[A case of Behçet disease associated with hepatocellular carcinoma].

A 31-year-old man, suffering from Behçet disease, was admitted to our hospital because of a palpable mass in the right hypochondrium. On testing, laboratory data showed an abnormal liver function, positive HBs antigens, and a high AFP level. An abdominal CT and a SCAG revealed large tumors in the liver. The patient died of hepatic failure 6 months after the first admission and an autopsy was performed. At autopsy, hepatocellular carcinomas were found to occupy most of the entire liver. Histologically, the residual liver showed no remarkable changes, such as liver cirrhosis or chronic hepatitis. To date, few cases of Behçet disease complicating a malignant neoplasma have been reported. The possible relationship between Behçet disease and the occurrence of malignant tumors is described with a review of the literature.

[Clinical assessment of pulmonary hypertension in collagen diseases].

The occurrence of pulmonary hypertension (PH) has been reported in association with collagen diseases. Because of its grave consequences it is of greatest importance to make an early diagnosis of PH and initiate adequate treatment. For this purpose we have carried out right heart catheterization study to determine pulmonary arterial pressure (PAP), pulmonary vascular resistance (PVR), and cardiac index in thirty-four consecutive patients with collagen diseases including cases of mixed connective tissue disease (MCTD). We also compared the hemodynamic data with clinical pictures and the results of non-invasive cardiopulmonary tests which were done before catheterization study. Pulmonary hypertension as defined by the mean PAP of greater than 20 mmHg was observed in 11 of the 34 cases. PH was observed in five of 6 cases with MCTD and six of 17 cases with PSS including 2 cases with overlapping SLE and PM. The frequencies of positive anti-nRNP antibody, serositis, Raynaud's phenomenon and muscle involvement were significantly higher in cases with PH than in cases without PH. Catheterization study was most useful for diagnosis of cases with mean PAP of less than 25 mmHg, whereas echocardiographic study identified all cases with mean PAP of greater than 25 mmHg. No significant correlation was observed between mean PAP and vital capacity, total lung capacity or diffusion capacity measured as the percent of predicted normal values, which suggests that vascular abnormalities play a more significant role in the pathogenesis of PH than pulmonary fibrosis in collagen diseases.

[A case of HBs antigen negative hepatocellular carcinoma without liver cirrhosis in a young woman].

A 27-year-old woman, post parturition, was admitted to hospital because of epigastric pain. Ultrasonography and abdominal computerized tomography revealed a giant mass in the left lobe of the liver. Celiac angiography showed a hypervascular mass with irregular vascularity. Serum HBs antigen proved negative. A left hepatic lobectomy was performed in March, 1985. Histology of the tumor revealed the trabecular type of a hepatocellular carcinoma with Edmondson grade 2 atypism, whereas the non-cancerous region of the liver showed chronic inactive hepatitis. This case is presented, because of the rarity of a hepatocellular carcinoma developing in such a young patient. In addition, reported cases of a hepatocellular carcinoma in the young in Japan are reviewed.

[A study involving early carcinomas of the gall bladder].

In recent years, with the development of diagnostic procedures, the number of cases of early carcinoma of the gall bladder has been seen to gradually increase. In this paper, 8 cases of early gall bladder carcinoma have been evaluated, using a diagnostic approach. Cholecystolithiasis was the complication in 6 cases, and abdominal pain was seen as the most common symptom. Findings by various image diagnoses, such as ultrasonography, DIC, ERCP, and CT scan are described and discussed with a review of literature. Ultrasonography was considered to be the most useful tool for the detection and diagnosis of an early carcinoma of the gall bladder. A survey is thought to be required, involving the cases that have accumulated throughout the entire nation.

[Early screening of cancer of the stomach in Japan].

Gastric cancer is the most frequent malignant tumor reported in Japan, and increasing effort is being directed towards its early diagnosis. Radiographic imaging is considered to be the main exploratory investigation for screening of this cancer, fibroscopy being reserved for cases with suspected lesions on radiology. In the series studied, 40% of cases of gastric cancer were diagnosed at an early stage, a non-negligible result when one considers that the 5-year survival rate is better than 95% at this stage.

Nutritional influences on aging of Fischer 344 rats: II. Pathology.

The aim of this study was to explore the effects of nutritional manipulations on the occurrence and progression of age-related pathologic lesions in male Fischer 344 rats. The following nutritional regimens were studied: (a) ad libitum feeding, (b) food restriction initiated at 6 weeks of age, (c) food restriction initiated at 6 months of age, (d) food restriction limited to a period of early life (6 weeks to 6 months of age), (e) protein restriction without caloric restriction. The major age-related lesions observed were chronic nephropathy, cardiomyopathy, and neoplasia. Food restriction initiated at 6 months of age was as effective as food restriction initiated at 6 weeks of age in slowing the progression of chronic nephropathy and cardiomyopathy and in delaying the occurrence of neoplasia. Food restriction limited to early life was much less effective. Protein restriction in the absence of caloric restriction did not delay the occurrence of neoplasia, but it did retard chronic nephropathy and cardiomyopathy, although much less effectively than caloric restriction involving a similar level of protein restriction.

Plasticity of fat cell number.

Rats fed ad libitum (group 1) had little change in fat cell number in epididymal depots from 6 to 27 months of age, but a marked increase in fat cell number in perirenal depots between 6 and 18 months. Food restriction of rats to 60% of group 1 from 6 weeks of age on (group 2) reduced fat cell number in both depots throughout life and prevented an increase between 6 and 18 months in perirenal depots. Food restriction of rats to 60% of group 1 from 6 weeks to 6 months of age and then feeding ad libitum (group 3) markedly reduced fat cell number in both depots at 12 months compared to group 1. However, by 18 months of age, group 3 rats had the same fat cell number as group 1 rats in both depots. Food restriction of rats to 60% of the intake of group 1 started at 6 months of age (group 4) had no effect on fat cell number in epididymal depots but prevented an increase between 6 and 18 months in perirenal fat cell number. Group 5 rats fed ad libitum but restricted in protein (same caloric intake as group 1 rats) had epididymal and perirenal depots with the same characteristics as those of group 1. These findings and others in the literature show that modifying fat cell number is a widely used means of modifying fat mass during both early and adult life.

Human anticentriole autoantibody in patients with scleroderma and Raynaud's phenomenon.

Unique autoantibody was found in sera from two different patients that reacted with centrioles of both cultured mammalian cells and human peripheral leukocytes as detected by the indirect immunofluorescent method. Sera from the same individuals, one with scleroderma and the other suffering from Raynaud's phenomenon, also stained the basal bodies of rat tracheal ciliated cells by the identical technique. Data from subsequent investigations have suggested that the antigen(s) involved in the reaction are water-insoluble protein(s) or polypeptide(s) associated with centrioles and are distinct from microtubular proteins or purine nucleoside phosphorylase.

[Case of simultaneous triple cancers of the duodenal papilla, gallbladder and stomach].

The patient was a 76-year-old man with simultaneous triple cancer arising in the duodenal papilla, gallbladder and stomach, whose complaint was epigastralgia. Endoscopic and radiologic studies revealed early gastric cancer (IIc) at the posterior wall of the antrum; ERCP disclosed obstruction with "apple core" appearance in the distal bile duct. Pancreaticoduodenectomy, cholecystectomy and distal gastrectomy with lymph node dissection were performed. Histopathological examination of the resected specimens revealed papillo-tubular adenocarcinoma in the duodenal papilla, well differetiated tubular adenocarcinoma in the gallbladder, and moderately differentiated tubular adenocarcinoma in the gastric mucosa.

Longitudinal study on Schistosoma japonicum infections in the Philippines. 3. Incidence and environmental modification at Dagami, Leyte.

The yearly change of incidence of schistosomiasis japonica was observed among 1800 children enrolled in 9 schools situated within a 5 km radius around Dagami Poblacion, Leyte, Philippines from School Year (SY) 1974/75 to 1979/80. The purpose of this observation was to know the effect of environmental modification undertaken in about 50 ha of snail-infested and abandoned rice field which is the most depressed and central portion of this project area. Children were examined by egg detection from stool and by circumoval precipitin test. Coverage of examination among the school population ranged from 40 to 80% yearly and the ratio of re-examination after about 1 year was from 30 to 60%. The annual incidence in the different schools differed greatly from 12.15 to 42.86% in contrast to the annual prevalence which differed only slightly. There was corresponding increase of incidence with age from 18.6% at age 7 to 33.9% at age 12 while no significant difference was observed between sexes. The overall annual incidence rates for 9 schools during the 5-year follow-up period (SY 1975/76 to 1979/80) were 22.2, 24.2, 26.9, 9.6 and 28.4%, respectively. Those of Dagami Central II, the nearest school to the reclaimed site, were 15.7, 18.2, 19.3, 5.2 and 11.6%, respectively, for the same period. Obviously, the incidence was increasing while the reclamation was going on except in SY 1978/79. Analysis of the sudden drop of incidence in that year in all the 9 schools showed that it was not due to the effect of reclamation, but was most likely due to scanty rainfall in the previous year (1977). These observations indicated that small scale environmental modification had doubtful effect on outlying endemic areas in reducing the danger of Schistosoma infection but may have a little effect on the immediate surroundings as shown by a lesser elevation of incidence after the abrupt decrease at the nearest school as compared to those of schools located farther from the reclamation area.
