increased in the tissues of patients with moyamoya disease; thus, basic FGF may play an important role in the pathogenesis of moyamoya disease.

[Hepatitis C virus (HCV) infection in childhood].

We studied the prevalence of hepatitis C virus (HCV) infection in childhood by determining HCV antibody (Ortho Diagnostic Systems, USA). Among patients with post-transfusion non-A, non-B hepatitis, anti-HCV was positive in 8 (72.7%) out of 11 patients. On the other hand, only 1 (25.0%) out of 4 patients of the children with acute non-A, non-B hepatitis, and only 1 (20.0%) out of 5 with chronic non-A, non-B hepatitis patients were positive for anti-HCV. Among the infants with non-A, non-B hepatitis, anti-HCV was positive in 3 (7.5%) out of 40 infants. To demonstrate mother-to-infant transmission of HCV, we examined 7 infants born to 4 anti-HCV positive mothers for their GPT values and anti-HCV. Liver dysfunction occurred in all the infants but one. And 1 infant was positive for anti-HCV. This infant with positive anti-HCV and liver dysfunction was considered to be a case of mother-to-infant transmission of HCV. And in the infants with liver dysfunction and negative anti-HCV, it was suggested that liver dysfunction occurred in association with HCV infection.

[Left atrial myxoma with cerebral oncotic aneurysms with special reference to the importance of serial angiography].

A case of atrial myxoma with cerebral oncotic aneurysms was presented. A 37 year-old woman was admitted complaining of right hemiparesis, and episodes of syncope. Neurological examination on admission revealed a right arm monoparesis, a right hemisensory disturbance, and a motor aphasia. Computed tomography (CT) demonstrated low density areas in the left fronto-parietal region, and multiple discrete enhancing high density spots scattered bilaterally in the parietal lobes. Cerebral angiography showed multiple fusiform peripheral aneurysms especially in the distribution of the bilateral middle cerebral arteries. Cerebral emboli from the cardiac source were suspected, and an echocardiography was performed, which disclosed a huge villous mass in the left atrium. The cardiac mass was resected uneventfully under cardiopulmonary bypass. It proved to be a left atrial myxoma. Postoperatively, her neurological deficit persisted. After the resection of the cardiac tumor, follow-up angiographies were undertaken twice. The second angiography performed 6 months later revealed spontaneous resolution or stabilization of most of the aneurysms detected before. On the other hand, newly-developed aneurysms were seen at the sites which had seemed normal previously on the first angiography. No remarkable changes were seen on the third one except the slight enlargement of one of the aneurysms in the middle cerebral territory. At 3 years she was neurologically stable. Because of the paucity of knowledge concerning the natural history of cerebral oncotic aneurysms and lack of definite treatment for them, long-term follow-up including serial angiography is mandatory after the resection of left atrial myxoma.

Granule cell behavior on laminin in cerebellar microexplant cultures.

In order to study roles of the extracellular matrix (ECM) in the cerebellar granule cell migration, cerebellar microexplants of neonatal to postnatal 11-day-old mice were cultured on 3 kinds of substrata, poly-L-lysine (PL), PL/fibronectin and PL/laminin. A prominent outgrowth of small granule cells, which did not uptake GABA, was observed only on the PL/laminin substratum. The granule cells showed the following sequence of events: (1) Many polygonal undifferentiated cells migrated out from the microexplants. These blast cells differentiated into small bipolar neurons with long fine neurites which extended radially from the explants. (2) These cells then changed their orientation perpendicular to their radial neurites, by protruding a short process from the cell body at right angles. (3) Finally, cell bodies of these granule cells adhered to each other to form cell aggregates. Quantitative labelings by bromodeoxyuridine revealed that there were less mitotic cells in explants from the later postnatal cerebellar compared to the earlier postnatal ones. Anti-MAP2 immunoreactivity was localized in short perpendicular processes of the aggregated granule cells. Thus, this unique cell behavior exhibited on the PL/laminin substratum provides the first defined experimental system for studying the granule cell differentiation in vitro.

Changes of lymphocyte subsets in peripheral blood before and after operation of patients with advanced ovarian carcinoma.

Comparison was made between lymphocyte subsets in peripheral blood from patients with benign ovarian tumor and those with advanced ovarian carcinoma. In addition, changes of lymphocyte subsets of patients with ovarian carcinoma before and after operation were also examined. The percentage and absolute number of CD3-/HLA-DR+ (B cells) in peripheral blood from patients with advanced ovarian carcinoma were significantly lower than values from patients with benign ovarian tumor, whereas both percentage and absolute number of CD3-/HLA-DR- (null cells) cells in patients with advanced ovarian carcinoma were significantly higher. Although there was no significant difference in natural killer (NK) cell subsets (CD57+ CD16- and CD57+ CD16+ cells) between patients with benign ovarian tumor and ovarian carcinoma, the percentage and absolute number of CD57-/CD16+ (highly differentiated NK cells) cells in patients with ovarian carcinoma were significantly higher than those in patients with benign ovarian tumor. Both the absolute number and percentage of CD3+/HLA-DR+ (activated T cells) cells in ovarian cancer patients with minimal residual tumors after operation were significantly increased, compared to the levels before operation, while the values in the patients with large residual tumors were significantly decreased. In addition, the percentage and absolute number of CD3-/HLA-DR- (null cells) cells in the patients with minimal residual tumors were significantly decreased after operation, while values in the patients with large residual tumors remained unchanged before and after operation. The patients with minimal residual tumors after operation were characterized by a significant increase in the percentage of CD57- CD16+ (highly differentiated NK cells) cells. On the other hand, in the patients with large residual tumors no change of the NK cell subsets was observed before and after operation.
