Comparison of chloroquine, albendazole and tinidazole in the treatment of children with giardiasis.

In a comparative trial, 165 Cuban children with confirmed giardiasis were randomized to receive albendazole (400 mg/day for 5 days), chloroquine (10 mg/kg twice daily for 5 days) or tinidazole (50 mg/kg, as a single dose). Parasitological follow-up was based on faecal samples collected 7 and 10 days after the completion of treatment. The tinidazole and chloroquine appeared equally effective (P > 0.05), curing 91% and 86% of the children treated, respectively, and significantly better (P < 0.01) than the albendazole, which only cured 62% of the children given it. All three drugs were well accepted and tolerated, with only mild, transient and self-limiting side-effects reported. Chloroquine appears to be a good alternative to tinidazole for the treatment of paediatric patients with giardiasis.

Total and methylmercury in a Brazilian estuary, Rio de Janeiro.

Guanabara Bay, in Rio de Janeiro state, is impacted by organic matter, oil and heavy metals. The present study evaluated the total mercury (THg) and methylmercury (MeHg) concentrations and the MeHg to THg ratio (%MeHg) in water samples from different points of the bay and in 245 organisms of three different trophic levels sampled between 1990 and 2000. Dissolved mercury concentration in estuarine water samples ranged from 0.72 to 5.23 ngl(-1). THg and MeHg in mussel, ranging from 11.6 to 53.5 microg THg kg(-1) wet wt. and 4.5-21.0 microg MeHg kg(-1) wet wt., varied according to sampling point and water quality. Planktivorous fish and mussel presented similar MeHg concentrations, meanwhile THg in planktivorous fish were lower than in mussel. Carnivorous fish showed higher THg and MeHg concentrations (199.5 +/- 119.3, 194.7 +/- 112.7 microg kg(-1) wet wt. respectively) than organisms from other feeding habits and lower trophic levels. There was a significant difference in the %MeHg among trophic levels: mussel presented lower MeHg percentage (33%) than planktivorous fish (54%) and carnivorous fish (98%).

Effect of dietary levels of vitamin E (all-rac-tocopheryl acetate) with or without added fat on weanling pig performance and tissue alpha-tocopherol concentration.

Two experiments involving 496 cross-bred pigs evaluated the efficacy of various dietary levels of vitamin E, with or without supplemental fat, on postweaning pig performance and weekly serum and terminal tissue alpha-tocopherol concentrations. The first trial involved 248 pigs weaned at an average of 15 d of age and 4.8 kg BW. The experiment was a randomized complete block design conducted in seven replicates. Vitamin E was added as dl-alpha-tocopheryl acetate at 0, 20, 40, 60, 80, 100, 150, or 200 IU/kg diet. Pigs were bled initially and at 7-d intervals for a 42-d period. Liver and s.c. adipose tissue samples were collected from six pigs per treatment group at 42 d. In Exp. 2, a 2 x 4 factorial arrangement of treatments in a randomized complete block design was conducted in seven replicates. The experiment used a total of 248 pigs weaned at 19 d of age and averaged 6.4 kg BW. Four vitamin E levels (0, 20, 40, and 60 IU/kg diet) and two added fat levels of 0 or 5% were fed for 35 d. Four pigs per treatment pen were bled weekly, and at 35 d a total of four pigs per treatment group were killed and liver, heart, and s.c. adipose tissues were collected and analyzed for alpha-tocopherol. The basal diet in both experiments contained an average 7.9 IU for period 1, and later diets averaged 11.0 IU vitamin E/kg. In both experiments serum alpha-tocopherol concentrations declined from weaning to 7 d after weaning and continued to decline each week after weaning when the basal diets were fed. Serum alpha-tocopherol concentrations increased (P < 0.01) each week as the dietary vitamin E level increased in both experiments. In Exp. 2, when fat was added to the diet serum alpha-tocopherol concentrations were higher (P < 0.01) than in diets without added fat. Liver, heart muscle, and adipose tissue alpha-tocopherol concentrations increased (P < 0.01) as vitamin E level increased, but at the higher dietary vitamin E level the liver surpassed the adipose tissue in its alpha-tocopherol concentration. Liver and adipose alpha-tocopherol concentrations were higher (P < 0.01) when fat was added to the diet. These results indicate that supplementation of 40 to 60 IU/kg diet with added fat resulted in a relatively constant balance of serum and tissue concentration of alpha-tocopherol during the nursery period, but when fat was not supplemented a dietary vitamin E level of 80 to 100 IU/kg diet may be needed. The current NRC recommendations for vitamin E for the pig from 5 to 20 kg BW may need to be reevaluated.

Methylmercury and total mercury in estuarine organisms from Rio de Janeiro, Brazil.

Guanabara Bay (GB), located in the Rio de Janeiro State, is still a productive estuary on the south-eastern Brazilian coast. It is an ecosystem heavily impacted by organic matter, oil and a number of other toxic compounds, including Hg. The present study aimed to comparatively evaluate the aquatic total mercury (THg) and MeHg contamination, and the ratios of MeHg to THg (% MeHg), in 3 species of marine organisms, Micropogonias furnieri-carnivorous fish (N = 81), Mugil spp.--detritivorous fish (N = 20) and Perna perna--filter-feeding bivalves (N = 190), which are widely consumed by the population. A total of 291 specimens were collected at the bay in different periods between 1988 and 1998. THg concentrations were determined by cold vapour AAS with stannous chloride as a reducing agent. MeHg was extracted by dithizone-benzene and measured by GC-ECD. Analytical quality was checked through certified standards. All organisms presented both low THg and MeHg concentrations and they were below the maximum limit of 1,000 micrograms Hg.kg-1 wet wt. as established for human intake of predatory fish by the new Brazilian legislation. Carnivorous fish showed higher THg and MeHg concentrations, and also % MeHg in muscle tissues, than organisms with other feeding habits and lower trophic levels. The average of THg concentrations in carnivorous fish was 108.9 +/- 58.6 micrograms.kg-1 wet wt. (N = 61) in 1990 and 199.5 +/- 116.2 micrograms.kg-1 wet wt. (N = 20) in 1998, but they presented different total length and body weights. The average THg content in detritivorous fish was 15.4 +/- 5.8 micrograms.kg-1 wet wt., whereas THg concentrations ranged from 4.1 to 53.5 micrograms.kg-1 wet wt. for the molluscs. The THg and MeHg contents of mussel varied according to the sampling point and water quality. MeHg concentration in detritivorous fish was similar to MeHg concentration in molluscs, but there was a significant difference in the MeHg/THg ratio: the carnivorous fish presented higher MeHg percentages (98%) than the detritivorous fish (54%) and the molluscs (33%). Weight-normalised average concentration of THg in carnivorous fish collected in 1990 (0.18 +/- 0.08 microgram.g-1/0.7 kg wet wt.) and in 1998 (0.16 +/- 0.09 microgram.g-1/0.7 kg wet wt.) presented no significant difference (t = 1.34; P < 0.5). In conclusion, the low THg and MeHg concentrations in the organisms from the GB ecosystem, are related to its eutrophic conditions and elevated amounts of suspended matter. In this situation, Hg could be strongly complexed or adsorbed by the particulate, which would dilute the Hg inputs and reduce its residence time in the water column, with a consequent decrease in its availability to organisms.

Heterogeneity of VH-JH gene rearrangement patterns: an insight into the biology of B cell precursor ALL.

Oligoclonal B cell proliferation, as defined by the presence of more than one leukemic clone, has been detected in approximately 20% to 30% of patients with acute lymphoblastic leukemia (ALL) using PCR or Southern blotting. An accurate assessment of these populations is required to avoid false negative measurements of minimal residual disease (MRD) in follow-up bone marrow (BM) samples of ALL patients. In this study, we analysed 29 ALL patients with two or more immunoglobulin heavy (IGH) chain gene rearrangements in the presentation samples using IGH fingerprinting PCR and sequence analysis. Thirty-nine (51%) of 76 sequences (from 15 patients), shared no VNDNJ homology (ie different CDR3 regions). In the remaining 14 patients, at least two related VH sequences were identified in each patient (identical DNJ sequences). Numerical abnormalities of chromosome 14 was detected in 10 patients. Eight patients were analysed at presentation and relapse. In four of them, expansion of a minor presentation-clone was detected at relapse while the major presentation clone disappeared, confirming 'subclonal evolution'. Finally, in our cohort of patients, the presence of related or unrelated IGH clones did not influence overall survival.

Sodium and chloride requirements of young broiler chickens fed corn-soybean diets (one to twenty-one days of age).

Sodium (Na+) and chloride (Cl-) nutritional requirements, dietary electrolyte balance (DEB), and their effects on acid-base balance, litter moisture, and tibial dyschondroplasia (TD) incidence for young broiler chickens were evaluated in two trials. One-day-old Cobb broilers were distributed in a completely randomized design with six treatments, five replicates, and 50 birds per experimental unit. Treatments used in both experiments were a basal diet with 0.10% Na+ (Experiment 1) or Cl- (Experiment 2) supplemented to result in diets with Na+ or Cl- levels of 0.10, 0.15, 0.20, 0.25, 0.30, or 0.35%, respectively. In Experiment 1, results indicated an optimum Na+ requirement of 0.26%. Sodium levels caused a linear increase in arterial blood gas parameters, indicating an alkalogenic effect of Na+. The hypertrophic area of growth plate in the proximal tibiotarsi decreased with Na+ levels. The TD incidence decreased with increases in dietary Na+. Litter moisture increased linearly with sodium levels. In Experiment 2, the Cl- requirement was estimated as 0.25%. Chloride levels caused a quadratic effect (P < or = 0.01) on blood gas parameters, with an estimated equilibrium [blood base excess (BE) = 0] at 0.30% of dietary Cl-. No Cl- treatment effects (P > or = 0.05) were observed on litter moisture or TD incidence. The best DEB for maximum performance was 298 to 315 mEq/kg in Experiment 1 and 246 to 264 mEq/kg in Experiment 2. We concluded that the Na+ and Cl- requirements for optimum performance of young broiler chickens were 0.28 and 0.25%, respectively.

Total mercury in Pema perna mussels from Guanabara Bay--10 years later.

Guanabara Bay (GB) is an important estuary in regard to the productivity of the southeastern Brazilian coast. It is heavily polluted by oil, land runoff, and sewage, had large areas reclaimed, and its basin has been seriously deforested. The objective of the present work is to compare the total mercury levels in mussels from the bay, measured at the beginning and at the end of a 10-year interval (1988 and 1998). The commonly occurring species of bivalve in the bay is the mussel Perna perna. The total mercury content in mussels from GB has probably remained constant for the last 10 years (17.3-74.1 microg Hg kg(-1)), with possible isolated fluctuations attributed to occasional changes in water quality. Mussels have a good potential as biomonitors of water quality in GB. The need for the establishment of long-term biomonitoring programs is also commented.

Genetic risk factors in acute coronary disease.

OBJECTIVE: We investigate whether each of the following: HPA-1, Factor V Leiden, prothrombin gene variant and the methylene tetrahydrofolate reductase gene (MTHFR) mutation, are risk factors for acute coronary disease in Portuguese patients. MATERIAL AND METHODS: 100 blood donors and 52 patients with an established diagnosis of myocardial infarction or unstable angina were evaluated for genetic risk factors, by determining HPA-1 genotype, Factor V Leiden, Prothrombin 20210 variant and MTHFR mutation. RESULTS: We found a prevalence of 2.0% for Factor V Leiden, 5.0% for the Prothrombin 20210 variant and 66% for the MTHFR mutation in blood donors. These values are similar to those found in the patients (1.9, 3.8 and 58%, respectively). We found that 28/100 controls had the PI(A2) polymorphism, a frequency statistically different from that in the patients (23/52). This difference was even more pronounced in patients less than 60 years old (27/96 vs. 13/24). CONCLUSION: Factor V Leiden, Prothrombin 20210 variant and MTHFR mutation do not seem to represent risk factors for acute coronary disease. However, the PI(A2) polymorphism could have a role in the pathogenesis of this disease. The presence of multiple genetic factors, more than single ones, could influence the development and outcome of myocardial infarction and unstable angina. Larger studies are needed in order to have a better insight into the pathophysiological mechanisms of this disease, along with its prevention and the development of new treatments.

Mercury in a widely consumed fish Micropogonias furnieri (Demarest, 1823) from four main Brazilian estuaries.

We analyzed mercury levels in the white muscular tissue of a fish (Micropogonias furnieri) originated from four Brazilian estuaries during the period between the summer 1990 and spring 1991. Mercury levels were below the maximum permissible limit established for food by the Brazilian legislation (500.0 ng.g-1 wet wt.). The measured mercury concentrations ranged from 17.4 to 266.0 ng.g-1 (wet wt.), for Guanabara Bay 19.5-252.2 ng.g-1 (wet wt.), for Sepetiba Bay 23.0-433.7 ng.g-1 (wet wt.), for Ilha Grande Bay 13.8-296.3 ng.g-1 (wet wt.) for Conceição Lagoon. Positive relationships were observed between mercury concentration in fish and the length and weight of the fishes. Weight normalized concentration showed Guanabara Bay presented the higher levels and the Conceição Lagoon presented the lowest levels. Ilha Grande Bay presented the second highest levels after Sepetiba Bay, although the former is considered a non-contaminated area.

Analysis of the essential sequences of the factor VIII gene in twelve haemophilia A patients by single-stranded conformation polymorphism.

We report the analysis by single-stranded conformation polymorphism of the essential sequences of the factor VIII(FVIII) gene (total length about 14 kb) including the entire coding sequence, flanking intronic sequences and the putative regulatory sequences 5' to the gene, in twelve unselected haemophilia A patients of Portuguese origin. Direct sequencing of the fragments with an altered migration pattern led to the identification of the disease-producing mutations in five patients. Three of these mutations, namely a 1 bp insertion in a motif of eight consecutive A residues at codon 1439 (FVIIIPorto3); a C to T transition at codon 1966 (Arg-->Stop), found in an inhibitor-positive patient (FVIIIMontijo); and a G to A transition at codon 479 (Gly-->Arg; FVIIIPorto1), have been reported in other ethnic groups. The two novel mutations are the substitution of AG by GG at the 3' end of intron 4 (FVIIILisboa1) destroying the invariant splice acceptor sequence, and a G to A transition at codon 1948 resulting in an aspartic acid substitution for glycine (FVIIIPorto2).

[Value of transesophageal echocardiography in the diagnosis of acute aortic dissection].

We describe our experience of six patients with clinical suspicion of acute aortic dissection (AAD) who were studied consecutively by transesophageal echocardiography (TEE) from April to July of 1991. All of them were previously submitted to transthoracic echocardiogram. The diagnosis was correctly established by TEE in five cases, confirmed by aortography and/or surgery (four cases), or by autopsy (one case). In one patient the diagnosis of AAD was excluded by TEE, and posteriorly by nuclear magnetic resonance. Four patients had a Stanford type A, and one patient a type B dissection. The site of entry was identified in three cases; the intimal entry tear of the type B dissection, not observed by TEE, was localized in the aortic arch by aortography. In three of the four type A dissection cases, a thrombus in the false lumen and an aortic regurgitation were found. No other noninvasive methods were used after the diagnosis of AAD by TEE. The surgical repair was successful in three cases, one of which, without previous necessity of aortography. In our experience, TEE increased extraordinarily the diagnosis efficacy of AAD, making possible an earlier therapeutic approach, and probably contributing to the improvement of the prognosis of this pathology.

[Pericarditis caused by cholesterol. Apropos of a clinical case].

A case of a 76 year-old woman with a large pericardial effusion is described. The microscopic examination of the scintillating, yellow-greenish fluid obtained by the first pericardiocentesis, showed numerous crystals of cholesterol. In search of the underlying diseases there were found values of T3, T4 and TSH diagnostic of hypothyroidism. Considering the rapid relapse of the effusion and after a second pericardiocentesis, it was decided to open a pleuro-pericardial window. The biopsy of the pericardium showed deposits of cholesterol on electronics microscopy. She was discharged from hospital two weeks after surgery, showing an evident clinical improvement. Six months later she was feeling well, keeping a normal activity for her age. The values of T3, T4 and TSH were normal, and there were no echocardiographic signs of pericardial effusion. It is presented a rare case of cholesterol pericarditis associated to sub-clinical hypothyroidism, showing the presence of cholesterol, as crystals in the pericardial effusion and, we think for the first time in this pathology, in vesicles of pinocytosis, in the pericardial biopsy. The six months period of follow-up showed a good clinical improvement with the selected therapeutic attitude.

The use of instrumental neutron activation and multivariate statistic analyses in differentiation of Brazilian phosphate ores.

Phosphated rocks from three ores located in the Middle-West region of Brazil can be used as low-level radioactive reference material due to the fact that they contain significant concentrations of thorium and, in some cases, of uranium. This paper describes the procedure to characterise those samples using several analytical methods. Statistical analysis and correlations studies have shown several geochemical similarities and the results obtained suggest the possibility to produce a reference material composed of a mixture of two similar ores.
