MRI findings of the upper eyelid and their relationship with single- and double-eyelid formation.

Before the invention of magnetic resonance imaging (MRI), it was impossible to observe an architectural deformation of the eyelid because of its movement. The authors observed MRI films of 15 eyelids in both closed and opened positions and obtained new information on the architecture of the upper eyelid and also the mechanism of single and double eyelids, and sunken eye formation. (1) Orbital fat is transposed when the lid moves. If the fat can not return into the orbit when the lid opens, it droops and interferes with the fold formation of the lid. (2) The thickness of the eyelid skin is associated with double-eyelid formation. The skin fold is observed at the junction between thick skin with subcutaneous fat and thin skin without it. (3) We could not confirm Doxanas and Anderson's assertion that septal insertion was lower in Orientals. However, we observed pretarsally drooped or herniated orbital fat in many slit-eye Orientals. (4) There are three angles at the tarso-levato-aponeurotic line: one at the junction with transverse ligament, one at the point of septal insertion, and another at the aponeurotic terminal on the tarsus.

[Ocular symptoms in mitochondrial myopathy].

The corneal endothelial cells in 6 cases of mitochondrial myopathies were examined by specular microscope, and electron microscopic studies of the cornea were made in one autopsy case. The cornea was clear and the thickness was within the normal range. The endothelial cell mean size was larger in 3 out of 10 eyes than among controls and the variance of endothelial cell size was larger in all of 10 eyes than among controls. The difference was found to be statistically significant. Electron microscopical findings of the cornea revealed mitochondrial abnormalities. Inclusions were observed in the stromal keratocyte.

Operative experience with malfunction of the eyelid.

During the past 10 years, the authors have observed several cases of malfunction of the eyelids, such as congenital lid retraction, lagophthalmos accompanying facial nerve palsy, ocular myopathy, blepharochalasis, traumatic lid ptosis, and Horner's syndrome. Several operational techniques were utilized in these cases. Some operative cases are presented together with pre- and postoperative photographs. To obtain aesthetically pleasing results, the authors felt the most exacting techniques were necessary for even minor deformities.

[Evaluation of the role of brain scintigraphy in pineal tumors (author's transl)].

From January, 1969 to December, 1975, 1700 patients underwent sequential brain scintigraphy with Tc-99-m-pertechnetate. All the scintigrams were obtained on a scintillation camera with high-resolution collimator in our clinic. Intracranial tumors were confirmed in 449 cases and 21 cases out of those were pineal tumors which were diagnosed by clinical symptoms, air study, angiography and Conray-or Myodil-ventriculography. In these pineal tumors, 14 (67%) out of the 21 cases had positive scan on scintigrams. All of our cases of teratoma (one case) and teratocarcinoma (two cases) showed clear abnormal uptake, though it had been reported that they do not tend to have positive results on scintigrams. We also performed scintigraphy on 11 cases out of the 21, both before and after 60Co radiation therapy. In the cases which showed either same grade or more increased abnormal uptake on scintigrams after radiation therapy, compared with that before radiation therapy, compared with that before radiation, their prognoses were poor. On the other hand, those cases which showed decreased abnormal uptake after radiation, have had a healthy daily activities. From these experiences, it may be concluded that brain scintigraphy is clinically valuable not only for making the diagnosis, but for evaluating the effect of radiation therapy and the prognosis in the cases of pineal tumor.

Control of solitary gastric fundal varices and portosystemic encephalopathy accompanying liver cirrhosis by balloon-occluded retrograde transvenous obliteration (B-RTO): a case report.

In a patient with liver cirrhosis complicated by solitary gastric fundal varices and portosystemic encephalopathy, Balloon-occluded retrograde transvenous obliteration (B-RTO) of the varices was performed. The gastric varices were decreased in size 2 weeks after treatment and had not recurred after 1 year. B-RTO successfully occluded the portosystemic shunt (gastrorenal shunt). Accordingly, the patient's blood ammonia levels, total bile acid level, and 15 min retention rate of indocyanine green decreased, and his hepatic encephalopathy improved. However, since consecutive increase in blood flow through the portal collateral vessels except for gastrorenal shunt vessel at 6 months and 1 year after B-RTO was noted, further careful follow-up may be required.

Percutaneous microwave coagulation therapy for superficial hepatocellular carcinoma.

We report a 67 year-old man with residual hepatocellular carcinoma after arterial embolization therapy, which was located on both the anterior and inferior surfaces of segment 6 of the liver. Percutaneous microwave coagulation therapy could be performed safely and the treated tumor became non-enhancing on contrast computed tomography. Two years after treatment, the tumor remains non-enhancing on contrast computed tomography and has decreased in size. Percutaneous microwave coagulation therapy appears to be useful even in patients who have superficial liver tumors associated with cirrhosis.
