An extracardiac, transepicardial approach to the atrioventricular junction, allowing selective cryoablation of the bundle of His, was performed in 10 mongrel dogs. This technique appears to minimize dissection complications, the risk of endocardial damage, and injury to the tricuspid valve, which are reported with endocardial cryoablation. All dogs survived this experiment, and no morbidity was reported. Cryodestruction of the atrioventricular junction is histologically selective and permanent, as was shown at the 3 month follow-up pathological examination.
A 7 month old infant underwent successful surgical removal of a huge interventricular septal fibroma. We believe this to be the youngest child to have undergone successful removal of such a lesion.

A cardiac stab wound is an uncommon and potentially lethal injury. With appropriate treatment, better than 90% of those admitted alive will survive. This paper outlines the presentation and management of patients with cardiac stab wounds, based on the experience of the Glasgow Royal Infirmary and a review of the literature, illustrated by relevant case histories.
The uniaxial stress/strain response of bovine pericardium up to a stress of 1 N/mm2 was studied. The tissue was chemically modified in 0.5% buffered glutaraldehyde for 24 hours under various conditions of constraint. Four groups were studied: an unrestricted control group, a group held at their original length, a group stretched to a stress of 0.04 N/mm2, and a group stretched to a stress of 0.4 N/mm2 during treatment. Histological examination revealed changes in structural organization of the collagen, which explained the alterations in the stress/strain response. A significant alteration in mechanical properties after glutaraldehyde treatment was observed, with considerable loss of extensibility for the prestretched tissues. A valve made from such a prestretched tissue may be stenotic and possibly subject to early failure.
Plasma levels of acebutolol and its major human metabolite, diacetolol, were determined before, during and after aortocoronary bypass grafting in 10 patients who had received a chronic oral regimen of acebutolol 200 mg t.d.s. for at least 6 days before surgery, a 200 mg dose with the premedication and 5-10 mg intravenously immediately before intubation. It was found that this regimen produced beta-adrenoceptor antagonist levels which were within the range in which attenuation of hypertension and tachydysrhythmia occurs. These effective plasma levels were sustained throughout surgery and persisted into the early recovery period.

From January 1984 to August 1985, 51 out of 2138 patients undergoing cardiac surgery (2.38 per cent) had an intra-aortic balloon pump (IABP) inserted or an attempted insertion. The patients ranged in age from 24 to 68 years (mean 54.2 years). An IABP was inserted preoperatively in four patients, all of whom were in cardiogenic shock; peroperatively in 27 patients who could not be weaned from cardiopulmonary bypass; and postoperatively in 16 patients who had haemodynamic deterioration. Failed insertion occurred in a further four patients. Twenty-eight patients (59.6 per cent) had percutaneous insertion of the balloon, in 17 (36.2 per cent) the common femoral artery was exposed prior to insertion and in two (4.2 per cent) the balloon was inserted into the thoracic aorta. The overall survival rate was 38.3 per cent for patients with successful insertion. Major complications that threatened life or limb occurred in 10.25 per cent of patients. These complications were confined to females and in these patients the percutaneous technique for inserting intra-aortic balloons should be used with caution.
The Wessex porcine bioprosthesis exhibits satisfactory hydrodynamic performance and durability comparable to other clinical porcine valves. In vivo performance and pathological studies of explanted valves confirm adequate biodurability and haemodynamic performance. Early clinical results are satisfactory and encourage further study.
