[Antihomotoxic drug therapy of endothelial function disorder in patients with hypertension].

A multimodality therapy of patients with hypertensive disease involving the institution of basic conventional therapy and antihomotoxic preparations Hepar compositum and Ubichinon composition (Heel, Germany) has been shown to promote restoration of the degree of initial postischemic constriction but without normalization of responsiveness of vessels to the ischemic factor and nitroglycerin. Administration of antihomotoxic drugs exerted apparent optimizing effect on the state of the endothelial function that was manifested by returning parameters characterizing initial postischemic vasoconstriction and maximum postischemic vasodilatation to normal with high responsiveness to the endothelium-independent vasodilator nitroglycerin being maintained.

[Modern views on rheumatic fever].

Submitted in the paper are modern conceptions on etiology, pathophysiology, diagnosis and treatment of acute inflammatory rheumatism (IR). Some aspects of IR course in different age groups are highlighted, high informative value of new laboratory methods of diagnosis of its latent forms are validated. The place of interferons and selective inhibitors of COG-2 in the treatment of IR is determined.

[Left ventricular function in patients with chronic obstructive bronchitis].

A study of 45 patients with chronic obstructive bronchitis by the method of radionuclide ventriculography with 99mTc showed that even the early stages of the disease were characterized by an inhibition of the contractile function of the left cardiac ventricle. With progression of respiratory insufficiency, maintaining of the left cardiac ventricle stroke volume is achieved by increase of the final-diastolic and final-systolic volumes. Appearance of signs of cardiac decompensation is accompanied by a breakdown of this compensatory mechanism accompanied by a reduction of the strike volume and diminished rate of contraction of the circular fibers of the left ventricle myocardium.

[Functional state of the kidneys in patients with chronic diffuse bronchitis].

Radionuclide renography and enzyme immunoassay used to evaluate renal function and renin--angiotensin--aldosterone system in 95 patients with chronic diffuse bronchitis demonstrate impairment of renal secretion and excretion in all the examinees. There are shifts in plasma renin activity and aldosterone concentration. The above defects depend on cardial--pulmonary insufficiency and respiratory--hemodynamic disorders.

[The phosphate and methemoglobin content of the erythrocytes in patients with ischemic heart disease].

Cardiac insufficiency due to chronic ischemic heart disease is accompanied by a reduction of total phosphate at the expense of its organic part with a simultaneous increase of the role of inorganic phosphate during increase of methemoglobin in the erythrocytes. After traditional therapy the methemoglobin level continued to rise while inorganic phosphate and the level of 2,3-DPH decreased. Treatment including myldronate caused an increase of 2,3-DPH in erythrocytes and a decrease of methemoglobin indicating an improvement of the oxygen transport function of the blood.

[The use of mildronat in heart failure in patients with chronic ischemic heart disease].

A new native cardioprotector mildronat was used for the treatment of cardiac insufficiency due to ischemic heart disease. It was established that mildronate produced a positive effect on the hemodynamics and gaseous composition of the blood. The patients also showed normalization of the nitrogen metabolism and activity of the membrane-bound erythrocytic enzymes.

[Prophylactic medical examination of patients with chronic pulmonary heart].

Results are analysed of the efficiency of complex treatment of patients with chronic cor pulmonale including cardiac glycosides, agents stimulating metabolism processes in the myocardium, immunoactive drugs and antiaggregants. The efficacy was evaluated of cardiotonic therapy with different glycosides in hospital and outpatient treatment. The necessity is substantiated of including into complex treatment drugs of metabolic action, immunoactive and antiaggregant agents with the purpose of reducing the threshold of sensitivity of the myocardium to the toxic effect of cardiac glycosides.
