[Marked variations in phototherapy policy for neonatal jaundice in Dutch centers].

OBJECTIVE: To determine what guidelines are used in Dutch hospitals in the management of neonatal hyperbilirubinaemia. DESIGN: Descriptive. MATERIAL AND METHOD: Paediatricians in 63 hospitals were sent a standard guideline diagram of phototherapy management and asked to describe how their management differed. The 10 neonatal intensive care units in the Netherlands were asked to send their phototherapy guidelines in more detail. RESULTS: Thirty-four hospitals (54%) and the 10 neonatal centres (100%) responded. The data obtained showed no general agreement; several different guidelines were used, notably with respect to the serum bilirubin concentration levels and the number of postnatal hours. CONCLUSION: We recommend that consensus should be reached on the use of phototherapy. A more restricted use of phototherapy probably would save costs without loss of quality.

[A survey of the diagnostic policy in nocturnal enuresis].

A written enquiry regarding the current diagnostic policy in nocturnal enuresis was conducted among paediatricians (response 38%, n = 22) and urologists (response 49%, n = 23) while an enquiry by telephone was held among GPs (n = 40) with the same questions. With reference to three case histories, the question was asked what supplementary examinations, if any, would be carried out. All respondents showed a tendency to perform extensive urological examinations. Since bedwetting in most cases is not associated with demonstrable anatomical or functional abnormalities, this is incorrect for medical reasons, even apart from the costs involved.

[Myocardial infarct in a newborn infant].

Myocardial infarction is rarely described in the newborn. We report a case in which the newborn had a normal heart and probably normal coronary arteries. The only presenting sign was transient extrasystolic beats. The course was uneventful and our patient recovered spontaneously. A review of the literature is presented.

[Wilms' tumor with an unusual presentation].

The case history of a 8 year old boy is described who presented with haematuria without other signs or symptoms. No abdominal mass could be palpated. Excretory urography revealed a tumor in the right pyelum . Histological examination confirmed the diagnosis. The tumor was mainly located in the submucosal area of the right pyelum and proximal ureter.

Urinary retention without organic cause in children.

Acute urinary retention without organic explanation is well known in adults but has only rarely been described in children. Six cases (four boys and two girls) of non-organic acute urinary retention are reported. The diagnosis was based on negative findings on extensive urological examination. So-called psychogenic urinary retention in children is associated with constipation and infrequent voiding.
