When blepharoplasty is performed by transconjunctival route, sometimes the excess of skin created by overlaxness of the lower lid does not retract to achieve the desired cosmetic result. We propose a method for controlled retraction of the eyelid by coagulating the skin using the CO2 laser. Thanks to this approach, the lower eyelid's tone recovers and the desired cosmetic appearance is achieved. Results of 37 patients that were treated by 10 W CO2 laser, 5-mm defocused beam diameter, and pulses of 100 ms, are presented. Follow-up shows that periocular skin reshaping by CO2 laser for laxened eyelid is a safe method and it can be considered a good alternative of treatment for blepharochalasis without skin excision.

Four patients who developed combined tendon and overlying skin defects following operative repair of ruptured Achilles tendon were presented. Three patients had an infected wound. The average interval from the first operation for repairing the ruptured Achilles tendon and the reconstructive procedure was 46.2 days (range, 5-65 days). All patients were treated with a one-stage operation including radical debridement, reconstruction of the Achilles tendon defects using vascularized or nonvascularized tendon grafts, transfer of peroneus brevis for augmentation, and skin coverage with a free flap. The patients recovered uneventfully. The average follow-up period was 39.2 months (range, 18-79 months). In all patients, an evaluation of the clinical outcome, the performance of the calf muscles using a computerized dynamometer, and structural changes of the reconstructed Achilles tendon using magnetic resonance (MR) imaging were made. The clinical outcome was excellent in three patients and good in one. In isokinetic testing (Cybex-Norm), strength was found to be normal in one patient and abnormal in three patients. MR images revealed an intratendinous area of homogenous and normal intensity signal, and a significant increase in thickness and width in all levels of the reconstructed Achilles tendon. The authors conclude that it is possible to obtain satisfactory function in patients with complex wounds in the region of the Achilles tendon.
Saliva coating all oral surfaces has a buffering capacity that neutralizes bacterial and cariogenic acids. The aim of our study was to determine the surface pH in different regions of the oral cavity in healthy volunteers and in patients with diseases affecting the oral mucosa. Oral pH was measured with a flat glass electrode on the anterior third of the ventral surface of the tongue, middle hard palate, buccal mucosa and inner lips in 32 healthy volunteers, 12 patients with Behçet's disease, 23 patients with oral lichen planus, and 11 patients with burning mouth syndrome. The present study showed that there was an uneven distribution of oral surface pH. The palate had a higher pH than most other sites in all groups, and in patients with lichen planus, the palate pH was higher than that in healthy controls. Those with dentures had lower pH values in the hard palate than dentate patients. The relatively high pH in the palate region in all patient groups as well as healthy volunteers needs to be further studied to clarify its mechanisms and clinical relevance.
A clinical trial with a "continuous" CO2 laser in the treatment of cutaneous lesions was instituted 2 1/2 years ago. The laser was used as a "light" knife for excision and for gross destruction of some lesions directly. Details of technique and a preliminary report assessing clinical results and general effects are given. Some results thus far have proved satisfactory, and we feel that further investigation of the laser as a surgical modality is warranted.
OBJECTIVES: Saliva has been suggested to have a protective role against carcinogens in the oral cavity in animals. Water-soluble 4NQO is a suitable carcinogen to use in examining the role of saliva in oral cancer. The purpose of this study was to follow the changes induced by the carcinogen at the molecular level, as well as the effect of lack of saliva on these changes. MATERIALS AND METHODS: Topical application to the palate of a 0.5% 4NQO solution dissolved in glycol was used for 4 months to induce malignant transformation in a desalivated rat model. Histomorphometric analysis of proliferating cell nuclear antigen (PCNA), a cell cycle regulator and a proliferation marker, was performed. RESULTS: Manifestation of PCNA significantly increased as the observed histologic changes progressed from hyperkeratosis, to mild or moderate dysplasia, severe dysplasia and squamous cell carcinoma. Differences in manifestation of PCNA among the diagnostic groups was significant (p< 0.05). In the desalivated group, PCNA expression was significantly higher than in control and normal groups, in both tongue and palate after 2 and 4 months (p< 0.05). CONCLUSIONS: An unknown component of saliva has a temporary anti-carcinogenic protective effect, which can both delay and decrease the level of proliferation induced by the carcinogen 4NQO. The specific salivary component and the mechanism by which this protective effect is rendered are yet to be discovered.
A total of 298 patients in geriatric institutions were surveyed; 62.8% were edentulous. The frequency of denture hyperplasia and of flabby ridge was related to denture quality which deteriorates with irregular dental visits. Denture stomatitis and inflammatory papillary hyperplasia of the palate were related to denture age only and not to quality. All denture-related lesion frequencies increased with patient age and denture age. The total frequency of soft tissue lesions was 16.7% in edentulous non-denture patients and 58.2% in patients with poor quality dentures. The lowest frequency of soft tissue lesions was in edentulous, non-denture patients. Poor oral hygiene increased the frequency of positive cultures for Candida from the dentures.
The three cases of children treated for excision of oral hemangiomas presented here illustrate the advantages of using the CO2 laser rather than a scalpel in surgery for vascular lesions of the oral cavity. This laser has the ability to coagulate, vaporize, or cut; its main advantage, however, is its ability to seal blood vessels during surgery. Satisfactory healing in infants and children makes the CO2 laser the preferred method for performing vascular surgery.
