Peutz-Jeghers syndrome associated with adenocarcinoma of the cecum and focal carcinomas in hamartomatous polyps of the colon: a case report.

We report herein a case of Peutz-Jeghers syndrome associated with an adenocarcinoma of the cecum and four focal cancers arising in hamartomatous polyps of the colon. There were a total 27 polyps in the intestine; 4 in the small intestine and the rest in the large intestine. The hamartomatous polyps in which the 4 focal cancers were found showed no component of adenoma, and the other polyps removed from the colon and small intestine at the same time were all hamartomas with no evidence of dysplasia or malignancy. This case was rare in that multiple focal cancers may have arisen directly from the hamartomous polyps of the colon.

Juvenile granulosa cell tumor in association with a high serum inhibin level.

Inhibin is a glycoprotein hormone produced mainly by ovarian granulosa cells. Recently measurement of serum inhibin levels was made possible by radioimmunoassay. We describe a patient with a juvenile granulosa cell tumor, a distinctive and rare form of granulosa cell tumor. This patient's serum inhibin level was extremely high preoperatively and returned to normal after the tumor was resected. This case suggests that serum inhibin level may be a useful marker of granulosa cell tumors.

[New inactivating enzyme of aminoglycoside antibiotics produced by genes on R-plasmid of Serratia spp. and nosocomial infection].

Though incident due to Serratia spp. has decreased with the use of third generation cephems, S. marcescens with multiple drug resistance are still clinically occasionally isolated S. marcescens strains 218, 219 and 220 we isolated clinically, were found to possess aminoglycoside inactivating enzyme activities which had not been expected to exist in Serratia spp. These isolates inactivated aminoglycoside antibiotics except strains 218 and 219 were sensitive to isepamicin whereas strain 220 was sensitive to isepamicin and gentamicin. It was found that strains 218 and 219 possessed aminoglycoside inactivating enzyme AAC(2'), and strain 220 AAC (6'). The genes coding for these inactivating enzymes were found to be on R-plasmid and transferred between bacterial cells. Frequencies of transfer of the R-plasmid from Serratia to Escherichia coli chi-1037 were be low, but the transfer definitely occurred as frequencies of 4.3 x 10(-7) from strain 218, 5.2 x 10(-7) from strain 219, and 8.3 x 10(-7) from 220 were determined. To prevent nosocomial infections, the eradication of multi-resistant Serratia strains is strongly recommended when they are isolated even apparent symptoms of infections are not observed.

Renal involvement in POEMS syndrome.

We describe a patient with POEMS syndrome whose renal biopsy specimen also showed nephropathy. Immunoelectron microscopy of the renal biopsy revealed the localization of immunoglobulin (IgA and lambda light chain) in the subendothelial space of the glomerular capillaries, a previously unrecognized finding. We conclude that the renal pathology in POEMS syndrome is unusual and distinct from microangiopathic glomerular involvement or idiopathic mesangiocapillary glomerulonephritis.

Expression of ion channel genes in Drosophila.

Ion channel transcripts from three genes were localized by the method of tissue in situ hybridization. The genes examined were the Drosophila Na+ channel genes, paralytic (para) and Drosophila Sodium Channel (DSC), and the K+ channel gene, Shaker( Sh). All three of the genes were expressed in cell bodies of the fly central nervous system, including optic lobes, central brain, suboesophageal ganglion, and thoracico-abdominal ganglion. Sh was additionally expressed in the photoreceptor cells of the retina and pupal flight muscle, while para and DSC were not. The temporal expression pattern of Sh in muscle was different from that in the central nervous system: muscle expression was transient and limited to mid-pupal stage while nervous system expression was observed throughout pupation, apparently peaking at the late-pupal stage. Only one class of 5' end was found in pupal muscle, possibly indicating regulation of splicing pathways.

[Laboratory diagnosis of left atrial thrombi in patients with mitral stenosis].

Left atrial (LA) thrombi sometimes occur in patients with mitral stenosis (MS) and the systemic embolization due to thrombi causes a serious, occasionally fatal complication. Several clinical techniques have been used to estimate the presence of LA thrombi. However, the hitherto available methods, even an echocardiography which has been most widely used, still have some drawbacks, depending on the size and location of thrombi. The author measured D-dimer, fibrinopeptide A (FPA) and thrombin-antithrombin III complex (TAT) in the patients with MS and evaluated the diagnostic value of these molecular markers to estimate the presence of LA thrombi. Twenty six patients with MS who had undergone cardiac operation were studied. Atrial fibrillation was found in all the patients. Episode of obvious thromboembolic diseases is a criteria of exclusion. Blood was drawn from the brachial vein several days (3 +/- 1 days: mean +/- SD) before the operation. The presence or absence of thrombus was confirmed at the surgery in all the cases. 1) Both levels of D-dimer and TAT were significantly higher in the patients with thrombi than those in the patients without thrombus or those in normal controls (mean: 378, 93 and 64 ng/ml, respectively; p less than 0.01 for both and 9.1, 2.0 and 1.7 ng/ml, respectively; p less than 0.01 for both). However, levels of FPA were not significantly different among the three groups (mean: 7.9, 4.9 and 3.7 ng/ml, respectively; NS for both). 2) both levels of D-dimer and TAT were significantly correlated with the weights of LA thrombus (r = 0.87, p less than 0.01: r = 0.79, p less than 0.01, respectively). 3) LA thrombi (ca. greater than or equal to 2 g) were always confirmed at the surgery in the patients who had levels of D-dimer higher than 200 ng/ml and/or TAT higher than 4 ng/ml. The plasma levels of D-dimer and TAT were further followed after the surgery in the same 18 patients (8 patients who had thrombus, the rest who didn't). 1) In the patients who had thrombi, levels of D-dimer were significantly decreased after the surgery (mean: from 267 ng/ml to 73 ng/ml, p less than 0.05). Levels of TAT were slightly but not significantly decreased (mean: from 82 ng/ml to 76 ng/ml, NS).(ABSTRACT TRUNCATED AT 400 WORDS)

[A case report of long-term post-thoracotomy pain management with intrapleural bupivacaine].

A 50-year old woman with right post-thoracotomy pain was referred to us for assistance with pain control. She required pentazocine 60-150 mg per day before our treatment. First, we treated her with intercostal nerve block or oral morphine sulfate. But the result was not satisfactory after five months. Then we tried intrapleural bupivacaine. An epidural catheter was inserted into the pleural space from eight intercostal space at the anterior axillary line and 10 ml of 0.5% bupivacaine was instilled. The treatment was effective for about 4-5 hours. We continued this method for 42 days with 10 ml of 0.25% or 0.5% bupivacaine once or twice a day. She felt so good from the intrapleural analgesia and could be discharged. There was no hypotension, respiratory depression, urinary retention except burning thoracic sensation. We think it is possible to use this intrapleural bupivacaine to treat a certain kind of unilateral chronic pain.

A somatostatin-secreting cell line established from a human pancreatic islet cell carcinoma (somatostatinoma): release experiment and immunohistochemical study.

Production and secretion of somatostatin (SRIF) were studied using a carcinoembryonic antigen (CEA)-producing cell line (QGP-1) established from a human pancreatic islet cell carcinoma. High concentrations of SRIF (274 +/- 51 ng/mg of protein, mean +/- SD, n = 5) and CEA (3083 +/- 347 ng/mg of protein, mean +/- SD, n = 5) were present in QGP-1 cells, and the basal secretion rates of SRIF and CEA by the cells (n = 5) were 46.4 +/- 4.8 and 1690 +/- 78 pg/10(5) cells/h, respectively. Immunohistochemical studies revealed the presence of SRIF in xenografts of QGP-1 cells and colocalization of SRIF and CEA. Secretion of SRIF by QGP-1 cells was stimulated in the presence of high K+ (50 mmol) and theophylline (10 mmol), but arginine (10 mmol) and glucose (300 mg/dl) had no effect on the SRIF secretion. The QGP-1 cell line may be useful for studying the regulation mechanism of SRIF secretion.

A rectal carcinoid tumor of less than 1 cm in diameter with lymph node metastasis: a case report and a review of the literature.

We report herein, a patient with a rectal carcinoid tumor of less than 1 cm in diameter with lymph node metastasis, and discuss a surgical policy for these lesions with reference to other such cases reported in the literature. A 40 year old female was admitted with a rectal mass and colonoscopy revealed a subpedunculated lesion, 1 cm in diameter, with a depression in its tip. A diagnosis of carcinoid was made by biopsy and transsacral excision performed. The excised specimen revealed a subpedunculated lesion measuring 7 X 6 X 6 mm with a central depression. The tumor was histologically confined to the submucosa but lymphatic invasion with pararectal lymph node involvement was observed. A radical proctectomy was thus performed. The incidence of metastasis from rectal carcinoids with a diameter of 1 cm or less is very low ranging from 1.5 to 3.4 per cent, and it therefore seems that most lesions of 1 cm or less in diameter can be treated by local excision alone. Thus, although it is recommended that local excision be performed first to determine the extent of spread, lymphatic vessel invasion and lymph node metastasis, radical surgery is indicated if lymphatic invasion or nodal involvement is present, even when muscle invasion is absent.

Establishment and characterization of human pancreatic cancer cell lines in tissue culture and in nude mice.

Three human pancreatic cancer cell lines, designated as KP-1N, KP-2 and KP-3 have been established in both tissue cultures and in nude mice. The KP-1N and KP-3 tumors were obtained from liver metastases of pancreatic tumors and the KP-2 tumors was of primary pancreatic origin. The patients' tumors from which KP-1N and KP-2 were derived showed characteristics of adenocarcinoma, and the KP-3 tumor had adenosquamous carcinoma characteristics. Inoculations of samples from surgical specimens into athymic nude mice resulted in tumor formation, with the tumors histologically closely resembling the original neoplasms. Subcutaneous injections of the established cell lines also induced tumor formation and the tumors histologically resembled the original lesion in the cases of KP-2 and KP-3 tumors, but the KP-1N tumors in the mice were histologically different from the surgical specimen. The KP-1N, KP-2 and KP-3 cell lines have been cultured continuously in a medium supplemented with 10% fetal calf serum for more than 20, 21 and 17 months, respectively. The KP-2 and KP-3 cell lines produced and released carbohydrate antigen 19-9 into the spent medium but the KP-1N cell line did not. KP-1N and KP-3, produced liver metastases after intrasplenic injection into nude mice, whereas KP-2 produced few liver colonies. Cell lines highly metastatic to the liver, KP-1NLs and KP-3Ls, were isolated from the liver colonies of KP-1N and KP-3, respectively.

Demonstration of kinin-release in the peritoneal exudate of kaolin-induced writhing in mice.

Using a bradykinin enzyme immunoassay, we measured the amount of kinin in the peritoneal washings of mice with the kaolin-induced writhing reaction. Simultaneous treatment with captopril, a kininase II inhibitor, significantly increased the kinin level at 1 min after kaolin injection. Soybean trypsin inhibitor injected simultaneously with kaolin almost completely suppressed the kinin level at 1 min with or without treatment of captopril. These results suggest that kinin is released through activation of the plasma kallikrein-kinin system by kaolin, and that kinin could be a main mediator for the writhing reaction.

[The reliability of fine needle aspiration cytology in the diagnosis of breast cancer].

Between September 1981 and December 1987, 1023 palpable breast tumors were submitted for cytological evaluation by fine needle aspiration before surgery (carcinoma: 793 cases, benign breast diseases: 230 cases). Clinical factor relating to the success of the aspirations were evaluated. Cases were regarded as positive when patients had a grade IV or more rating by Papanicolaou classification. The sensitivity was shown to be 73% (578/793); the specificity being 98% (226/230). A significantly lower rate of positive cancer identification occurred in smaller tumors (less than 2 cm, P less than 0.01) or scirrhous carcinomas (P less than 0.05). The comparison of positive rates between the aspiration cytology (66%) and the stamp cytology (95%) in 258 breast cancers (P less than 0.01) indicates the limited value of cytological diagnosis of breast cancers by fine needle aspiration.

[A case report of combined composite valve graft replacement of the aortic root and graft replacement of the abdominal aorta].

A 55-year-old man who had both aortic root aneurysm with severe aortic regurgitation and enlarging abdominal aortic aneurysm was admitted to our hospital. We employed the combined composite valve graft replacement of the aortic root and Y-graft replacement of the infrarenal abdominal aorta at one stage operation. At the operation, median sternotomy was made and the composite graft replacement of the aortic root was performed under cardiopulmonary bypass prior to the abdominal procedure. After cardiopulmonary bypass was removed, abdominal incision was made. Although ventricular arrhythmias and mild hypotension transiently occurred when the abdominal aorta was clamped, the Y-graft replacement was also completed uneventfully, and no complication occurred postoperatively. We conclude that the combined operation of the aortic root and the abdominal aorta is clinically feasible in certain situation.

[Clinical examination of ceftibuten in acute bronchitis].

Clinical evaluation of ceftibuten (CETB, 7432-S) was performed in 20 patients with acute bronchitis. They were consisted of 10 males and 10 females aged from 20 to 80 years old. CETB was given orally in daily dose of 300 mg (18 cases) or 600 mg (2 cases) in three divided portions. The duration of administration was 3 to 14 days. Especially they were given for 7 days in 16 cases. A total of 11 strains comprising 4 strains of Staphylococcus aureus, 2 strains of beta-Streptococcus and 1 strain each of Streptococcus pneumoniae, Branhamella catarrhalis, Klebsiella oxytoca, Serratia marcescens, Acinetobacter lwoffii were identified from sputa before administration. All of the above bacteria were eradicated but, in 1 case, a strain of Streptococcus pyogenes appeared after the treatment (eradication ratio = 100%). The clinical efficacy rate was 100%: Responses were excellent in 3 cases and good in 17 cases. There was no side effect and no abnormal changes in laboratory test results. From the avobe results, it is concluded that CETB is effective, safe and useful new oral cephem on acute bronchitis.

[Operative results of distal aortic arch aneurysms--approaching methods, bypass techniques and complications].

Ten patients underwent repair of aneurysms of the distal aortic arch from 1985 to 1989. There were 8 men and 2 women: aged 58 to 77 (average age 67 years). Seven patients had sacciform aneurysms which were closed by graft patch aortoplasty, and three patients had fusiform aneurysms which were corrected by inserting tube grafts. Seven aneurysms operated since 1988 were approached through median sternotomy continued with left anterior thoracotomy, so called door open method. This approach presented good view of the diseased aorta, and effective for preventing recurrent and phrenic nerve palsy. We used temporary bypass for 4 patients, cardiopulmonary bypass for 4 patients (separate carotid artery perfusion for 2 patients) and centrifugal pump for 2 patients during aortic cross clamping. One patient died intraoperatively from intractable bleeding and two patients died postoperatively from brain damage due to embolic episodes during the operations. These patients showed the severely irregular intima in the aortic arch and were complicated with rupture of the aneurysm or dissections arising from the aneurysms. It should be noticed that careless manipulation of the aortic arch and the brachiocephalic vessels cause cerebral complications in such cases.

Functionally active high molecular weight-kininogen was found in the liver, but not in the plasma of brown Norway Katholiek rat.

Plasma and liver levels of high molecular weight (HMW-) kininogen were assessed by a newly developed radioimmunoassay in B/N-Katholiek strain rats, which is congenitally deficient in plasma HMW- and low molecular weight (LMW-) kininogens. The plasma level of immuno-reactive HMW-kiniogen in this strain was about 4% of that of the normal strain, B/N-Kitasato, whereas its level in B/N-Katholiek liver was approximately 60% of that in normal strain liver. There was no significant difference of half-life of HMW-kininogen in the circulating blood between the deficient and normal rats. When secretion of HMW-kiniogen from the liver cells of the two strains was examined by primary culture of their hepatocytes, the hepatocytes from the deficient strain did not secrete HMW-kininogen in the medium. HMW-kininogens were isolated from the liver microsomal fractions of both strains by use of an immuno-affinity column. The isolated protein from B/N-Katholiek liver showed similar mobility on SDS-PAGE to that from normal rat liver, and had biological activities of HMW-kininogen purified from normal rat plasma, such as intrinsic blood clotting cofactor, thiol-proteinase inhibitor, and kinin precursor. These results indicate that the plasma deficiency in the B/N-Katholiek strain is due to a defect of HMW-kininogen secretion from the liver.
