Experimental pancreatolithiasis in the dog.

Little is known about the pathogenesis of pancreatolithiasis, and although several theories have been proposed, no experimental models of pancreatic calculi production are documented in the literature. This study examines such a model in which partial or complete pancreatic duct obstruction was surgically created in dogs. We found that (1) pancreatic calculi were demonstrated in 46.7% of the dogs (14 of 30) with partial outflow obstruction for 4 months or longer; (2) no pancreatic calculi were found in any of the dogs with complete pancreatic duct obstruction; (3) all calculi produced were localized in the ductal system; (4) the organic and inorganic composition of canine and human pancreatic calculi were quite similar; and (5) connective tissue proliferation and mucous cell metaplasia of the ductal epithelium, often seen in association with clinical pancreatolithiasis, are also detected in the pancreata of dogs that have had partial obstruction of pancreatic excretion. These findings suggest that an experimental model of pancreatolithiasis can be accomplished in dogs by the partial obstruction of pancreatic excretion.

The superselective and the selective one shot methods for treating inoperable cancer of the liver.

Forty-five patients with inoperable cancer of the liver were treated by the one shot administration of 15-40 mg of Mitomycin C into the hepatic artery, either by the superselective or by the selective one shot method. Fourteen of the patients had primary cancers of the liver, and 31 had metastases to the liver from primary cancers of the stomach, or from the colorectal or other organs. Subjective symptoms improved in 73%, and objective signs improved in 60%. Nineteen patients who received this treatment more than twice showed a mean survival time of 10 months and a 50% survival time of 7.8 months. Therapeutic effects of the selective one shot method were recognized mostly in patients with tumors which were rich in vessels. However, a fairly good result was obtained using the superselective one shot method, even in patients with tumors having relatively few vessels.

Transplantation of preserved pancreatic islets into the portal vein of rats.

Langerhans islets were isolated from the exocrine pancreata of Wistar rats by the improved collagenase-digestion method. The isolated islets were preserved in a tissue culture medium for seven days. Transplantation of these preserved islets into the portal vein of streptozotocin-induced diabetic rats resulted in a significant reduction of hyperglycemia, polyuria and glucosuria, and a restoration of weight gain. It was found that these effects could be maintained for 16 weeks. In order to normalize the K-values and plasma insulin levels, at least 600 islets had to be transplanted into each diabetic rat.

Insulin-releasing activity and successful transplantation of pancreatic islets preserved by tissue culture.

Preserving pancreatic islets for 7 days by a making use of the organ culture, we studied the insulin-releasing activity at the time of administration of glucose and various digestive tract hormones for the purpose of clarifying the function of preserved pancreatic islets. Furthermore, we transplanted pancreatic islets preserved for 3 to 5 days into the portal vein of rats with streptozotocin-induced diabetes and reached the following conclusions: (1). The islets of Langerhans of the pancreas responded well to glucose up to the seventh day of preservation and showed patterns similar to those of fresh pancreatic islets with respect to both the dose response and the time response. (2). Preserved pancreatic islets of the pancreas had insulin-releasing activity almost equal to that of fresh pancreatic islets against stimulation by glucagon, tolbutamide, and various digestive tract hormones. (3). Rats with streptozotocin-induced diabetes showed a marked improvement in blood glucose and urine glucose following transplantation of preserved pancreatic islets into the portal vein, and this effectiveness persisted for 6 to 8 weeks.

Clinical effects of anticancer drugs to pancreatic diseases as protein synthesis inhibitors.

The anticancer drugs, like 5-Fluorouracil, which are believed to interfere with enzyme protein synthesis in the exocrine cells of pancreas were administered intravenously to fifteen patients with various pancreatic diseases. The improvement of clinical symptoms and the diminution of serum and urinary amylase levels were observed in four cases with acute pancreatitis and two cases with chronic relapsing pancreatitis. The postoperative complications, namely the formation of pancreatic fistula and the rupture of pancreaticojejunostomy, or the aggravation of concomitant pancreatitis were not observed in three cases with benign surgical pancreatic diseases and six cases with pancreatic carcinoma. Furthermore, the diminution of amylase and protein output of pancreatic juice from canulae inserted into pancreatic ducts were observed.
