[Measuring the looseness of the knee joint by using the Kt 1000 arthrometer].

Authors measured the stability in the sagittal plane of the intact knee joints of 50 young men with the MEDmetric KT 1000 apparatus. The greatest movement during the measurements on 100 knees was observed in 25 degrees flexion and during the application of force 89N. In 70 degrees flexion the movement was only half of that observed in 25 degrees flexion. The difference of the average movements measured bilaterally was neither in active nor in passive experiments more than 0.5 mm. The difference between the average values of the compliance indexes on the left and right side was only 0.01 mm. In all measurements the frequency distribution of the differences between the two sides was essentially the same. The difference between the movements on the two sides did not exceed 2 mm-s in 90-100 percent of the cases.

[Arthrosis following knee joint instability and its surgical prevention].

Authors describe the more important injuries that must be corrected by all means during the operative treatment of the acute instability of the knee joint to prevent later arthrosis. They call attention to the fact that in the treatment of the acute instability of the knee joint the prevention of the arthrosis can be realized only with a reconstructive operation, performed in time and with modern technique. The biomechanical causes of the arthrosis, developing during the persistent instability are analysed. The extraarticular operative methods used by them in chronic cases are described.

Complex pattern of alternative splicing generates unusual diversity in the leader sequence of the chicken link protein mRNA.

We report here the isolation of the 5' end and the promoter region of the gene for chicken cartilage link protein, and demonstrate extensive heterogeneity of the leader sequence arising from differential utilization of multiple splice sites within the 5'-most exon. The 500-base pairs (bp) exon 1 consists of solely untranslated sequence and is followed by an intron greater than 33 kilobase pairs (kb). Together, the five exons predict a gene size longer than 100 kb. Multiple transcription initiation sites were mapped 34, 46, 56, 66 and 76 bp downstream of a TATA-like motif. Sequence analysis revealed that in addition to the non-spliced variant, multiple mRNA species were generated by alternative splicing resulting in the exclusion of 92, 166, 170, 174 and 263 nucleotides (nt), respectively, from exon 1. Polymerase chain reaction confirmed the existence of various splice forms, and showed cell type- and developmental stage-specific expression for one group of them. Secondary structure predictions indicated that the leaders of the splice forms could form stable hairpin structures with different free energies of formation (up to delta G = -110 kcal/mol), suggesting translational control. The splice variant detected in the largest amount had the least stable predicted hairpin (delta G = -31.7 kcal/mol).

The effect of high and low ultraviolet-B dose exposure on the degree of hairless mouse skin wrinkling.

Chronic exposure of hairless mice to ultraviolet light (UVB 290-320 nm) causes degradative changes in the dermal matrix and wrinkle production. We compared the effects of two different UVB dosing regimens on wrinkle production and dermal damage in female Skh:HR-1 hairless mice using a bank of unfiltered FS-40 lamps. One group of mice, the low dose group, was exposed to a sub-erythemal UVB dose of 12 mJ/cm2 (1 MED = 14 mJ/cm2), 3 times per week for 20 weeks (total dose = 0.72 J/cm2). A second group, the high dose group, was exposed also 3 times per week for 15 weeks to a UVB dose which started with the sub-erythemal dose of 12 mJ/cm2 at Week 1, and 1 MED at week 2. The dose was then increased weekly by 1 MED until reaching 4 MED at week 5. The animals were then dosed at 4 MED for 10 additional weeks (total dose = 2.1 J/cm2). Visual results indicate that, as expected, within the same group, the degree of wrinkling was generally dependent on the total UVB dose administered. However, comparison between the low dose and high dose groups shows that equal cumulative UVB doses did not always result in identical wrinkle grades. For example, at a cumulative dose of 0.5 J/cm2, the mean wrinkle grade for the low dose group was 1.75 compared to that of 1.2 for the high dose group (age-matched = 0). This observation may suggest that there are other factors in addition to total cumulative dose which are important for the appearance of wrinkling in this model.

Hierarchy of glycemic thresholds for counterregulatory hormone secretion, symptoms, and cerebral dysfunction.

To define glycemic thresholds for activation of counterregulatory hormone secretion, initiation of symptoms (autonomic and neuroglycopenic), and onset of deterioration of cognitive function, we measured indexes of these responses during glycemic plateaus of 90, 78, 66, 54, and 42 mg/dl in 10 normal volunteers, with the use of the hyperinsulinemic glucose clamp technique. Activation of glucagon, epinephrine, norepinephrine, and growth hormone secretion began at arterialized venous plasma glucose concentrations of 68 +/- 1, 68 +/- 1, 65 +/- 1, and 67 +/- 2 (SE) mg/dl, respectively. Autonomic symptoms (anxiety, palpitations, sweating, irritability, and tremor) began at 58 +/- 2 mg/dl, which was significantly (P = 0.0001) lower. Neuroglycopenic symptoms (hunger, dizziness, tingling, blurred vision, difficulty thinking, and faintness) and deterioration in cognitive function tests began at 51 +/- 3 and 49 +/- 2 mg/dl, respectively, values that were both significantly (P = 0.018 and 0.004, respectively) lower than that for initiation of autonomic symptoms. We therefore conclude that there is a distinct hierarchy of responses to decrements in plasma glucose, such that the threshold for activation of counterregulatory hormone secretion occurs at higher plasma glucose levels than that for initiation of autonomic warning symptoms, which in turn occurs at higher plasma glucose levels than that for onset of neuroglycopenic symptoms and deterioration in cerebral function. Such a hierarchy would maximize the opportunity to avoid incapacitating hypoglycemia.

Results of surgical treatment of knee instabilities.

Between January 1, 1981 and January 5, 1987, 146 reconstructive operations for acute (39) and chronic (107) knee instabilities were performed. THe majority of the acute cases were anteromedial (24), and of the chronic instabilities, 33 were anteromedial, 14 anterolateral, and 53 combined anteromedial-anterolateral. In terms of suture, replacement, and reinforcement of the damaged structures of the acute cases, 32 were good, 6 fair, and 1 poor, while the reconstruction, according to Müller, for chronic instabilities produced good results in 34, fair in 59, and poor in 14. It is essential that such operations be performed by a trained surgical team. For success, precise knowledge of anatomy, exact examination, accurate diagnosis, adequate surgical techniques, and careful postoperative treatment are essential.

BMY 30047: a novel topically active retinoid with low local and systemic toxicity.

In the treatment of various dermatological disorders, topically applied retinoids have potential therapeutic use with the advantage of improved localized activity and lower toxicity over systemically administered retinoids. However, most retinoids cause a significant degree of local irritation. In the present study, the ability to produce local activity with low local irritation potential was evaluated with a novel retinoic acid derivative. BMY 30047 (11-cis, 13-cis-12-hydroxymethylretinoic acid delta-lactone) is one of a series of retinoic acid derivatives in which the carboxyl function of the polar end was modified with the aim of achieving reduced local irritation and systemic toxicity while retaining the local therapeutic effect. BMY 30047 was evaluated and compared with all-trans retinoic acid for topical retinoid activity in several preclinical assay systems, including the utricle reduction assay in rhino mice, 12-o-tetradecanoylphorbol 13-acetate ester-stimulated ornithine decarboxylase induction in hairless mice and the UV light-induced photodamaged skin model in hairless mice. BMY 30047 was assessed for retinoid-type side effects by evaluating the skin irritation potential in rabbits after repeated topical application, and hypervitaminosis A-inducing potential in mice after i.p. injection. BMY 30047 demonstrated significant topical retinoid activity in several in vivo models with less skin irritation potential relative to the most used clinical concentrations of all-trans retinoic acid. BMY 30047 also showed very little systemic activity and did not produce any evidence of hypervitaminosis A syndrome at systemic doses 20 times greater than the no-effect dose of all-trans retinoic acid.

[Blood pressure lowering effect of nifedipine in smokers and non-smokers].

The authors summarized the relationships between the cardiovascular diseases and smoking. In their retrospective study they studied the acute antihypertensive effect of nifedipine in patients with essential hypertension who were smokers and nonsmokers. They suggest that the antihypertensive effect of nifedipine did not decrease in smokers. Therefore the nifedipine can be the first choice in the antihypertensive therapy in smokers with essential hypertension.

Uptake and distribution of Cd in the ovaries, adrenals, and pituitary in pseudopregnant rats: effect of Cd on progesterone serum levels.

Pseudopregnant (PSP) rats were treated with 3.5 or 7.0 mg/kg body wt of CdCl2 on Day 1 of PSP sc. In the lower dose Cd content of the ovaries (luteal and nonluteal tissues), adrenals, pituitary, and blood on Days 1, 2, 5, 8, 10, and 12, and in the higher dose that of luteal and nonluteal tissue on Days 2 and 5 of PSP were determined with atomic absorption spectrophotometry. A rapid incorporation into the corpora lutea was measured on Day 1 and Day 2 of PSP followed by a decrease of Cd content toward the end of PSP whereas the nonluteal tissue, adrenals, and pituitary accumulated Cd gradually until the fifth to 10th day, respectively. Progesterone (P) serum levels were measured with RIA in the blood collected daily from the jugular vein following administration of 3.5 to 7.0 mg/kg body wt of CdCl2 sc on Day 1 or Day 8 of PSP. The serum levels of P remained unchanged when CdCl2 was administered on Day 1 of PSP; however, 7.0 mg/kg body wt CdCl2 given on Day 8 of PSP induced a significant decrease in serum levels of P. It is supposed that the regressing luteal tissue is more sensitive to the toxic effects of Cd than the developing one.

Identification of positive and negative regulatory regions controlling expression of the cartilage matrix protein gene.

A complex pattern of regulation of the cartilage matrix protein gene was revealed by transient expression experiments. A minimal promoter from positions -15 to +64 functioned in chondrocytes and fibroblasts. An enhancer located in the first intron exerted chondrocyte-specific stimulation on the minimal promoter activity. The same fragment, however, had a negative effect in fibroblasts. Between -334 and -15, a silencer was found which inhibited the gene expression driven from its homologous as well as heterologous promoters both in chondrocytes and fibroblasts. Additional positive and negative control regions were mapped further upstream of the promoter.

[Suture of the meniscus in acute instability of the knee joint].

The authors describe a multiform function of the menisci and the causes of frequently occurring simultaneous injuries of the meniscus and of the ligament. In 26 cases of acute instability of the knee the authors put sutures over the meniscus. In all the cases the results were good, there were no complications and there was no need to make a repeated operation in any of the cases. Making use of the suture over the meniscus, it is possible to improve the results of the surgical treatment of instability of the knee to a greater extent.

[Experiences with the use of ioxithalamate (Telebrix)].

The effects of Ioxitalamate/Telebrix-BYK GULDEN were tested for excretion urography and various angiographic applications and in animal experiments (selective renal angiography in 36 dogs). The contrast medium was well tolerated and the radiographs were of good quality. Significant effects were not detected, neither in blood pressure, heart rate and ECG, nor in Se-creatinine, nor in UN-, SGOT and Se-bilirubin values. In 4 cases transient proteinuria was observed. In the kidneys of the dogs no mentionable effects, histological or ultrastructural, were found. The use of Ioxitalamate seems advisable for investigations that require contrast media with low osmotic pressure.

[Significance of low osmolarity contrast media in state of the art roentgen diagnosis].

The authors used low-osmolality contrast material intravascularly in 546 patients, in 371 cases ioxaglate, in 80 patients iopamidol and in 95 iohexol. On each occasion good detail-rich pictures were obtained. Sensation of heat was indicated only by 1 patient (iopamidol), pain by 7 patients (ioxaglate 4, iohexol 2, iopamidol 1). The blood pressure, pulse rate and the UN, SGOT and SeBiru values showed no significant changes. The experiments were carried out on 101 dog kidneys. Selective renal angiography was done with the aforementioned materials and compared with the conventional, high-osmolality materials (iodamide, metrizoate). The histological changes were only slight, of small extent, and reversible when the new materials were used. Use of the new, low-osmolality contrast materials is justified whenever they must be given intravascularly, but it is imperative in the examination of children, patients of high risk groups and sensitive organs (nervous system, kidney, coronary vessels).

Structure of the gene for cartilage matrix protein, a modular protein of the extracellular matrix. Exon/intron organization, unusual splice sites, and relation to alpha chains of beta 2 integrins, von Willebrand factor, complement factors B and C2, and epidermal growth factor.

The entire gene for chicken cartilage matrix protein (CMP) has been isolated and characterized by restriction mapping, electron microscopy, nuclease S1 mapping, and sequence analysis. The gene, which is present in a single copy in the chicken genome, is 18 kilobase pairs long and comprises eight exons and seven introns. It has two transcription initiation sites, 8 base pairs from each other. A sequence very homologous to the consensus nuclear factor III binding-site sequence, a CAT- and a TATA-like sequence are found in the promoter region and ATTAAA is used as a polyadenylation signal. The nucleotide sequence defines a primary translation product of 493 amino acids which consists of a 23-amino acid signal peptide and two large repeated domains connected by an epidermal growth factor module. Amino acid sequences homologous to those of the repeated domains are present in the type A repeats of von Willebrand factor, complement factors B and C2, and in the alpha chains of the integrins Mac-1, p150,95, and LFA-1. The exon-intron structure indicates that the CMP gene may have arisen by exon duplication and exon shuffling during evolution. The GT-AG splice rule cannot be applied for the excision of the last intron of the CMP pre-mRNA. The donor splice site of intron G is basically different from the consensus sequence indicating that a novel type of splicing mechanism might exist in cartilage.

[Diagnostic and therapeutic significance of automatic circadian monitoring in hypertension].

A noninvasive, fully automatic blood pressure monitoring system has been developed to make the measurement of blood pressure more reliable and to help individualizing anti-hypertensive therapy. The authors draw attention to the importance of the circadian blood pressure variability and the "white coat hypertension" phenomenon. The error due to overlooking the changes in circadian blood pressure and to the "white coat" effect could be avoided with automatic blood pressure monitoring. Their system consists of two subunits: the Nippon Colin (BP 203 MII) blood pressure monitor, which has been fitted to the Commodore 64 computer by their own interface and data transfer program. Data analyzing and graphic displaying programs have also been developed by them. The diagnostic and therapeutic importance of blood pressure monitoring are demonstrated with representative cases of patients with essential, renoparenchymal and renovascular hypertension. The perspectives of this method in clinical pharmacology are also illustrated and discussed.

[Congenital type I antithrombin III deficiency with serious complications in a 7-year-old girl].

This case report concerns a child admitted to the County Hospital of Zalaegerszeg with the symptoms of ataxia, focal convulsions and hemiparesis. Anticonvulsive therapy abolished the epileptic manifestations, but hemiparesis remained unchanged. At the age of six and half years progressive venous thrombosis developed first on the left and some days later on the right lower limb. Phlebography revealed on both sides thrombosis of the vena iliaca which led to stenosis of the right femoral vein and dilated venous collaterals on the abdomen and right thigh. Coeliacography showed an enlarged spleen and varicosity around the portal vein. Later thrombosis of the arteria dorsalis pedis developed indicated by the gangrene the fifth toe. At this stage the child was transfered to the Pediatric Department of the University of Pécs for further evaluation. Examination of the hemostasis showed hypercoagulability due to antithrombin III deficiency pointing towards a common cause, namely thromboembolism of the earlier and recent clinical manifestations. A reduced activity of the antithrombin III was also observed in the mother and two sisters of the child. The response to Syncumar therapy was beneficial, arterial thrombosis regressed and no further thromboembolic complications developed.

Nifedipine-prazosin interaction in patients with essential hypertension.

In an acute, double-blind, crossover study on 12 patients with essential hypertension, the interaction between nifedipine and prazosin was investigated in the supine position. The effectiveness of the combination of the two active drugs was greater than the combination of either of the drugs plus placebo. The effect of nifedipine was not modified by prazosin pretreatment; however, the effect of prazosin was partly inhibited, i.e., delayed, by nifedipine pretreatment. Therefore, giving prazosin as a first drug and nifedipine as the second drug seems to be a better approach than the reverse.
