Tc-99m colloid lung uptake in a rare case of toxoplasmosis with liver involvement.

Intensive lung accumulation of colloid (Tc-99m phytate) was demonstrated in a child suffering from acquired toxoplasmosis with a rare manifestation of severe liver damage. The possible mechanism and clinical importance of colloid lung concentration in this case is briefly discussed, including a review of the literature on this subject.

The noninvasive diagnosis of isolated right ventricular infarction.

A rare case of isolated right ventricular infarction (RVI) is described. The lack of clinical and electrocardiographic findings characteristic of isolated RVI makes this pathology a diagnostic challenge. The role of the radionuclide scintigraphic procedures as a single possible tool for the diagnosis of isolated RVI is emphasized.

The use of gallium-67 liver imaging for the early diagnosis of Budd-Chiari syndrome.

A gallium-67 study, performed in a rare case of Budd-Chiari syndrome (BCS) in a 6-yr-old child, showed marked concentration of the tracer in the left and caudate lobes of the liver. Ten days later, a less specific and clear-cut appearance of left- and caudate-lobe concentration of Tc-99m phytate was obtained. The possible use of Ga-67 citrate imaging to diagnose BCS in its earliest stage is suggested, although no firm conclusion can be made from this single case.

Uncommon sites of bone infarction in a sickle cell anemia patient.

Unusual sites of bone infarction, in the skull and sternum, were observed in a patient suffering from sickle cell anemia. A 99mTc-MDP scan was performed and demonstrated foci of decreased activity in the symptomatic regions. The differentiation of bone infarction from osteomyelitis in sickle cell anemia patients is illustrated.

Effects of prolonged stress on coping style in terminal renal failure patients.

Fifty-nine patients with terminal renal failure and 59 comparison subjects matched on age, sex, origin, education and marital status were administered the Shanan Sentence Completion Test at the onset of hemodialysis, to test the hypothesis that 1) prolonged stress reduces the tendency to cope actively and that 2) the extent of reduction would vary according to the patients' background. Findings provided massive support for the first hypothesis; in nearly all aspects of coping style investigated, hemodialysis patients obtained significantly lower scores, indicative of passivity, negative self-perception and of tendencies to withdraw by denial from the harsh reality. Findings on the second hypothesis were equivocal. Only sex and education showed interaction with illness and coping. Women appeared to be more vulnerable than men. The effects of education on coping were reduced as a consequence of the illness. Findings are discussed in terms of their implications for stress research and for the advancement of preventive measures in hemodialysis treatment.

Advisability of surgical treatment for chronic membranous obstruction of the hepatic portion of the inferior vena cava.

Transatrial membranotomy was performed in a patient with membranous obstruction of the hepatic segment of the inferior vena cava, manifesting as longstanding varicosity and edema of the legs, chronic type of Budd-Chiari syndrome, liver cirrhosis and hypersplenism. The course in this patient and similar ones reported in the literature suggests a conservative approach.

Coping behavior and intelligence in the prediction of vocational rehabilitation of dialysis patients.

As part of an intensive psychiatric and psychological examination before starting hemodialysis, forty-seven patients with terminal renal failure, thirty-one men and sixteen women with an age range from the late teens to the mid fifties, had been administered with WBII and the Shanan Sentence Completion technique. The hypothesis was that coping style would predict adaptation to hemodialysis as assessed by vocational rehabilitation. Intelligence, level of education, and the tendency to perceive sources of conflict in the outer world rather than within oneself, were found predictive of subseuqent adaptations for the whole group. On all these variables, men and women showed different patterns of significant correlations between the different aspects of coping and vocational rehabilitation. Subsequent multiple step-wise regression showed women (percentage of explained variance: 75) to be more predictable than men (40%) with different variables contributing differentially to predictability. These findings, interpreted to support the general working hypothesis, are discussed in terms of their implications for treatment.
