[Histamine liberation and specific IgE against Dermatophagoides pteronyssinus in parasitized patients].

We studied 98 patients with different parasitosis, without clinical symptoms of mite sensitization, most of them coming from Guinea. Histamine release to Dermatophagoides pteronyssinus was performed using whole blood. Specific IgE to the same antigen was measured by EAST, as well as by an immunodot with the same antigen extract employed for the histamine release test. Finally, the EAST positive sera were studied by immunoblotting. The presence of specific IgE by EAST could be proved in 31 patients, but these antibodies were nor detected by dot, blot and histamine release. On the other hand, only two patients showed a positive histamine release test to D. pteronyssinus and in these two cases the EAST to mites was negative. There was no relation between total IgE levels and specific IgE to mites. The presence of mite-specific IgE showed a significative association to the parasite Trichuris trichiura (odds ratio 3.09). This fact suggest that the specific IgE values found in this population can reflect some cross-reaction between parasites and allergens. It is the author's opinion that the same study should be performed in european patients in order to test the relationship between mite-specific antibodies and the presence of parasites, specially of Trichuris trichiura.

Occupational asthma caused by Papaver somniferum.

Twenty eight workers of a pharmaceutical factory were studied. The company produced morphine and other alkaloids extracting the active principles from shells of Papaver somniferum. Six of the exposed subjects had clinical symptoms of sensitization to this allergen and positive skin tests. A bronchial provocation test was found to be positive on 4 workers and specific IgE could be found on the 6 sensitized patients by an ELISA and a RAST test using an aqueous extract of P. somniferum. A histamine release using the same antigen was also positive in the 4 samples from sensitized patients available. An SDS-PAGE of the extract revealed a major protein band with an estimated mol wt of 52,000 d. This band had the highest IgE-binding capacity as shown by immunoblotting. All these facts suggest that P. somniferum allergy is mediated by an IgE mediated mechanism and not by a pharmacological or toxic effect of the alkaloids or polyphenols.

Do latex allergens potentiate other skin tests?

We report the case of a 33 year old atopic female patient, who worked for the last 4 years as a cleaner in the surgical area of a hospital. One year after starting the work in this area, the patient noticed upper respiratory symptoms and soon later dyspnea immediately after entering the operating room. Other allergic symptoms were perennial rhinoconjunctivitis and urticaria after contact with peaches. Common skin prick tests were positive for Dermatophagoides pteronyssinus. Fifteen min later a skin prick test with latex was done with a strong positive result after 10 min. At the same time, several allergens (cat dander, grass and olive pollens) showed positive results. The patient experienced a fall in the FEV1 that returned to basal levels after two inhalations of a bronchodilator. New skin tests to common allergens performed one week later were only positive to mites.

Covalent-binding native blot in the determination of specific IgE to latex gloves allergens. Comparison with other methods.

Six sera from patients with clinical symptoms of latex hypersensitivity and latex-RAST values ranging from class 1 to 4 were used to characterize latex-glove allergens by immunoblotting. Twelve % acrylamide-SDS-PAGE of the latex glove crude extract revealed a single band with an estimated mol wt lower than 14,000 D. The proteins bound to nitrocellulose after electrical transfer as shown by Ponceau red staining of the membrane. However, standard immunoblotting methods using alkaline-phosphatase anti-IgE did not detect the presence of specific IgE. Different variants of SDS-PAGE immunoblotting were also unsuccessful. On the other hand, the use of IEF and capillary transfer (native blot) to a covalent-binding hydrophilic membrane was found to be the method of choice for the study of this allergen. The authors believe that this procedure can be helpful in the study of other antigen-antibody systems, particularly when standard immunoblotting assays fail.

Renaturation of blotted allergens increases the sensitivity of specific IgE detection.

Several authors have demonstrated that renaturation is an essential step for the appropriate recognition of blotted proteins. The use of nonionic detergents has been described as a useful alternative to enhance the antigenicity in immunoblotting, although elution from proteins by detergents has been observed. To measure the influence of different factors on the sensitivity of specific IgE by immunoblotting, we used twenty human sera from atopic patients who were allergic or nonallergic to a common, reliable allergen (grass pollen mixture). The use of Nonidet-P40 was found to be a useful alternative for the renaturation of the allergens. No elution from the membrane was found when employing this detergent, even at high concentrations (3%), and its use gave better sensitivity than methanol. On the other hand, we detected that methanol possessed renaturing properties. A transfer method using diffusion instead of electric transfer gave the best results and two membranes could be obtained from each gel. Using this method, we found that after NP-40 incubation of the membrane, the use of bovine albumin could be omitted as blocking agent and that its use had even deleterious effects.
