Panniculitis associated with cutaneous T-cell lymphoma and cytophagocytic histiocytosis.

A 36-year-old woman had a 6-year history of recurrent panniculitis with development of an angiocentric and angiodestructive cutaneous T-cell lymphoma (CTCL) of the helper cell phenotype. She subsequently developed a rapidly progressive fatal syndrome characterized by cytophagocytic histiocytosis and hyperlipidaemia. Cytophagocytic histiocytosis has previously been reported in association with panniculitis, malignancy and infection, but not with CTCL and the precise relationship between panniculitis, CTCL, cytophagocytic histiocytosis and hyperlipidemia is unclear.

Adult

Splenic embolization.

While transcatheter embolization of the spleen has shown promise in the treatment of a wide spectrum of disorders, the incidence of serious complications with this technique has limited its use as an alternative to operative splenectomy. In our institution 41 patients were treated with a modified technique involving partial splenic embolization, careful antibiotic prophylaxis, and adequate pain control. There was no mortality and only few instances of clinically significant complications: a splenic abscess in one patient, pancreatitis in one patient, severe pleural effusions in two patients, and pneumonia in five patients.

Adolescent

Epithelioid sarcoma of penis.

A rare case of an epithelioid sarcoma of the penis arising as a superficial plaque in the glans and causing pain on erection is reported. This tumor is characterized by a slow growth and infiltrative pattern with a high propensity for local recurrence. Proper treatment is wide local excision.

Adult

Signet-ring cell lymphoma.

In a small cleaved, follicular center cell (poorly differentiated) lymphoma the majority of the cells appeared vacuolated by light microscopy. This is an example of the recently described signet-ring cell lymphoma. Immunoperoxidase staining demonstrated that the cells contained IgG/k. Ultrastructural observations indicated that small vacuoles coalesce to form the large vacuoles, which contain microvesicles approximately 50 nm in diameter.
