We report the nucleotide sequence of a 1.45 kb segment containing the eta gene, coding for staphylococcal exfoliative toxin A (ETA), isolated from the recombinant plasmid pETA-J3. The coding region of 840 bp specified a polypeptide of 280 amino acid residues which included a putative 38 residue signal sequence. The amino acid composition deduced from the structural gene was in agreement with the results of peptide analysis of the ETA molecule reported by others. The sequence of the 35 N-terminal amino acid residues of ETA derived from Staphylococcus aureus strain ZM was also consistent with that deduced from the DNA sequencing.
Staphylococcal exfoliative toxin A (ETA) had a splitting effect at the granular layer of skin in humans and neonatal mice, but not in rabbits, guinea pigs, golden hamsters, or rats. Besides its splitting effect, ETA could stimulate productions of neutralizing antibody to ETA in rabbits, rats and B10D2 mice, but not in golden hamsters, guinea pigs, or ICR, HRS/J, and C57BL/10 mice. In our epidemiological investigation of human sera, the percentage of antibody to ETA in sera obtained from patients with impetigo (8%) was lower than those in sera of healthy males (23%) and females (29%). The relationship between susceptibility and immune response to ETA in these mammalians could be divided into three groups: the possession of resistant skin and high production of antibody to ETA; the possession of resistant skin and low production of antibody to ETA; the possession of sensitive skin and various titers of antibody to ETA.

1) Using two histologically different human renal cell carcinomas heterotransplanted into nude mice, the antitumour effect of UFT was investigated, and an attempt was made to analyze the properties of the tumour strains where a drug efficacy was noted. 2) The two strains used were the JRC 1 strain (tumour doubling time of 9.4 days, clear cell type, papillary histologic pattern, grade 3) and the JRC 11 strain (tumour doubling time of 2.72 days, granular cell type, anaplastic histologic pattern, grade 4). 3) Two dose groups were set up, one receiving 10 mg/kg of tegafur (FT) and 22.4 mg/kg of uracil and the other receiving 20 mg/kg of FT and 44.8 mg/kg of uracil. Each group was further divided into an early administration group (start of administration 3 days after the tumour transplantation) and a late administration group (start of administration at a time when the transplanted tumour proliferated to weight of 100 to 300 mg). 4) Effect as noted in the tumour proliferation inhibition rate was seen only in the group receiving 20 mg/kg of FT and 44.8 mg/kg of uracil in both early and late administration groups of the JRC 1 strain. Among these groups only the early administration groups showed a histological positive effect. 5) The fact that the measured 5-FU intra-tumour concentration in the JRC 1 strain was only 1/4 that of the JRC 11 strain demonstrates more susceptibility of JRC 1 strain to UFT. Moreover, intratumoral concentration of 5-FU differed markedly even with the same administration method and dosage level. This result indicates that intra-tumour concentration will be different if the histological pattern differs.
Eleven cases of hydrocele testis were treated with injection of a 1% or 2% dedoxycycline hydrochloride or minocycline hydrochloride, and a cure was obtained in 9 cases (81.8%). Of these 9 cases, 7 cases were cured after one injection, and 2 cases after 2 injection. Both of the latter 2 cases in which physiological salt solution was used as the dissolving solution experienced scrotal pain immediately after injection. Among the cases in which 1% lidocaine was used as the dissolving solution only 1 injection (11.1%) was associated with slight pain out of 9 injections. This treating method is highly effective and little or no pain is experienced if lidocaine is used. In conclusion, we recommend this method as a treatment for hydrocele testis.
Conservative therapy, preserving bladder function, is most commonly done for the treatment of bladder tumors. However, prevention of intravesical ectopic recurrence remains to be the major postoperative problem. Between April, 1969 and March, 1986. 144 patients were treated conservatively at our Hospital. Initial recurrence occurred in 36.8% of the conservatively treated cases. Second recurrence occurred in 73.2% twice the rate of initial recurrence. No differences were noted between the recurrence rate of single and multiple tumors. Overall recurrence rate according to histological grade was 0% for Grade 0, 36.4% for Grade 1, 36.8% for Grade 2, and 50.0% for Grade 3. The overall recurrence rate of the group without postoperative prophylactic adjuvant therapy was 37.0%. The overall recurrence rate according to postoperative prophylactic adjuvant therapy was 28.6% for intravesical Mitomycin C (MMC) instillation, 30.0% for intravesical Adriamycin (ADM) instillation, 36.8% for external irradiation, 42.9% for fluorouracil (5FU) administration and 100% for 5FU + intravesical instillation. As a whole 5FU is not effective. However, in 1 patient who had had seven previous recurrences, no recurrence has occurred for over seven years after 5FU administration. Effect of adjuvant therapy differs in each case and cannot be regarded in the same light. High grade, high stage, and large (greater than 3 cm) tumors show a high recurrence rate. Progression (grade up, stage up) occurred in 11.3% of the recurrence cases.

Auditory brain-stem responses (ABRs) were recorded from a group of 12 mentally retarded males with the fragile X (fra[X]). The responses were analyzed in terms of ABR thresholds, absolute latencies, and interpeak latencies. One patient had increased ABR thresholds, indicating hearing impairment. Five fra(X) subjects had prolonged I-V interpeak latencies. Comparisons between the fra(X) group (excluding one possible hard-of-hearing subject) and a control group of age-matched males with normal intelligence showed that the fra(X) group's interpeak latencies were significantly prolonged for the III-V and I-V but not for the I-III. This pattern of prolongation of interpeak latencies suggests that central, as opposed to peripheral, nervous-system dysfunction predominates in many patients having this syndrome. In addition, frequently observed prolongation of the transmission time may indicate that brain-stem white-matter functioning is also apt to be involved in this syndrome.
A 63-year-old man underwent total gastrectomy and partial resection of the transverse colon for Borrman IV gastric cancer on August 27, 1985. On August 8, 1986, 11 months after the operation, he visited our department with chief complaint of swelling of both groins. The spermatic cord was swollen to about 3 cm in diameter from both groins to inside of the scrotum and was hard as a club. The diagnosis of bilateral spermatic cord metastasis of gastric cancer was established and a biopsy was carried out. Histopathologically the diagnosis of spermatic cord metastasis of gastric cancer was confirmed.

Further recurrence of superficial bladder cancer after transurethral resection is frequent in patients who have already experienced recurrence. In an attempt to prevent or delay further recurrences in such patients, the effect of preoperative doxorubicin instillation was investigated. A total of 51 patients with recurrent superficial bladder cancer were randomized to receive either TUR alone or TUR with preoperative doxorubicin instillation. Doxorubicin was administered twice a week for 3 weeks before TUR surgery. An objective response (CR + PR) of the tumors was observed at operation in 12 of 25 (48%) evaluable doxorubicin-treated patients. Chemical cystitis was seen in 32% of the patients. Further recurrence after TUR was observed in 13 of 25 (52%) patients in the doxorubicin group, as against 15 of 23 (65%) evaluable patients in the control group. The mean disease-free interval was significantly longer (11.8 as against 7.1 months) in the doxorubicin group. These preliminary results suggest that preoperative doxorubicin instillation might be effective for prolongation of the disease-free interval in patients with recurrent bladder cancer.
We present here the first familial cases (a mother and son) of dominantly inherited retinoblastoma with a 13q14 deletion [46,XY or XX, del(13)(q14.1q21.2)]. Their esterase D activities in red blood cells were as low as 50% of the normal control and the haplotype of esterase D was a type 1-0 in the mother and a type 2-0 in the son. They had peculiar facies characterized by a high forehead, low and broad nasal root, a short and bulbous nose, a long philtrum, and open mouth with a thin upper lip, and prominent earlobes. Chromosome and esterase D analysis should be performed in patients with retinoblastoma even if retinoblastoma seems to be transmitted through an autosomal dominant inheritance. This family indicates that one of the causes of dominantly inherited retinoblastoma is a chromosome deletion of part of the 13q14 band whether it is detectable by chromosome analysis or not.
The fragile X [fra(X)] syndrome was screened on 190 Japanese institutionalized females with moderate to severe mental retardation. Two inmates with severe mental retardation (IQ 20) had the fra(X) chromosome in 26% and 15% of the cells examined, indicating that the prevalence of the fra(X) syndrome was about 1% in all female inmates and was about 3.27% in severely mentally retarded females without known causes. However, no female with fra(X) syndrome was found in 35 moderately retarded females. Both had brothers with the fra(X) syndrome and the prevalence was 10% in females with a family history of mental retardation. In addition, the replication study of the fra(X) chromosome in the patients supported the proposal that an excess of the early replicated fra(X) chromosome is related to the mental capacity in heterozygous females. Therefore, the fra(X) syndrome should not be ignored even in severely mentally retarded females with a family history, though the heterozygotes are commonly normal to subnormal in their mental development. In addition, the replication study of the fra(X) chromosome may help to estimate mental development in the carrier children.
