Predictive value of serial carcinoembryonic antigen levels in long-term follow-up of ovarian cancer.

The predictive value of serial levels of carcinoembryonic antigen (CEA) in tumor monitoring was examined in 213 patients with ovarian cancer; each patient had been followed-up at monthly intervals for at least 12 months. CEA was not detectable throughout the period of observation in 35% of the patients. In general. patterns showing a disappearance of CEA or persistently low levels were associated with a good prognosis, whereas those showing a reappearance or highly elevated and rising levels were associated with a poor prognosis. A transient reappearance of CEA was observed in 10 patients; this did not appear to be associated with tumor recurrence or progression. "False positive" results were obtained in 6 patients in whom no tumor has been clinically detectable to date. "False negative" results were obtained in 4 patients with obvious tumor progression. In terms of a good or poor prognosis, the use of CEA levels was highly accurate in patients with minimal or no residual disease (97% and 89%, respectively); the rate fell to 62% in patients with extensive disease. As the clinical significance and limitations become better known, serial CEA levels should contribute substantially to the monitoring of patients with ovarian cancer.

The value of phenformin and ethyloestrenol in the prevention of deep venous thrombosis in patients undergoing surgery.

The effect of phenformin and ethyloestrenol on the incidence of post-operative deep venous thrombosis was studied in 314 surgical patients in a double-blind randomised trial. Although the laboratory tests suggested that the regime produced an increase in activators of the fibrinolytic system, the drugs used did not lower the incidence of post-operative deep venous thrombosis. Possible explanations of this paradox are advanced.

Estrogen excretion patterns in induced ovulation.

Observation of response to gonadotropins in the treatment of anovulation has allowed us to define the estrogen excretion pattern which leads to a successful single pregnancy. The typical pattern shows a low pretreatment urinary total estrogen excretion; treatment with gonadotropins, of human pituitary origin, is continued for about 14 days. There is a predictable rate of rise of preovulatory estrogen excretion (30 microgram/24 hr2). Human choronic gonadotropin--about 4000 IU--should be given when an estrogen excretion of 75-100 microgram/24 hr has been obtained. The use of this pattern of ovarian response is put forward as a useful guide in the planning of gonadotropin therapy.

Studies on the in vitro incubation procedure for [35S]sulphate labelling of articular cartilage glycosaminoglycans.

Articular cartilage from cow and calf femoral condyles was incubated in Tyrodes solution containing [35S]sulphate for different periods up to 80 min. Glycosaminoglycans from the cartilage tissue and incubation medium were fractionated on Cetylpyridinium chloride and ECTEOLA cellulose microcolumns. The incorporation of [35S]sulphate into all individual fractions of chondroitin sulphate and keratan sulphate was found to be linear from 20 to 80 min incubation time. As a rule the total specific activities of keratan sulphate and chondroitin sulphate were similar for both calves and cows. The proteoglycan material recovered from the medium amounted to about 1% of the tissue dry weight and was found to have a higher chondroitin sulphate: keratan sulphate ratio than the corresponding cartilage tissue for both calf and cow. The solubility profiles for the newly synthesised glycosaminoglycans, obtained from determination of the radioactivity in the individual fractions, were compared with those of glycosaminoglycans already present. These curves indicated that newly synthesised chondroitin sulphate had a higher average molecular size than that present in the tissue whereas the newly synthesised keratan sulphate had a smaller average molecular size. These newly synthesised components were also detected in the proteoglycans recovered from the incubation medium.

Grass pollen content of the air in the Bristol Channel region in 1976.

The grass pollen content of the air at four sites bordering the Bristol Channel (at Bristol, Cardiff, Penarth and Penmaen) was studied. The trap at Penmaen was situated at an official climatological station which provided the meteorological data used in the surveys. Although at certain times large differences could be seen between the concentrations of grass pollen at the different sites, overall the findings for the four sites were similar. Changes in the atmospheric pollen concentration related to variations in meteorological conditions. At the peak of the season high concentrations of grass pollen persisted throughout the night.

Stress markers in relation to job strain in human service organizations.

BACKGROUND: Workers in human service organizations are often confronted with conflicting demands in providing care or education. The aim of this cross-sectional study was to relate levels of endocrine stress markers to perceived job strain in two human service organizations. METHODS: Employees in two local units of the social insurance organization and two local units of the individual and family care sections of the social welfare in Sweden were selected and 103 employees participated (56% participation rate). The perceived job strain was assessed with a standardized questionnaire containing questions of the demand-control model. Questions specially designed to measure emotional demands were also included. The stress markers cortisol, prolactin, thyroid-stimulating hormone, testosterone and IgA and IgG were analysed in blood samples. RESULTS: The main finding was an association between high emotional strain and increased levels of prolactin. The levels of cortisol, but none of the other four stress markers, increased slightly with emotional strain. CONCLUSIONS: Emotional strain experienced in human service work may cause psychological stress. The increase in prolactin was modest but consistent with findings in other published studies on stress-related endocrine alterations.

Pressure area care in infants and children: Nimbus Paediatric System.

Survival rates in both critically and chronically ill infants and children have improved dramatically in recent years and new challenges exist in the nursing care given to these patients. Among these is the increased risk of pressure ulcer development. Children in intensive care environments are especially at risk. Prevention and management of pressure ulceration in the paediatric population requires clinical judgement and skill. The use of pressure ulcer risk assessment tools can assist in this process; however, to date, there is a lack of research evidence and further studies are needed. The pressure relief requirements of the paediatric patient are significantly different to those of the adult patient. In children under the age of 36 months, the ears and occiput are the areas most at risk of pressure injury as a result of the fact that this area is proportionately the largest and heaviest bony prominence. Despite the abundance of specialist pressure redistributing devices for adults, there is little available specifically for the paediatric patient. This article describes a review of the literature on these subject areas and follows with a short report of the evaluation of the new Paediatric Nimbus System undertaken at the Royal Hospital for Sick Children in Edinburgh.

The erythrocyte uptake of 75Se as an indicator of selenium status in lambs.

The glutathione peroxidase activity (GSH-Px) and the in vitro erythrocyte uptake of 75Se (ESeU) was followed in blood samples from 31 lambs for 105 days. The lambs were divided into three groups; untreated (group U), vitamin E treated (group E) and selenium treated (group Se). The animals in group E were administered intramuscularly 250 mg vitamin E at two weeks interval and those in group Se 0.1 mg Se/kg as sodium selenite monthly. None of the lambs exhibited symptoms of muscular degeneration or any other disease during the trial. Vitamin E had no effect neither on the GSH-Px nor on the ESeU. Thirty days and later after the first Se treatment the GSH-Px was significantly higher in group Se than in the other two groups, but the most rapid increase of GSH-Px in this group was noticed after 44 days. On day 51 a significant decrease of GSH-Px was shown in groups U and E compared with day O. The lambs in groups U and E had significantly higher ESeU on day 36 and later compared with day 0. The administration of selenium in the mineral supplement (10 mg Se/kg) to the lambs in group U and E during the 26 latest days of the study gave an almost significant increase of GSH-Px and a decrease of ESeU. Our experience of the ESeU-test is that it is a simple method, and can be used to evaluate Se status of animals.
