Liver granulomas: a possible paraneoplastic manifestation of hypernephroma.

A patient with hypernephroma presented with signs of liver dysfunction. Liver biopsy disclosed the presence of granulomas. Extensive search for a known cause of hepatic granulomas was negative. The presence of granulomas in lymph nodes draining the neoplasm, the improvement of liver dysfunction after nephrectomy, and its aggravation preceding overt metastatic manifestation support the existence of a cause and effect relationship between the renal cell carcinoma and the liver granulomatous disease.

Thromboembolism in congenital factor VII deficiency.

An Israeli family of Persian origin was investigated for factor VII deficiency. The proposita, derived from a consanguineous marriage, had repeated thrombophlebitis attacks, pulmonary embolism and rectal bleeding. The family studied and the proposita had low factor VII procoagulant activity and low factor VII antigen.

Neurological complications following rubella infection.

Neurological complications following rubella are only rarely encountered. However, in many cases severe neurological impairment may occur, leading to permanent disability. In a recent epidemic of rubella in Israel during the years 1978-1979, 20 patients with severe neurological complications have been seen. We report on 5 cases of which 3 are described in detail. Considering the efficacy of immunization against rubella, we suggest that in countries such as England and Israel in which periodic epidemics of rubella occur, a generalized plan of immunization should be undertaken during the first years of life. Such an approach will prevent the neurological complication of congenital, as well as acquired, rubella infection.

Pulmonary function abnormalities in Sjögren's syndrome and the sicca complex.

The frequency of pulmonary involvement in a group of 20 patients with Sjögren's syndrome or the sicca complex was evaluated with pulmonary function studies. In 12 patients pulmonary functional abnormalities were demonstrated. The most common abnormality was airway obstruction. Nine out of 13 patients with the limited variant of the disease (sicca complex) and three out of seven patients with the complete syndrome had abnormal pulmonary function.

Blood hyperviscosity in psoriasis.

A study of platelet function and whole-blood viscosity in 11 patients with the confirmed diagnosis of psoriasis revealed a significant elevation in whole-blood viscosity (3.33 +/- 0.37 cP) as compared with that found in a control group (2.80 +/- 0.1 cP). The platelet count and platelet aggregation with ADP, epinephrine and collagen as well as platelet malonyldialdehyde were all within the normal range in all the patients. It is suggested that the increased blood viscosity may contribute to the higher incidence of occlusive vascular disease occurring in patients with psoriasis.

Clinical trial of bromhexine in Sjögren's syndrome.

Tear secretion and lysozyme tear content were measured in 30 patients with Sjögren's syndrome after treatment with oral bromhexine, 32 mg/day. In 21 patients (70%) there was a marked increase in tear secretion and in lysozyme content. In patients with keratoconjunctivitis sicca (KCS) good results in clarifying the mucoid eye discharge were obtained. A remarkable amelioration of xerostomia was also noted. Six other patients, serving as controls, were given a placebo and bromhexine. The placebo had no influence on the rate of tear secretion, while bromhexine caused it to increase in 70% of the controls. This side effects of bromhexine treatment encountered in the present study were negligible and transient. We consider bromhexine to be the drug of choice in the treatment of Sjögren's syndrome.

Lysozyme content of tears in patients with Sjögren's syndrome and rheumatoid arthritis.

In 37 patients and 143 control patients we estimated tear fluid lysozyme content by the Micrococcus lysodeikticus agar diffusion assay. We found no correlation between the titer of lysozyme in tear fluid and the rate of tear flow. Decrease in lysozyme production was found to be a sensitive indicator of the involvement of the lacrimal system in Sjögren's syndrome.
