Acrylic bone cements with bismuth salicylate: Behavior in simulated physiological conditions.

In a previous work, we reported the development of acrylic bone cement formulations for application in percutaneous vertebroplasty, by using bismuth salicylate (BS) as the radiopaque agent. Our objective was to obtain high radiopacity along with a therapeutical effect produced by the release of salicylic acid in situ. To follow that study, the setting kinetics and static and dynamical mechanical properties of the BS cements were studied in simulated physiological conditions. Moreover, radiopacity after various times of immersion in saline and the wettability of the cements surfaces were determined. The study finished with the analysis of the biological response. From the results, it can be concluded that physiological conditions did not affect negatively to the cements performance, since all BS-loaded cements fulfilled the ISO standard requirements. Radiopacity of the formulations was maintained over time and cements with BS were found to be biocompatible.

The intake of dietary indigestible fraction in the Spanish diet shows the limitations of dietary fibre data for nutritional studies.

OBJECTIVE: To compare the intakes of dietary fibre (DF) and dietary indigestible fraction (DIF) in the Spanish diet and also to show the limitations of DF values for nutritional and epidemiological studies. DESIGN: This includes the following: (i) estimation of plant foods consumption in Spanish diet from national food consumption data obtained from annual surveys (6000 households, 700 hotels and restaurants and 200 institutions); (ii) determination of DIF content in plant foods using analytical methods that mimic physiological conditions; (iii) estimation of daily intakes of DIF and DF in the Spanish diet. RESULTS: DIF intake in the Spanish population was estimated at 41.5 g/person/day. DF intake (18.3 g/person/day) was considerably lower than the amount of carbohydrate necessary to maintain the daily bacterial cell turnover in large intestine ('carbohydrate gap'). The differences between DIF and DF values are a consequence of conceptual and methodological aspects. DIF, as the part of foods reaching the colon, comprises not only DF but also other indigestible compounds such as a fraction of resistant starch, protein, polyphenols and other associated compounds. Analytical conditions are closer to actual physiological conditions in the determination of DIF than in DF analysis. CONCLUSIONS: DF intake underestimates a major part of the dietary substrates that enter the colon. DIF intake more closely matches the amount of substrates needed to maintain a typical human colonic microflora. DIF may be a good alternative to DF for nutritional and epidemiological studies.

Wear behaviour of the pair Ti-6Al-4V-UHMWPE of acrylic bone cements containing different radiopaque agents.

The objective of this study was to improve the wear behaviour of acrylic bone cements by substituting the conventional inorganic radiopaque agents (BaSO(4), ZrO(2)) for different iodinated radiopaque monomers which can co-polymerize with the methyl methacrylate monomer, MMA. To this aim, the wear behaviour of the pair Ti-6Al-4V-UHMWPE (ultra high molecular weight polyethylene) was studied in the absence and in the presence of cement particles (the third body).

Propagation of fatigue cracks in acrylic bone cements containing different radiopaque agents.

In this work three iodine-containing monomers were proposed as new radiopaque agents for acrylic bone cements. In previous studies the addition of iodine-containing methacrylate monomers provided a statistically significant increase in tensile stress, fracture toughness and ductility, with respect to the barium sulphate (BaSO4)-containing cement. However, since fatigue resistance is one of the main properties required to ensure a good long-term performance of permanent prostheses, it is important to compare the fatigue properties of these new bone cement formulations with the radiolucent and BaSO4-containing bone cements. Because the acrylic cements have initial cracks, fatigue crack propagation studies were performed. It can be observed that these acrylic cements followed the Paris-Erdogan model. The results showed that the addition of some organic radiopacifiers (DISMA, TIBMA) increased the fatigue crack propagation resistance as compared to the radiolucent cement, being similar to the BaSO4-containing cement. The radiolucent cement showed a low crack propagation resistance.

A radiopaque polymeric matrix for acrylic bone cements.

As part of the search for an alternative to inorganic radiopaque agents, this work studies the possibility of modifying bone cement formulations by incorporating a radiopaque monomer, that is, 4-iodophenol methacrylate (IPMA), in the liquid phase. The monomer was synthesized in the laboratory, and cements were prepared by the standard method. The influence on the different cement characteristics of various monomer concentrations was studied. It was seen that the setting time decreased as the percentage of monomer increased. The radiopacity attained in the 15 vol.% IPMA formulations was about the same as that for a cement containing 10 wt.% barium sulphate. Dynamic and static mechanical properties were measured. The materials did not show significant differences in the glass transition temperature. However, static mechanical properties showed enhanced compressive strength, tensile strength, and elastic modulus with respect to conventional cements formulated with barium sulphate. Histological studies showed a good response of muscular tissue to implanted specimens.

Elimination of barium sulphate from acrylic bone cements. Use of two iodine-containing monomers.

The aim of this work is to present a new approach to joint arthroplasty failure related to bone cement mantle. As barium sulphate is considered one of the main causes of mechanical weakness in the cement, we substituted this inorganic radiopacifier of the solid component for radiopaque monomers in the liquid component. We obtained two different cements, one containing 5 vol% 2-[2',3',5'-triiodobenzoyl] ethyl methacrylate (TIBMA) and the other containing 3,5-diiodine salicylic methacrylate (DISMA). In both cases, the mechanical properties of these new cements were better than those of the barium sulphate-containing cement. The radiopacity obtained was comparable to that of the aforementioned cement and all the samples showed good biocompatibility.

Capacity of edible seaweeds to modify in vitro starch digestibility of wheat bread.

The capacity of two species of edible seaweeds (Wakame, Undaria pinnatifida and Chondrus, Chondrus crispus) to modify the rate of white bread starch digestibility by an in vitro digestion system, as well as glucose retardation index and apparent viscosity were studied. Both Algae showed different effect on glucose retardation index. While Wakame did not make difficult dialysis of glucose, it only retained 2.50 +/- 2.99% to respect a negative control. Chondrus impaired the diffusion of a 28.70 +/- 2.35% of glucose, which are very close to those of citrus pectin (31.50 +/- 4.12%). On the other hand, viscosity of Chondrus solution was higher than Wakame and slightly lower than citrus pectin solution. The analysis showed a very low content of total starch in Wakame (0.51%) and Chondrus (0.47%). Algae reduced the digestion of white bread starch. The profile of starch hydrolysis was characteristic for each alga. Glycaemic index estimated from the degree of starch hydrolysis within 90 min was low (79) with respect to white bread (value 100). Seaweeds showed a suitable capacity to modify in vitro starch digestibility of white bread. Chondrus produces a more pronounced response. This fact seem to be due to different composition of samples.

Synthesis of hydroxypropyl methacrylate/polysaccharide graft copolymers as matrices for controlled release tablets.

Hydrophilic matrices are an interesting option when developing drug delivery systems. With this aim, hydroxypropyl methacrylate was grafted onto hydroxypropyl starch and hydroxypropyl cellulose substrates by following the Ce(IV) redox initiation method. Different amounts of ethyleneglycol' dimethacrylate, 7 and 34 mol%, as the crosslinking monomer, were also added. The drying of grafted products was carried out by lyophilization, obtaining white powders. Reaction yields (percent grafting, grafting efficiency, etc.) and some physical characteristics of the powders (particle size, moisture uptake, density, morphology, etc.) were determined. These parameters indicate how useful these products may be as potential matrices for direct compressed tablets. In this light, the powder flowability and the binding properties of each copolymer were determined. The graft copolymers can be considered of great interest as direct compression excipients. Due to their different chemical structure and composition, they showed differences in viscoelastic properties that revealed an interesting range of possibilities for use in drug delivery formulations. Tablets formulated with conventional excipients were also tested. Dissolution tests of various tablets were carried out. In 12 hr, 60-80% of the model drugs was released.

Dietary modulation of bacterial fermentative capacity by edible seaweeds in rats.

The effect of edible seaweeds [nori (Porphyra tenera) and wakame (Undaria pinnatifida)] on the modulation of colonic microbiota was studied in adult male Wistar rats. Each alga was fed to rats as the only source of dietary fiber and compared with cellulose. After 12 days, animals were sacrificed and cecal contents used as inoculum to ferment lactulose, citrus pectin, cellulose, nori, and wakame in vitro. Dietary treatment did not affect food intake or food efficiency, yet alga caused a significant increase in cecal weight. Nori and wakame were poorly fermented by the cellulose inoculum, with intermediate substrate degradation (76 and 57% for nori and wakame, respectively) and low metabolism to short-chain fatty acids (SCFA) (30% fermentability compared with lactulose). Cecal contents from rats fed nori and wakame showed a reduced ability to ferment all of the studied substrates compared with the cellulose inoculum, causing a reduction in SCFA production and dry matter disappearance. Only nori induced a bacterial adaptation that brought about a higher fermentation of this substrate. The different behaviors of the two algae could be due to their distinct chemical compositions. In conclusion, nondigestible components of edible seaweeds modified the metabolic activity of intestinal microflora, leading to a reduction of its fermentative capacity.

[Human milk oligosaccharides. The rule in the health and development of the infants].

Breast-feeding is the optimal mode of feeding for the normal full-term infant. Human milk composition knowledge has been basis for recommended dietary allowances for infants. Few studies about human milk carbohydrates have been done until the last decade. However, carbohydrates provide approximately 40-50% of the total energy content of breast milk. Quantitatively oligosaccharides are the third largest solute in human milk after lactose and fat. Each individual oligosaccharide is based on a variable combination of glucose, galactose, sialic acid, fucose and N-acetylglucosamine with many and varied linkages between them, thus accounting for the enormous number of different oligosaccharides in human milk. The oligosaccharides content in human milk varies with the duration of lactation, diurnally and with the genetic makeup of the mother. At present, a great interest in the roles of human milk oligosaccharides is raising. They act as a the soluble fibre in breast milk and their structure is available to act as competitive ligands protecting the breast-fed infant from pathogens and act as well as prebiotic. They may also act as source of sialic acid and galactose, essential for brain development. This is why today there is an increasing health and industrial interest in human milk oligosaccharides content, with the main purpose of incorporating them as new ingredients in infant nutrition.

Effect of processing on potato starch: in vitro availability and glycaemic index.

The content of digestible starch (DS) and resistant starch (RS) in processed potatoes was assessed. In addition, the effect of domestic cooking on the in vitro digestibility of starch in this tuber, which may influence the glycaemic response, was studied. Resistant starch in raw potato is high, however different RS values were obtained when processed, ranging from 1.18% in boiled potatoes to 10.38% in retrograded flour. In general, cooked potatoes have high levels of DS. Starch digestibility is improved after processing and it is affected by the cooking methods. Boiled and mashed potatoes showed the highest rate of digestion, on the contrary raw potato was hardly digested. The estimated Glycaemic Index (GI) from the degree of starch hydrolysis within 90 min was in accordance with the reported GI values, for potatoes processed in the same way.

In vitro determination of the indigestible fraction in foods: an alternative to dietary fiber analysis.

Dietary fiber (DF) intakes in Western countries only accounts for about one-third of the substrates required for colonic bacterial cell turnover. There is a general trend among nutritionists to extend the DF concept to include all food constituents reaching the colon. In this line, a method to quantify the major nondigestible components in plant foods, namely, the indigestible fraction (IF), is presented. Analytical conditions for IF determination are close to physiological. Samples, analyzed as eaten, were successively incubated with pepsin and alpha-amylase; after centrifugation and dialysis, insoluble and soluble IFs were obtained. IF values include DF, resistant starch, resistant protein, and other associated compounds. IF contents determined in common foods (cereals, legumes, vegetables, and fruits) were higher than DF contents. Calculated IF intakes were close to the estimated amount of substrates reaching the colon. IF data could be more useful than DF data from a nutritional point of view; therefore, IF is proposed as an alternative to DF for food labeling and food composition tables.

Characterization of new acrylic bone cement based on methyl methacrylate/1-hydroxypropyl methacrylate monomer.

New formulations of acrylic bone cement based on methyl methacrylate/1-hydroxypropyl methacrylate (MMA/HPMA) monomers were developed with the purpose of obtaining more ductile materials with reduced polymerization shrinkage. In this way, the ductility of such materials increased, but the introduction of high percentages of the hydrophilic component produced an important decrease in Young's modulus and strength. To ascertain the reason for the deterioration of the tensile parameters, an analysis by scanning electron microscopy of these formulations was carried out; it revealed poor adhesion between the matrix and poly(MMA) beads. We also observed that the polymerization shrinkage increased as the amount of hydrophilic monomer in the formulation decreased, and the 50% (v/v) HPMA modified bone cement compensated for this volume reduction with its water uptake swelling. Measurements taken on the setting time and polymerization exotherm showed a decrease in the former and an increase in the latter, because of the introduction of a more reactive monomer in the bone cement formulation.

Modified acrylic bone cement with high amounts of ethoxytriethyleneglycol methacrylate.

One cause of arthroplasty failure is the brittle mechanical behavior of bone cements. However, the improvement of cement formulations must also be accompanied by the maintenance of a wide variety of characteristics. New bone cements were obtained by the substitution of high percentages, up to 60% (v/v), of methyl methacrylate (MMA) by a higher molecular weight and more hydrophilic monomer, ethoxytriethyleneglycol methacrylate (TEG). The essential advantages of these materials were the decrease of maximum temperature together with a decrease in the residual monomer content with respect to conventional cement formulations. The water absorption process obeyed diffusion laws and the equilibrium water content increased by the introduction of higher percentages of the hydrophilic component. This characteristic had an appreciable effect on the viscoelastic behavior analyzed by DMTA. These modified bone cements had reduced polymerization shrinkage and similar levels of porosity. Tensile test revealed that the introduction of TEGMA gave rise to an important modification of the mechanical behavior, with a noticeable increase in the fracture strain. This fact was also confirmed by means of the analysis of the fracture surfaces by SEM.

Influence of the modification of P/L ratio on a new formulation of acrylic bone cement.

The use of smaller powder/liquid (P/L) ratio favours the handling and wetting of poly(methyl methacrylate) (PMMA) beads in bone cement formulations. In this paper a P/L ratio of 1.86 is tested to overcome adhesion problems found in hydroxypropyl methacrylate (HPMA) modified bone cements and the influence on bone cement characteristics was analysed. The reduction of the P/L ratio leads to higher temperature peaks and shorter setting times, whereas the residual monomer content increases slightly. Water uptake obeys the diffusion laws, and the introduction of a more hydrophilic monomer gives rise to an increase of this parameter, which does not present significant changes with modification of the P/L ratio. Polymerization shrinkage is slightly greater because of the introduction of higher proportions of monomer in the formulation. Mechanical properties are similar to those obtained with conventional P/L ratios. The analysis of scanning electron microscopy (SEM) reveals an improvement of the adhesion between phases with respect to P/L = 2 formulations.

Synthetic PMMA-grafted polysaccharides as hydrophilic matrix for controlled-release forms.

This paper describes the rheological behavior of starch and cellulose acrylic graft copolymers synthesized with the aim of obtaining controlled-release excipients. The rheological characteristics determine the final release properties of matrix tablets. The study of the storage and loss moduli (G' and G", respectively) and the viscosity allowed us to know if the polymer behavior was that of a gel and, hence, if it could act as a barrier to drug diffusion. Since dynamic measurements showed a storage modulus higher than the loss modulus, we assessed that all the polymers were gels. Thus, knowing that all the graft copolymers had acceptable properties for compression, the release of theophylline as a model drug at different pH was studied. Polymers with higher absorption capacity, viscosity, and compactibility allowed formulations with slower release rates.

[Immunohistochemical tumor markers in infiltrating ductal breast carcinoma].

BACKGROUND: Immunohistochemical techniques allow the study of tumoral markers with predictive or prognostic value. AIM: To study tumoral markers in mammary carcinoma and to relate these markers with age and tumoral histological grade. MATERIAL AND METHODS: The expression of nuclear estrogen receptor (RE), nuclear progesterone receptor (PGR), p53 and c-erb-B2 proteins was analyzed in 382 cases of infiltrating ductal carcinoma of the breast. RESULTS: The histological grade was I in 22% tumors, II in 56% and III in 23%. Forty nine percent of tumors had positive staining for RE, 41% for RPG, 45% for p53 and 57% for c-erb-B2. Correlation was found between histological grade and expression of RE, p53 and c-erb-B2 between RE and PGR, RE and c-erb-B2 and between c-erb-B2 and p5. RE expression correlated with age. CONCLUSIONS: A reliable and rapid evaluation of these markers can be achieved using immunohistochemical staining. The present results are similar to other biochemical and immunohistochemical studies.

Application of tertiary amines with reduced toxicity to the curing process of acrylic bone cements.

4-Dimethylaminobenzyl alcohol (DMOH) and 4-dimethylaminobenzyl methacrylate (DMMO) were used as the activators in the benzoyl peroxide initiated redox polymerization for the preparation of acrylic bone cement based on poly(methylmethacrylate) beads of different particle size. The residual monomer content of the cured cements was about 2 wt %, independent of the redox system used in the polymerization, indicating that the activating effect of the tertiary aromatic amines DMOH or DMMO was sufficient to reach a polymerization conversion similar to that obtained with the benzoyl peroxide (BPO) N,N-dimethyl-4-toluidine (DMT) system. The BPO/DMOH and BPO/DMMO redox systems provided exotherms of decreasing peak temperature and increasing setting time, and the cured materials presented higher average molecular weight and similar glass transition temperatures in comparison with those obtained when DMT was used as the activator. In addition, these activators are three times less toxic than the classical DMT.
