[Reactive arthritis].

In most cases, reactive arthritis occurs after urogenital and gastrointestinal infections. Anamnesis, clinical picture, antigen HLA-B27 determination and serologic antibody ascertainment are of essential importance to the diagnosis. The therapeutic value of antibiotics is questionable, owing above all to the fact that in chlamydia-induced arthritis the causative agent was found in the intraarticular biopsy specimen. The prognosis of the disease is favourable although relapses are frequent. Especially Reiter's disease may have a chronic course.

[Identification of crystals in synovial fluid].

The basic principles of the determination of crystals in synovial fluid by means of the polarisation microscope are described. The presence of crystals in synovial fluid was determined in 48 patients with the following clinical diagnoses: rheumatoid arthritis, gout, pseudogout, reactive arthritis, in patients with a diagnosis of extraarticular rheumatism: peritendinitis, in patients with reactive arthritis and in 2 patients with injured knee joint. Crystals of Na urate, Ca-pyrophosphate dihydrate and cholesterol were found. The analysis of synovial fluid for the presence of crystals in an important diagnostic procedure contributing greatly to the quick and correct diagnosis of arthritis.

[Sulfasalazine in the treatment of ankylosing spondylitis].

Ninety-five patients with ankylosing spondylitis received either sulphasalazine (less than or equal to 3 g/day) or placebo for 24 weeks. The patients who received sulphasalazine showed significantly improved clinical parameters [duration of morning stiffness (p less than 0.05), the number of painful and swollen joints (less than 0.05)] and laboratory parameters [erythrocyte sedimentation rate (p less than 0.001), haptoglobin (p less than 0.05), IgG (p less than 0.05), IgA (p less than 0.001), IgM (p less than 0.05)]. No statistically significant differences were seen in the patients receiving placebo. The results suggest that sulphasalazine is effective for the treatment of patients with ankylosing spondylitis. In these patients, there was also a diminution of the daily dosage of nonsteroidal antiinflammatory drugs. In the majority of patients, clinical and laboratory improvements were expressed more pronouncedly in the peripheral form of ankylosing spondylitis than in the axial form, but statistically no significant differences were found between the two groups.

[Alternative methods of therapy in rheumatic diseases].

Four hundred and forty patients with various rheumatic diseases were tested. We asked their opinion regarding an eventual attempt to treat their disease by alternative methods of treatment, about the success of treatment, their relationship to these methods before and after treatment. Among the investigated persons there were 218 women and 222 men. Thirty per cent (134) tried out one or more methods of alternative treatment. As regards the number, among our patients those with lumbar syndrome hold first place (N = 151), followed by those with rheumatoid arthritis (N = 123). Thirty per cent of patients with lumbar syndrome and 51% of those with rheumatoid arthritis tried out one or more methods of alternative treatment. In both groups of patients, a significantly longer duration of treatment was established (p < 0.005). In the patients with rheumatoid arthritis the most frequently chosen method was acupuncture (62%), in patients with lumbar syndrome it was chiropraxis (76%). From among the total number of patients trying out alternative methods of treatment (N = 134), before treatment 79% had a positive and only 3% negative relationship to it. Eighteen per cent, evaluated this kind of treatment merely as an experiment or were indifferent to it. After the treatment, only 37% of the previous 79% still had a positive opinion, while the share of negative opinions increased from 3% to 32%, 31% of the patients were indifferent. After the treatment, 36% of our patients with rheumatoid arthritis who had tried out alternative methods of treatment still had a positive relationship, while 41% had a negative and 23% an indifferent relationship to this kind of treatment.(ABSTRACT TRUNCATED AT 250 WORDS)
