The peak velocities of horizontal saccades were measured in patients with myasthenia gravis (MG) to determine whether they can differentiate MG from other causes of ophthalmoplegia. Eye movements were recorded with electrooculography (EOG) or infrared scleral reflection (IR) in 42 patients with MG, 26 patients with sixth cranial nerve palsy (CNP), 19 patients with chronic progressive external ophthalmoplegia (PEO) and 28 normal subjects. Despite limitation of ductions in MG, the group means of velocities of 10 deg saccades recorded with IR were similar in MG and normal subjects. With EOG, small but statistically significant decreases in mean velocities of 10, 20 and 30 deg saccades were found in MG, compared to those in normal subjects. Twenty-one to 28% of MG patients had velocities outside of the normal range (outliers). In contrast, the group means in CNP and PEO were markedly lower than those in MG and normal subjects. The frequencies of outliers were 89 to 100% in CNP and 88 to 100% in PEO. Measurement of saccadic velocities can be helpful in differentiating MG from other causes of ophthalmoplegia.
The response of eye movements to edrophonium is easily missed by clinical observation alone. Binocular horizontal ten degree saccades were recorded by infrared oculography, whilst the vision of one eye was occluded, before and after fatigue repeated intravenous injection of dilute edrophonium, and fatigue induced during anticholinesterase inhibition by intravenous edrophonium, in 26 patients with diplopia or ptosis of uncertain aetiology. The most reliable criterion of a positive response was an increase in the amplitude of the saccades of the fixating eye by 10% or more after each of several injections of dilute edrophonium. The response was positive in 13 patients and was difficult to observe clinically when the responses of the saccades of the eyes moving conjugately were unequal and when the patient presented with ptosis and no diplopia. Edrophonium infrared oculography proved to be a sensitive test for weakness due to the neuromuscular junction defect of myasthenia gravis affecting extraocular muscles.
Computed tomography detected an enlarged thymus in a 27-year-old man with myasthenia gravis of recent onset. Pathological examination of the thymus revealed lymphoid hyperplasia and a thymic seminoma (or germinoma), which was arising as an intramural nodule in a thymic cyst. This is the first reported association of thymic seminoma, lymphoid hyperplasia, and myasthenia gravis.

See-saw ocular movements are described in two patients, one having obstructive hydrocephalus and the other a thalamic infarct. Electro-oculographic studies demonstrated that the eye movements in patient 1 moved in and out of phase at irregular intervals, in both a horizontal and a vertical direction. The disconjugate eye movements were exaggerated in bright light and less evident during fixation. We suggest a lesion impairing the function of that circuitry of cells thought to include the nucleus centromedianus of the thalamus, the zona incerta, the interstitial nucleus of Cajal and eye movement related cells in the mid brain and pons, causes the disconjugate eye movements but that the controlling influence of multiple connections with other parts of the brain results in the ever changing pattern of disconjugate eye movement.
This study describes microthrombotic emboli (with their ultrastructure) and emboli of fat and of dimethylpolysiloxane with silica in the retinal microcirculation of two patients who died-each after his second open heart operation. The presence of silica was confirmed by electron probe x-ray micro-analysis.

The paper describes the principal achievements of the UN/Thai programme for drug abuse control which was carried out during the years 1972-1979. The programme was of a pilot nature and covered five selected key villages and 25 satellite villages in the north of Thailand. The major components of the programme were crop replacement and community development, treatment and rehabilitation, and drug information and education. The Royal Thai Government has accepted the achievements of this project as models for greatly expanded programmes for the eradication of the opium poppy cultivation. Two key villages-Ban Mae Tho and Hui Tund Jaw which were included in a pilot programme have provided to be opium free.
