A study of complications in preterm deliveries after prolonged premature rupture of the membranes.

The risk of infection for mother and baby after spontaneous rupture of the membranes was evaluated in a prospective study of 24 patients with ruptured membranes before the 36th week of pregnancy. The mean length of pregnancy after rupture was 10 days and 2 hours. Only patients harboring Group B streptococci or E. coli in the urogenital tract were treated with antibiotics (during delivery). With the exception of 1 woman, all patients harbored one or more pathogens in the urogenital tract. Four mothers were infected but all recovered. One of 26 infants died from infection. The study did not confirm any association between prolonged rupture of the membranes and the frequency of idiopathic respiratory distress syndrome, nor did it contradict attempts to actively prolong pregnancy after rupture of the membranes.

The relaxing effect of terbutaline on the human uterus during term labor.

The uterine and cardiovascular responses to the adrenergic beta2-receptor stimulator terbutaline (TRB) were investigated in 14 patients in normal term labor. TRB, administered intravenously at a rate of 10 to 20 mug per minute, effectively inhibited uterine activity in advanced labor and also expulsion. Intravenous injection of TRB, 250 mug, diminished oxytocin-induced uterine hyperactivity. No serious side effects of the drug were observed; the circulatory effects were minimal, except for a tolerable maternal tachycardia. The mode of action and clinical application of TRB for inhibition of unwanted uterine activity are discussed.

Terbutaline inhibition of midtrimester uterine activity induced by prostaglandin F2alpha and hypertonic saline.

The effect of the beta receptor stimulator, terbutaline, on uterine activity induced by intra-amniotic injection of prostaglandin F2alpha or hypertonic saline was investigated in 20 patients undergoing therapeutic abortion in the second trimester. Intrauterine pressure was recorded, and the basal tone, intensity and frequency of contractions and "total" activity (expressed in Montevideo Units or MU), were measured. Uterine activity was higher in the prostaglandin group (mean value 393 MU) than in the hypertonic saline group (mean value 238 MU). However, terbutaline, infused at a rate of 5 to 20 mug/minute, decreased the activity by approximately the same absolute amount in the two groups to a mean value of 214 MU in the prostaglandin group and to 68 MU in the saline group. In some patients of the saline group, terbutaline completely inhibited uterine contractions. Basal tone, which was high in the prostaglandin group (mean value 28 mm Hg) decreased to a mean value of 17 mm Hg during terbutaline infusion. In the saline group the mean value for basal tone decreased from 10-5 to 7-5 mm Hg. It is concluded that uterine activity, induced by intra-amniotic injection of prostaglandin F2alpha or hypertonic saline, can be inhibited by terbutaline. The mechanisms of action for prostaglandins and hypertonic saline are discussed.

Effects of terbutaline on human uterine motility at term.

The effects of the selective beta-2-receptor stimulator terbutaline on the activity of gravid, human myometrium were investigated in vitro and in vivo, before and after administration of different beta-receptor blockers. Terbutaline, 0.2-1.0 mu-g/ml, inhibited the spontaneous contractile activity of isolated strips of myometrium. This effect was unaffected by the selective beta-a-receptor blockers practolol, 1 mu-g/ml, and H 93/26, 1 mu-g/ml. However, the non-selective blocker propranolol, 0.1 mu-g/ml, completely inhibited the terbutaline effects. The in vitro effects of terbutaline could be correlated with findings in vivo. Intra-uterine pressure was recorded in 4 pregnant women at term. Infusion of terbutaline, 10-15 mu-g/min, for 20-40 min, effectively inhibited both spontaneous and oxytocin-stimulated uterine activity. There was a moderate increase in maternal heart rate, but no consistent effect on maternal blood pressure. Fetal heart rate was little affected. The uterine effects of terbuline were not influenced by practolol, 5-20 mg i.v., but completely inhibited by propranolol, 1-2 mg i.v. The results suggest that terbutaline inhibits uterine motility by effects on uterine beta-2-receptors and that it can be given in clinically effective doses without adverse circulatory effects on mother or fetus.

Conization as treatment of carcinoma in situ of the uterine cervix.

A series of 343 consecutive cases with the preoperative diagnosis of carcinoma in situ of the uterine cervix treated by cervical conization is presented. In 93% of these patients cervical cytology returned to normal. The follow-up period ranged from 1 to 5 years. There was significantly greater chance for an apparent cure in cases in which histologic evaluation indicated free margins of resection (98%) than in those where the margins were involved by the neoplastic process (70%). Colposcopic evaluation to determine the site for punch biopsies and adequate resection margins could greatly enhance the success of therapeutic conization. The urgent need for conservative management of cervical intraepithelial neoplasia because of its predominance among young women is discussed.

Ultrasound measurement of the fetal cerebral ventricles: a prospective, consecutive study.

Real-time ultrasound was used in 654 consecutive pregnancies to obtain standard growth parameters for the fetal brain. Measurement of the width of the lateral ventricle (LVW) and hemisphere and their relationships to menstrual age, biparietal diameter, and birth weight were determined. The growth of the LVW was to a great extent independent of birth weight but dependent on menstrual age. Thirteen fetuses with a single ventricular width measurements exceeding +2 SD from the mean were separately evaluated, and all but one case were found to be normal.
