"Health Watch": the Australian petroleum industry health study surveillance programme.

A surveillance system, capable of linking experience in specific jobs with certain health outcomes, has been established within the Australian petroleum industry. A defined population of 11 596 employees has been assembled, after personal interviews. Events recorded are death and its cause, and diagnosis of cancer. Members of the study population are followed up after leaving the industry. By the end of 1983, 33 deaths had occurred, fewer than would be expected from the prevailing rates in the general Australian population. With progressive accumulation of follow-up data, questions of greater complexity will be addressed.

Micromanipulation of sheep morulae to produce monozygotic twins.

Donor ewes were treated with an intravaginal sponge containing 30 mg fluorogestone acetate (FGA) which was removed 12 days later. On the mornings of days 10, 11, and 12, each animal received 30 mg horse anterior pituitary (HAP) extract. Donors were mated twice daily during oestrus. Egg recovery was attemped seven days after progestagen with-drawal, when late morula/early blastocysts could be expected. The zona pellucida was opened with the aid of a micromanipulator, and the embryo was removed by gentle positive pressure from an evacuating pipette. After removal from the zona pellucida, the embryo was bisected using a fine glass needle; each demi-embryo was immediately placed either into the original zona pellucida or into one from an evacuated oocyte. The original opening was closed and both halves were transferred bilaterally into previously synchronized recipients. From 17 embryos split and transferred to 17 ewes, nine became pregnant and produced seven sets of monozygotic twins and two singletons.

Immunolocalization of neuroblastoma using radiolabeled monoclonal antibody UJ13A.

The monoclonal antibody UJ13A, raised after immunization of mice with human fetal brain, recognized an antigen expressed on human neuroblastoma cell lines and fresh tumors. Antibody was purified and radiolabeled with iodine isotopes using chloramine-T. In preclinical studies, 125I-labeled UJ13A was injected intravenously into nude mice bearing xenografts of human neuroblastoma. Radiolabeled UJ13A uptake by the tumors was four to 23 times greater than that by blood. In control animals, injected with a similar quantity of a monoclonal antibody known not to bind to neuroblastoma cells in vitro (FD44), there was no selective tumor uptake. Nine patients with histologically confirmed neuroblastoma each received 100 to 300 micrograms UJ13A radiolabeled with 1 to 2.8 mCi 123I or 131I. Sixteen positive sites were visible on gamma scans 1 to 7 days after injection: 15 were primary or secondary tumor sites, and one was a false positive; there were two false negatives. In two of the 15 positive sites, tumor had not been demonstrated by other imaging techniques; these were later confirmed as areas of malignant infiltration. No toxicity was encountered.

Radiolabelled leucocytes: a new diagnostic tool in occult infection/inflammation.

Autologous white blood cells were labelled with Indium 111 (111In wbc) and reinjected into 18 children with suspected infection and 10 children with suspected inflammatory bowel disease aged 5 weeks to 15 years. In addition two immunosuppressed children with suspected infection received donor 111In wbc. Gamma camera images were obtained between one and four hours and 20 and 24 hours. Other appropriate investigations were also undertaken in these children. Of 18 positive 111In wbc scans 17 were true positive. There were 12 negative scans of which 9 were true negative and three were false negative. The overall results showed a sensitivity of 85%, a specificity of 90%, and an accuracy of 86%. Used appropriately this technique forms a valuable back up investigation.

Comparison of aortography, renal vein renin sampling, radionuclide scans, ultrasound and the IVU in the investigation of childhood renovascular hypertension.

The results of the investigations of all patients who underwent renal arteriography for hypertension due to renovascular disease over a three and a quarter year period prior to January 1st 1981 are reviewed and discussed. The experience gained has helped demonstrate the usefulness of DMSA scans, segmental renal vein renin estimations, and AP and oblique selective renal arteriograms with macroradiography.

Renal parenchymal determination and analysis during dynamic 99Tcm-DTPA scans in children.

Twenty-six children underwent a total of 75 99TCm-DTPA scans. Two experienced operators, using different techniques were able to identify parenchyme in 77 of 111 kidneys. Parenchymal regions of interest were drawn by an entirely operator dependent and a relatively operator-independent technique. Transit time analysis, using a matrix algorithm, through the parenchyme of both normal and abnormal kidneys showed a high degree of correlation (tau = 0.595, P less than 0.001). The technique concurred in the studies for which the analysis could not be applied, generally due to poor technical factors or gross hydronephrosis with insufficient parenchyme to generate a time activity curve. Either technique may be used in clinical practice to identify renal parenchyme for further analysis.

Postural effects on gas exchange in infants.

In adults with unilateral lung disease, pulmonary gas exchange is better when the patients is positioned with the good lung dependent. We studied the effects of body position on gas exchange in 10 infants with unilateral lung disease by measuring transcutaneous oxygen and carbon dioxide pressures in the supine and right and left lateral positions. We also performed krypton lung scans and measured changes in thoracic gas volumes in four of the infants. Transcutaneous oxygen pressure (mean +/- S.E.) was greater with the good lung uppermost (82 +/- 7.6 mm Hg) than with the good lung dependent (73 +/- 7 mm Hg) (P less than 0.02) or in the supine position (78 +/- 7 mm Hg). There were no changes in transcutaneous carbon dioxide pressure. The proportion of ventilation to the good lung was greater with the good lung uppermost than with the good lung dependent (P less than 0.01) or in the supine position (P less than 0.02) (64 +/- 3, 46 +/- 6, and 59 +/- 7 per cent, respectively). There were no significant changes with position in functional residual capacity, tidal volume, or dynamic lung compliance. We conclude that oxygenation in infants with unilateral lung disease is best with the good lung uppermost--the reverse of what has been observed in adults.

Mouse interferon receptors: a difference in their response to alpha and beta interferons.

The interferon receptors of C3H/10T1/2 mouse cells respond differently to alpha and beta interferons under certain conditions. If C3H/10T1/2 cells which have been maintained in logarithmic growth phase are exposed to trypsin or Pronase immediately before they are treated with mouse interferons, they evince an antiviral response to alpha interferon but not to beta interferon. In contrast, contact-inhibited C3H/10T1/2 cells, L-929 mouse cells or human HEL cells lost the ability to respond to both alpha and beta interferons after treatment with trypsin or Pronase. When L-929 cells are incubated at 37 degrees C following exposure to these proteolytic enzymes, they completely regain their ability to respond to mouse beta interferon within 2 h. These observations suggest that the receptors for alpha and beta interferons are different in their topographical distribution in C3H/10T1/2 cells.

Tibial dysplasia. A study of the anatomy.

Three amputated legs with tibial dysplasia were studied by radiography, arteriography and anatomical dissection. The radiographic appearances were the same as the Type 1b tibial dysplasia described by Jones, Barnes and Lloyd-Roberts (1978) in that the tibiae were absent but the lower femoral epiphyses were normal. However, our anatomical findings differed from those of Jones et al. since no bony or cartilaginous anlage of the proximal tibia was found in any of the three legs. The pattern of vascular anomaly was identical in the three legs and similar to the findings of Hootnick et al. (1980) in congenital short fibula. Congenital fusion of the subtalar joint was a constant finding. These results support the hypothesis that the arterial and skeletal systems are vulnerable to a teratogenic insult in the fifth week of embryonic life. The bony and arterial anomalies should be borne in mind by the surgeon attempting reconstructive surgery for this condition.

Effect of repeated horse anterior pituitary extract treatment on ovulatory response in the ewe.

Twenty-five oestrus synchronised ewes were subjected to superovulation treatment with horse anterior pituitary extract, and 23 of these were similarly treated between two and six months later. Seven of these 23 ewes were treated a third time, approximately four months after the second treatment. Ovulatory response was low after the second treatment within two months, but it was satisfactory if a longer period of time was allowed to elapse. Recovery rates of eggs decreased with successive treatments, but fertilisation rates were high. Results of the present experiment demonstrate that ewes can be successfully superovulated three times within a six month period; if a period greater than two months is allowed between successive treatments, high ovulatory responses can be obtained.

The analysis of motivation-specific migration streams.

"This paper investigates the usefulness in migration analysis of disaggregation according to motives, constraints, and/or search procedures. A two-stream gravity model distinguishing between 'long-distance' and 'short-distance' migrational types is developed and applied to the analysis of intercounty flows in Great Britain, by means of a distance cut-off algorithm. The results confirm the distinctiveness and importance of these two streams which account for almost all the variance in flows at the intercounty level. The two streams are shown to respond differentially to environmental, housing, and employment factors. The two polarised types of movement cannot be equated simply with particular motives, however, but reflect basic decisions about search strategy."

Pseudomonas infection, allergy, and cystic fibrosis.

The clinical significance of the high prevalence of positive immediate skin tests in cystic fibrosis is unclear. Using analysis of variance, we have tested the hypothesis that patients with allergic cystic fibrosis have worse lung disease than non-allergic patients. Clinical data, skin prick tests, total or specific IgE antibody levels, chest radiographs, and pulmonary function tests were obtained in 104 cystic fibrosis patients. Patients with positive immediate skin reactions to at least one allergen were more likely to be persistently colonised by Pseudomonas aeruginosa than skin test negative patients. The skin test positive patients were also significantly older (mean difference 2.15 years). Analysis of variance showed that pseudomonas infection was the most significant factor contributing to lung damage and the effect of allergy was not significant. Similar longitudinal analysis of pulmonary function over 5 years and study of the hospital admission rate showed that the only statistically significant factor associated with deterioration was colonisation with P. aeruginosa.
