[Diagnosis and treatment of diaphragm traumatic ruptures].

29 patients with traumatic ruptures of left cupola of the diaphragm were studied. Pain, dispnoe, tachycardia more intensive after a meal due to repletion and dislocation of the stomach into pleural cavity and its pressure on the organs of the mediastinum were the main symptoms of the disease. The diagnosis of traumatic rupture of the diaphragm was made on the base of clinical and roentgenological examination. In the acute period of trauma the diagnosis is difficult, especially in combined trauma. In 92.3% of cases the stomach and the bowel move into the pleural cavity, which may simulate pneumothorax or relaxation of the diaphragm. Contrast examination of the gastro-intestinal tract and pleural puncture help in differential diagnosis. All the cases of traumatic ruptures of the diaphragm should be subjected to surgical treatment. Urgent operation is indicated in continuing bleeding, incarceration of organs, acute cardiopulmonary insufficiency due to the collapse of the lung and dislocation of mediastinal organs of into contralateral position. Thoracotomy in VII intercostal space is considered as a valid approach. There were 2 (6.9%) lethal outcomes after 29 operations. Long-term results are quite favourable.

[Selection of therapeutic tactics for nonspecific spontaneous hemopneumothorax].

The results of treatment have been analyzed for 45 patients with nonspecific spontaneous hemopneumothorax. Therapeutic policy was decided upon individually basing on the patient's condition and clinical appearance of the disease. Urgent surgery was performed in 68.0% of patients for continuous intrapleural bleeding and tense pneumothorax. The rest cases underwent pleural puncture. Pleural drainage failed to eliminate spontaneous hemopneumothorax in all the patients. The causes of SHP were established mostly intraoperatively. In 81.8% of the cases it resulted from a rupture of a bullous lung. Long-term follow-up (1-18 years) of 28 patients treated surgically and conservatively recorded no recurrences.

[Echinococcosis of the diaphragm].

According to the authors, echinococcosis of the diaphragm accounts for 2.24% of all hydatid lesions of the thoracic and abdominal organs. It is pointed out that preoperative diagnosis of concurrent and secondary echinococcosis of the diaphragm is difficult despite the performance of pneumoperitoneum and scanning of the liver. Ten of 12 patients were operated on for echinococcosis of the liver or right lung. One-staged closed echinococcectomy with removal of the residual cavity is the operation of choice in a single echinococcosis of the diaphragm or uncomplicated concurrent echinococcosis. Excision of the dome together with the cyst is recommended for suppurative echinococcosis and irreversible changes of the diaphragm.

[Diagnosis and treatment of giant air cysts of the lung].

Giant air cysts of the lung were found in 6 of 180 patients admitted to the hospital with spontaneous pneumothorax. The preoperative diagnosis was established in all the patients on the basis of their past history, clinical course, X-ray examination and puncture of the pleural cavity. The operative treatment of cysts of the lung is recommended. All the 6 patients were operated upon with good results (in 3 patients--lobectomy, cysts were removed in 3 patients).

[Intrathoracic complications of hepatic echinococcosis].

Five clinical forms of intrathoracic complications of hepatic Echinococcosis were distinguished on the basis of examination of 115 patients with hepatic Echinococcosis. They were: bilio-bronchial, hepato-pleural, hepato-pleuro-bronchial, hepato-pulmonary and hepato-mediastinal form found in 15 patients (13%). The intrathoracic complications must be treated surgically, cases with bilio-bronchial complications requiring emergent operative interventions. Thoracodiaphragmatomy is thought to be the optimal access.
