Immunochemotherapy of bladder carcinoma with glucan and cyclophosphamide.

Recent evidence suggests a role for both immunotherapy and chemotherapy in the treatment of transitional cell carcinoma. Glucan, a derivative of the cell wall of Saccharomyces cerevisiae and a potent immunostimulant, was used in combination with cyclophosphamide for treatment of implanted murine transitional cell carcinoma (MBT 2). Cyclophosphamide prevented tumor appearance when tumor burden was low and decreased tumor growth rate in larger tumor volumes, but was unable to eradicate established tumors. Glucan did not reduce tumor incidence but decreased animal mortality. These experimental observations may correlate well with clinical evidence and suggest future clinical use of these agents.

Medullary sponge kidney and congenital hemihypertrophy.

We have presented a case of medullary sponge kidney associated with congenital hemihypertrophy. Evaluation by calcium loading revealed no abnormal handling of this ion. The recognition of this unusual syndrome is important because it appears that other disease processes common to medullary sponge kidney alone or congenital hemihypertrophy alone do not appear with the same frequency when both processes occur simultaneously.

The teaching of legal medicine in British medical schools.

In view of recent public anxiety concerning possible inadequacies in the field of legal medicine and ethics, a questionnaire survey was made of medical students, newly qualified housemen, medical school deans, coroners and medical defence and protection organizations. The results indicate that the teaching of legal medicine and ethics is insufficient, and a recommendation is made for an increase in such teaching.

Tryptophan aminotransferase activity in rat liver.

By using an antiserum raised against rat liver tyrosine aminotransferase, it was shown that about 60% of tryptophan aminotransferase activity in rat liver extracts is catalysed by this enzyme. Induction of tryptophan aminotransferase activity by intraperitoneal injections of tryptophan or triamcinolone can be entirely attributed to the effects of these agents on tyrosine aminotransferase. The origin of the other 40% of tryptophan aminotransferase activity remains to be established. This activity increases after starvation for 48 h. It is unlikely that tryptophan transamination plays a quantitatively important role in the metabolism of tryptophan by the liver.

Adenocarcinoma of the prostate: results of routine urological screening.

The results of routine urological screening of 2,005 men between 40 and 70 years old were reviewed. Digital rectal examination was used to screen for adenocarcinoma of the prostate. Of 65 nodules detected 17 proved to be adenocarcinoma. Clinical staging revealed no evidence of metastases in all but 2 patients but pathological (surgical) examination resulted in upstaging of disease in 66 per cent of the patients who underwent an operation. Digital rectal examination proved to be an insensitive screening device with poor predictive value. The use of adjunctive screening tools for detection of adenocarcinoma of the prostate is suggested.

The renal quantitative scintillation camera study for determination of renal function.

The renal quantitative scintillation camera study assesses glomerular filtration rate and effective renal plasma flow based upon renal uptake of 99mtechnetium-iron ascorbate and 131iodine-hippuran, respectively. The method was compared to inulin, para-aminohippuric acid and creatinine clearance studies in 7 normal subjects and 9 patients with various degrees of reduced renal function. The reproducibility of the technique was determined in 15 randomly selected pediatric patients. The values of glomerular filtration rate and effective renal plasma flow were not significantly different from those of inulin and para-aminohippuric acid studies. The reproducibility of the technique was comparable to that of inulin and para-aminohippuric acid studies. Patient acceptance of the technique is excellent and the cost is minimal. Renal morphology and excretory dynamics also are demonstrated. The technique is advocated as a clinical measure of renal function.

Prostatism and inguinal hernia.

We reviewed a series of 70 patients who had transurethral resection of the prostate to determine the incidence of inguinal hernia. We found inguinal hernias in 20% of the patients, a figure significantly higher than in the general population. In 47% of the patients a hernia was present on admission or herniorrhaphy had been done previously. Urine flow rate has been previously found to be an excellent screening measurement for prostatism, a concept confirmed by this series. Because benign prostatic hyperplasia has associated morbidity and because its presence is not desirable in patients having herniorrhaphy, urine flow rate is advocated as a routine screening test for prostatism in patients with inguinal hernias.

Coronal renal ultrasound: I.

With recent advances in ultrasound technology, concomitant improvement in sonographic techniques have been required. Sonographic scanning in coronal orientation, both in transverse but especially in longitudinal direction, has demonstrated a number of advantages over the routine transverse and sagittal scans. Closer correlation with functional anatomy of the kidney, more comprehensive scanning with less inerference from adjacent tissues, and better understanding by the clinician are the most obvious advantages of the coronal approach.

Coronal renal ultrasound: II.

Coronal renal imaging with ultrasound has a number of advantages over other routine scanning orientations in nephrosonography. In a series of 214 patients coronal transverse but especially longitudinal orientation was found to be superior to other scanning methods. The main advantages were: differentiation between renal cysts and hydronephrosis as well as demonstration of renal lithiasis. This approach also should be used to augment the diagnostic sequence of investigating renal masses.

Ultrasound followup of renal cyst puncture.

The place of ultrasound and cyst puncture in the evaluation of renal masses is reviewed. Complications of cyst puncture, although rare, do occur. A review of 16 cases of renal cyst puncture revealed only 2 minor complications: in 1 case a pericystic hemorrhage occurred and in 1 case an intracystic hemorrhage was detected. Both complications were asymptomatic and detected by ultrasound. Ultrasound has proved to be an excellent means of followup for renal cyst puncture.

Permanent cutaneous ureterostomy: 18 years of experience.

We have reviewed 84 patients managed with cutaneous ureterostomy during the last 18 years. If at least 1 hypertrophied and dilated ureter is available cutaneous ureterostomy is a safe and effective diversion, with satisfactory late functional results. Stomal stenosis remains a continuing problem but usually is managed easily if present. Cutaneous ureterostomy should be used in selected patients until the ideal form of vesical substitution is achieved.
