Day-long reduction of oral malodor by a two-phase oil:water mouthrinse as compared to chlorhexidine and placebo rinses.

Few scientific investigations have addressed the ability of mouthrinses to reduce oral malodor for periods longer than 3 hours. In the present report, we have employed simple, recently described techniques to assess the day-long reduction in oral malodor of a novel 2-phase oil:water mouthrinse (TPM), as compared to a corresponding placebo rinse, and to a commercial 0.2% chlorhexidine mouthrinse. Sixty dental students were divided randomly into 3 groups, and instructed to use one of the rinses prior to bedtime and the following morning. Measurements carried out in the late afternoon, about 8 to 10 hours following rinsing, were compared with baseline measurements carried out in the late afternoon of the previous day. Volatile sulphide levels were measured using a portable industrial sulphide monitor. Microbial levels were estimated using a simple rinsing technique employing sterilized milk. These quantitative techniques were corroborated by organoleptic (hedonic) ratings of a single odor judge. Both TPM and chlorhexidine brought about significant decreases in volatile sulphides (P less than 0.05) as compared to the placebo group. These results were corroborated by the organoleptic data. Similarly, both chlorhexidine and TPM were highly effective in reducing microbial levels as measured by the rinsing technique, in comparison to the placebo group. Chlorhexidine appeared to be more effective than TPM in all measurement categories, although only in the case of microbial activity was there a significant (P less than 0.05) difference between the two groups.

Halitosis measurement by an industrial sulphide monitor.

Previous studies have established that hydrogen sulphide and mercaptans are the primary components of halitosis (bad breath). In the present investigation, we report a simple, rapid technique for measurement of halitosis-related sulphides. The technique is based on a portable instrument generally used for environmental safety applications. Seventy-five volunteers were measured using this technique, and the results (in peak ppb hydrogen sulphide equivalents) compared with organoleptic assessment by 7 judges. A highly significant overall correlation (r = 0.603; P less than 0.001) was obtained between these 2 methods. Moreover, in most cases, the organoleptic ratings of the individual judges correlated more highly with sulphide monitor values than with one another. The simplicity of the technique suggests its use in clinical studies as well as in diagnosis and treatment of patients with this complaint.

[Prognostic factors in multiple myeloma].

There have been major advances in the treatment of multiple myeloma in the past 20 years, but for the individual patient the prognosis still remains uncertain. As the length of survival varies from several months to over 10 years, definition of prognostic parameters at the time of diagnosis, and early detection of disease activity are most important. In our study, median survival was 42 months with very good quality of life. Factors not helpful in prognosis were sex, WBC and platelet counts, BUN, serum M protein type, extent of osteolytic lesions, percentage of plasma cells in bone marrow and plasma cell asynchrony. However, age, hemoglobin, calcium, uric acid, Bence-Jones proteinuria and polyclonal Ig concentrations had a certain degree of prognostic importance. Due to more sensitive and more specific laboratory methods, peripheral blood findings are lately gaining in importance. With new "salvage" protocols, the detection of additional prognostic parameters and sensitive indicators of disease activity may be most important for further improvement in the survival of patients with multiple myeloma.

Blood-spotted filter paper measurements of thyroxine and thyroid-stimulating hormone in the follow-up of children with congenital hypothyroidism.

Neonatal screening for congenital hypothyroidism using T4 and TSH measurements from blood-spotted filter paper is a well-established method. However, it has not been used to monitor T4 and TSH concentrations in the follow-up of these children. In 22 treated children with congenital hypothyroidism whom we follow up in our Clinic, T4 and TSH concentrations were concomitantly measured from venous blood and blood-spotted filter paper. There was a significant positive correlation between both T4 and TSH measurements from venous blood versus blood-spotted filter paper (r = 0.7, p = 0.001; r = 0.78, p less than 0.05). Filter paper T4 values above 7 micrograms/dl could exclude hypothyroxinemia in 98% of the specimens. When TSH values were above 10 microU/ml, it was confirmed in 94% of the specimens, and when they were above 20 microU/ml, it was confirmed in 97% of the specimens. Measurements of both filter paper T4 and filter paper TSH did not increase the reliability of the results obtained by examining the two hormones separately. We therefore suggest that filter paper T4 and/or TSH measurements have distinct advantages in monitoring the treatment of children with congenital hypothyroidism. It can be performed in the community, enabling assistance in the follow-up of children in remote areas who are unable to show up for serum tests. The results are obtained quickly and allow improved follow-up by providing useful information such as excluding hypothyroxinemia or suggesting the possibility of noncompliance, and by a psychological effect on parents. However, they cannot replace serum T4 and TSH measurements altogether.

MRI parameters in multiple sclerosis patients.

Magnetic resonance imaging (MRI) findings of 20 patients with clinically definite multiple sclerosis (MS) are presented. The studies were performed on a 0.5 Tesla magnet using spin-echo technique. Analysis of the MRI findings included detailed linear measurements of the ventricular and the subarachnoid spaces and reading of the intensity of the grey and white matter and intensity of the MS plaques. The plaques were sorted according to their number and size. The younger patients (20-40 years) had overall more plaques than the older ones (over 40 years). The small plaques were the most numerous and the large ones were the least common. Statistically significant association was found between the number of plaques and the cella media width. The intensity ratios between the non-plaque white matter/grey matter showed a significant correlation with the ventricular score. A significant negative correlation was found between the antero-posterior diameter of the spinal cord and the number of MS plaques in the brain. The plaque/white matter ratio had a significantly negative correlation with the cervical cord's width.

Influence of oral contraceptive treatment on blood pressure and 24-hour urinary catecholamine excretion in smoking as compared with non-smoking women.

Microgynon-30 was administered to 15 smoking (group B) and 14 non-smoking young women (group A) for a period of 4 months. Prior to the institution of treatment supine blood pressure and daily urinary catecholamine excretion (DUCE) were measured in both groups of patients. Both values were elevated in group B in comparison with group A. Treatment with Microgynon-30 for 4 months induced a significant increase in DUCE in both groups, the increase being most marked in group B (p less than 0.0004). The increased catecholamine excretion accompanied small non-significant elevations in blood pressure. It is concluded that treatment with low-dosage estrogen oral contraceptives produces a significant increase in daily catecholamine excretion, especially in patients who smoke.

Frequency of carriers of chronic (type I) Gaucher disease in Ashkenazi Jews.

In this study we estimate the frequency of carriers of chronic (type I) Gaucher disease among Ashkenazi Jews by examining the glucocerebrosidase activity in leukocytes in a population of 635 blood donors (441 Ashkenazi) and 57 obligatory heterozygotes. Estimation using the defect in the enzyme glucocerebrosidase (beta-glucosidase) in leukocytes is complicated by the existence of considerable overlap between enzyme activity in normals and in heterozygotes. The assay was carried out with a natural substrate labeled with 14C. Discriminant analysis was used to establish an optimal cutoff point between the obligatory heterozygotes and normal (non-Ashkenazi) subjects for the purpose of estimating frequency of carriers. Applied to the Ashkenazi group, the cutoff point identified 3.17% as heterozygotes. Corrected for errors in classification, the carrier rate was estimated as 4.67%. This figure is in good agreement with a carrier rate of 4% estimated from the number of known cases of clinical Gaucher disease ascertained in Israel.

Morbidity and recurrence rates after surgical treatment of malignant melanoma by scalpel versus CO2 laser beam.

A retrospective study was carried out on 219 patients who underwent surgical treatment of a malignant melanoma. A scalpel was used in 96 patients in group 1 and CO2 laser beam was used in 123 patients in group 2. The average length of hospitalization for group 2 was longer (16.3 versus 12.8 days for group 1). This was due to failure of the skin graft; 32.5 per cent in group 2 versus 15.6 per cent in group 1 (p = 0.005). The accumulative rate of recurrence for both groups was almost the same although there were significant differences according to the various parameters. Male patients in group 2 had a significantly higher rate of recurrence as compared with female patients in the same group (p less than 0.001) and male patients in group 1 (p = 0.002). In both groups, there was a significantly higher rate of recurrence for ulcerated primary lesions and those lesions more than 1.6 millimeters thick (p = 0.05). Patients in group 2, with lesions more than 3 millimeters in thickness, had a higher rate of recurrence than those in group 1 (54.6 versus 40.6 per cent). In both groups, patients who underwent elective regional dissection of lymph nodes had a lower rate of recurrence (19.4 per cent) than those patients who did not undergo dissection (53.6 per cent) (p = 0.001). It is suggested that thermal damage to the blood and lymph vessels incurred during laser excision may be more extensive than has been reported. These damaged walls may cause the higher rate of distal metastases of malignant melanoma from a primary lesion more than 1.6 millimeters in thickness; primarily in male patients.

Studies on the level of natural antibodies reactive with various tumor cells during urethane carcinogenesis in BALB/c mice.

Serum from young normal BALB/c mice was found to contain IgM antibodies able to mediate complement-dependent lysis of certain syngeneic or allogeneic tumor target cells. The titer of such naturally occurring antitumor antibodies ( NATA ) was found to increase with aging. A longitudinal serological study comparing the cytotoxicity potential of NATA from normal and from urethan-treated BALB/c mice was performed. It was found that urethan-treated mice that did not develop primary lung-adenomas within the duration of the experiment had significantly lower NATA titers, against one out of 4 target cells assayed, than urethan-treated animals that developed lung adenomas. This difference was evident in two independent experiments. The results suggested that the lower NATA activity of the urethan-treated mice that did not develop tumors existed even before exposure to the carcinogenic insult. This raises the possibility that certain populations could be segregated according to their natural antibody profile into those individuals which will develop primary tumors within a certain period if exposed to a subthreshold amount of carcinogen, and those which will not.

Glycohemoglobin (Hb A1) levels in oral contraceptive users.

In a group of 33 healthy women, among whom 15 utilized the combined oral contraceptive pill, and 18 utilized the progesterone oral pill (ethynodiol diacetate, 0.5 mg, Femulen, we have examined the percentage of glycohemoglobin (Hb A1). In the progesterone oral pill users the percentage of Hb A1 remained unchanged, that is 5.4% before, and 5.2% 3-6 mth after using it, whereas in women taking the combined oral contraceptive pill the values increased from 5.9% before to a mean of 8.7% after 3-6 mth after administration. This increase seems to be related to the estrogen of the which, as in pregnancy, may induce glucose intolerance. From this study it may be seen that, despite its disadvantages such as reduced effectiveness and menstrual disruption, the progesterone-only pill is preferable. At the same time, the screening by the Hb A1 evaluation, a sure, inexpensive and easily performed procedure, makes it possible to detect a nonsuspected diabetes or a borderline diabetes and thus to change the contraception method used for another method.

Effect of oral contraception by progesterone pill on lipids and glucose metabolism.

The influence of oral contraception with 0.5 mg ethynodiol diacetate (Femulen) on blood lipid and glucose tolerance test (GTT) was studied in a group of 14 nulliparous women ages 17-24 years before and 3 months after treatment. In another group of 13 women of matched age, contraception was managed by providing a combined pill (estrogen and progesterone). The continuous treatment of ethynodiol diacetate did not result in the increase in serum cholesterol and triglyceride levels and did not affect the GTT, as observed when the combined pill was administered. Femulen had no side effects, except for 2 cases of vaginal spotting. Ethynodiol diacetate does not induce the decrease in high-density lipoprotein cholesterol levels which is an anti-risk factor for the prevention of accelerated atherosclerotic disease. Oral contraception with this pill is indicated for all women, especially when there is an absolute contraindication for the use of combined pills

Capacitance in ant cuticle is frequency dependent: a statistical model.

The capacitance and the electric resistance of the cuticle of the ant Cataglyphis bicolor nigra Andr é (Hymenoptera, Formicinae ) were measured. The measurements were done at the frequencies 100 Hz and 1000 Hz and at a temperature range of 27.5-45 degrees C. Inverse correlation was observed between the capacitance and the frequency, so that at the higher frequency the capacitance was lower. Thus, in some instances, at 1000 Hz the capacitance ranged between 0.49 and 2. 16nF , while at 100 Hz it ranged between 5.74 and 19. 39nF at the measured temperature range. A similar inverse correlation was detected also between the resistance and the frequency. At 1000 Hz, the resistance values in some specimens ranged between 0.166 and 0.278 M omega whereas at 100 Hz they varied between 0.342 and 0.883 M omega. At both frequencies measured there was a temperature-dependence of the capacitance and of the electric resistance. With increase in temperature there was increase in the capacitance and a decrease in the resistance. Invariably, the trend of cuticular behavior was similar under cooling as under warming, but the values differed, creating a gap (hysteresis). Under cooling the resistance values were higher and the reverse was true for the capacitance. A statistical model is offered which graphically describes the behavior of the ant cuticle (resistance and capacitance) under changes in temperature and frequency. X-ray analysis of the cuticle revealed the presence of Ca as the most prominent element. Additional elements, but less prominent were P, S and K. It also contains Fe and Zn. The finding of a correlation between the capacitance and the frequency might lead to the following conclusions: that the measured system contains polarized substances; that the measured value in each case represents a resultant of values obtained from the measurement of more than one electrical circle (and probably more than one network of electrical circles); and that possibly a combination of (a) and (b) prevails. Presumably these changes in the capacitance and resistance at different temperatures and frequencies indicate that the ant cuticle is capable of responding to changes in the physical ambience , thus promoting proper ant spatial orientation and the pursuant behavior.

Healing of dehiscence defects at delayed-immediate implant sites primarily closed by a rotated palatal flap following extraction.

In 21 patients, 28 maxillary teeth were extracted because of periapical or periodontal infection, root fracture, or untreatable caries. A rotated palatal flap procedure was used to achieve primary soft tissue closure over extraction sites. At 5 to 7 weeks postextraction, 28 implants were placed. Buccal dehiscence-type defects were treated with guided bone regeneration procedures using bovine bone mineral and resorbable collagen membranes. Mean defect area at the time of implant placement (23.7 mm2, SD 11.49) was significantly reduced at uncovering (0.7 mm2, SD 0.99). The mean percentage of defect reduction (clinical bone fill) was 97% (SD 4.26). Implants placed in compromised sites shortly postextraction according to the presented 2-stage protocol gave good short-term clinical results.

Clinical coverage of dehiscence defects in immediate implant procedures: three surgical modalities to achieve primary soft tissue closure.

In 61 patients, 61 consecutive implants were placed immediately after extraction of one anterior or premolar maxillary tooth. One of 3 surgical approaches based on rotated full (RPF) or rotated split (RSPF) palatal flaps, with and without the use of barrier membranes to enable primary soft tissue closure, was applied. A bovine bone mineral graft was used in all cases. At the time of implant placement, the distance between the most apicobuccal alveolar crestal bone and the coronal aspect of the implant body was measured; this was measured again at second-stage surgery. All implants appeared clinically stable. The buccal crestal bone gain was statistically significant for all groups (RPF = 2 mm, RSPF = 1.6 mm, RSPF with membrane = 3.7 mm) (P < .001). Analysis of covariance showed a significant covariant for preoperative measurements; however, this was not significant between groups.

Lymphocyte subpopulation before and after operation in patients with benign vs malignant breast disease.

Multiple parameters of immune function were measured serially before and one and five weeks following operation in 14 patients with fibrocystic disease of the breast (Group A) and in 20 patients with stage 1-2 infiltrating duct carcinoma (Group B). These parameters included the following: WBC, total number and percentage of lymphocytes, numbers of B cells, T cells, T-active, T-helper and T-suppressor cells and the ratio between the latter as well as spontaneous suppressor or helper activity and the graft-versus-host reaction. Prior to operation no statistically significant difference was found between the two groups except for the number of T-helper cells, which was higher in Group B (p less than 0.05), and the spontaneous suppressor activity, which was higher in Group B (p less than 0.05). The finding of such a high percentage (80%) of negative graft-versus-host reactions five weeks after operation together with the high suppressor activity may indicate the presence of tumor micrometastases. The burden of surgery and general anesthesia was stronger in Group B, with a pronounced difference found between the groups (p = 0.0005), but the interaction between the influence of time (surgery and anesthesia) and the groups was not as great (p = 0.4864) and was found to be different for each group.
